Objective:In order to evaluate the clinical effect of bloodletting therapy on patients with spastic wrist dorsiflexion dysfunction after stroke,we observed the changes of related symptoms and surface electromyography signal before and after treatment,and sought a more efficient and simple treatment method for wrist dorsiflexion functional reconstruction after stroke.Methods:In this study,a total of 70 patients with spastic wrist dorsiflexion after stroke were collected,and randomly divided into the control group(35 cases)and the experimental group(35 cases)according to the patients’ visiting sequence.All patients received basic treatment(routine medical treatment,rehabilitation treatment).The control group received acupuncture treatment on the basis of basic treatment,30 minutes each time,once a day,5 times a week,for 4 weeks.The experimental group was treated with bloodletting on the basis of basic treatment,once every other day,three times a week,for 4 weeks.The modified Ashworth Scale(MAS),passive joint range of motion(PROM),Fugl Meyer assessment(FMA)and integrated electromyography(s EMG)were compared before and after treatment Finally,all the data were analyzed by spss22.0 software.Results:1.Modified Ashworth rating scale: after treatment,the MAS rating of the two groups were lower than before(P<0.05),and the improvement of spasticity in the experimental group was better than that in the control group(P<0.05).2.Passive range of motion measurement: Prom of the two groups increased after treatment(P<0.05),and the experimental group was better than the control group in improving passive wrist extension(P<0.05).3.Short form Fugl Meyer motor function scale score(upper limb part): after treatment,the FMA score of the upper limb in the two groups were higher than before(P<0.05),and the experimental group was better than the control group in improving the motor function of the upper limb(P<0.05).4.Detection of the integral electromyography value of the flexor carpi: the i EMG value of the two groups decreased after treatment(P<0.05),and the experimental group was better than the control group in reducing i EMG(P<0.05).5.Root mean square value of flexor carpi: the RMS value of the two groups decreased after treatment(P<0.05),and the RMS value of the experimental group was better than that of the control group(P<0.05).6.Clinical efficacy: the total effective rate of the control group was 80.65%,the total effective rate of the experimental group was 87.50%,and the effect of the experimental group was better than that of the control group(P<0.05).Conclusion:1.Both bloodletting therapy and conventional acupuncture can improve the spasticity of wrist flexor muscle in patients with spastic wrist dorsiflexion dysfunction after stroke,reduce the abnormal contraction of muscle,and improve the passive range of motion of wrist joint and limb motor function.2.The clinical effect of bloodletting therapy in improving spastic wrist dorsiflexion after stroke is better than that of conventional acupuncture.
