Objectives:1.To observe the characteristics of population biological indexes, glucose and lipid metabolism, diabetes complications and complications in patients with type 2 diabetes combined with gastrointestinal symptoms and without gastrointestinal symptoms.2.The distribution of disease location and syndrome elements in patients with type 2diabetes mellitus complicated with gastrointestinal symptoms,as well as the correlation between them and metabolic indexes were obtained.Methods:1.Source of research subjects: A cross-sectional study was conducted to collect type 2diabetes patients who were admitted to the Department of Endocrinology of Xiamen Hospital of Traditional Chinese Medicine from January 2020 to December 2020,confirmed the diagnosis of T2 DM,agreed to cooperate in data collection, and met the inclusion criteria;2.The four diagnostic information and related data collection: adopt by fujian university of traditional Chinese medicine of traditional Chinese medicine from the research base and home-made general table of the four diagnostic information collection data collection form to TCM four diagnostic information of 283 patients, and demographic data and glucolipid metabolism index of the collection, use of TCM syndrome factor dialectical system input data, draw the corresponding syndrome factor points, according to the TCM syndrome factor score standard(syndrome factor score of 70 or higher)concluded that patients with type 2 diabetes mellitus and gastrointestinal symptoms of primary disease, disease syndrome factor distribution.3.Statistical analysis: SPSS20.0 software was used to analyze the correlation between the main disease sites, disease syndrome elements and glucose and lipid metabolism indexes;Statistical methods mainly included descriptive analysis,t-test of two independent samples,chi-square test, dichloristic unconditional multivariate Logistic regression analysis,etc.P<0.05 was considered as significant difference.Results:1.The incidence of gastrointestinal symptoms: Up to 79.2% of T2 DM patients have gastrointestinal symptoms, and lower gastrointestinal symptoms are more common; From the incidence of the disease, the most common symptoms are sparse stools, increased exhaust, and reduced defecation. From the degree of severity,to exhaust increased, reduced defecation, increased defecation and so on the most significant.2.General situation:(1)There was a significant difference in the incidence of gastrointestinal symptoms in male and female subjects, and the incidence in female subjects was significantly higher than that in male subjects, with a statistical difference(P > 0.05).(2)Compared with the group without gastrointestinal symptoms, there were no significant differences in age,BMI,abdominal circumference, course of disease, blood pressure, tobacco and alcohol history, and treatment methods in the group with gastrointestinal symptoms(P> 0.05).3.Glycemic and lipid metabolism indicators: Compared with the group without gastrointestinal symptoms,Hb A1 c in the group with gastrointestinal symptoms was higher than that in the group without gastrointestinal symptoms, with statistical significance(P <0.05),and the score of gastrointestinal symptoms showed an increasing trend with the increase of Hb A1 c,with statistical significance(P < 0.05).FPG,2HPG,TG,TC,HDL,LDL,had no significant changes, and there was no statistical difference(P < 0.05).4.Distribution of disease sites and syndrome elements in T2 DM patients with gastrointestinal symptoms:(1)The frequency of disease location syndrome from high to low was spleen, liver, large intestine, stomach, kidney, mind and so on.Compared with the patients without gastrointestinal symptoms group, the proportion of spleen, large intestine, stomach, small intestine, surface and gallbladder syndrome elements in the patients with gastrointestinal symptoms group was increased, and the proportion of spleen, large intestine and stomach syndrome elements was statistically significant(P < 0.05).(2)The frequency of syndromes from high to low was Qi deficiency, dampness,Yin deficiency,Qi stagnation, blood deficiency, phlegm,Yang deficiency, etc.Compared with the patients without gastrointestinal symptoms group, the proportion of qi deficiency syndrome elements, dampness syndrome elements,Yin deficiency syndrome elements,qi stagnation syndrome elements, blood deficiency syndrome elements, heat syndrome elements, jin deficiency syndrome elements, dryness syndrome elements and cold syndrome elements in the patients with gastrointestinal symptoms group was increased, among which the qi deficiency syndrome elements,qi stagnation syndrome elements and blood deficiency syndrome elements had statistical significance(P < 0.05).(3)Analysis of related factors of the two groups of syndromes:For patients with gastrointestinal symptoms, the indexes related to heparin were: FPG,HOMA-IR;The indexes related to colonic syndromes were Hb A1 c.The index related to Qi deficiency syndrome element is FPG.For patients without gastrointestinal symptoms, the index related to spleen syndrome was Hb A1c;The index related to wet syndrome element is TC.Conclusions:1.There were significant differences in the incidence of gastrointestinal symptoms and GSRS score between male and female diabetes patients, with women significantly higher than men,and the difference was statistically significant. The main symptoms of lower digestive tract were sparse stools, increased exhaust and reduced defecation.2.For patients with type 2 diabetes,Hb A1 c and HOMA-IR were higher in patients with gastrointestinal symptoms than in patients without gastrointestinal symptoms, and GSRS score increased with the increase of Hb A1 c.3.The main disease loci of type 2 diabetes mellitus complicated with gastrointestinal symptoms in Xiamen area: spleen, liver, large intestine and stomach; The number and composition ratio of syndrome elements of spleen, large intestine and stomach were higher than those of asymptomatic group. The main syndromes: qi deficiency, dampness,Yin deficiency,qi stagnation and blood deficiency, among which the number and composition ratio of qi deficiency,qi stagnation and blood deficiency were higher than those of the asymptomatic group.4.In type 2 diabetes patients with gastrointestinal symptoms, heparin is related to fasting blood glucose and insulin resistance, and colonic syndrome is related to hemoglobin A1c;Spleen syndromes were associated with Hb A1 c and wet syndromes were associated with total cholesterol in type 2 diabetes patients without gastrointestinal symptoms.
