| Objective:To explore the clinical,endoscopic,pathological characteristics and related factors of cardiac polyps,and to provide guidance in the diagnosis and treatment of cardiac polyps.Methods:From January 1,2016 to December 31,2020,564 patients with cardiac polyps were diagnosed by endoscopy.The relationship between sex,age,BMI,GERD-Q score,microscopic manifestationsyears,accompaned diseases,pathologic types and Hp infection rate were recorded,and statistical analysis was conducted.Results : 1.Cardiac polyps were 0.39% of the totle endoscopy number,and male vs female was 1.37.Patients with cardiac polyps were highest in the middle age group(47.16%).Male in youth group were significantly more than other groups(P <0.001).2.Cardiac polyps are more common in the dentate line and in single,0-Is type and≤0.5cm.3.69.15% of cardiac polyps patients were GERD-Q score ≥8,and 35.45% of patients have RE.Patients with RE in youth group(51.03%)were significantly more than the other groups(P < 0.001).Patients with RE in inflammatory polyps(39.50%)were more than other groups(P =0.001).18.44% of cardiac polyps patients were infected Hp.4.Pathological types of cardiac polyps included inflammatory polyps,hypertrophic polyps,fundus gastric polyps and neoplastic polyps,which inflammatory polyps were the most common(56.56%)..FGPs were mainly in female,and the others were mainly in male(P < 0.001).The mean age of patients with neoplastic polyps were significantly older than other groups(P < 0.001),and the size was highest in ≥1.0cm(76.47%).5.Logistic regression analysis on related factors of IPs and HPs.Male(P=0.011,OR=1.64,95%CI:1.12-2.39)and RE(P=0.039,OR=1.51,95%CI:1.02-2.23)were risk factors for IPs.The middle age group(P=0.008,OR=1.96,95%CI:1.19-3.23)was a risk factor for HPs.Conclusion:The male with cardiac polyps are more than female.Cardiac polyps are mainly single,0-Is type and ≤ 0.5cm.IP were the most common pathological type.Patients with neoplastic polyps in elderly group were more than other groups.Most patients with cardiac polyps have reflux symptoms,Patients with RE were significantly more than normal people.Patients with Hp were significantly lower than normal people.Male and RE are risk factors for IP.Objective:To study the value of blue laser imaging in pathological diagnosis of cardiac polyps.Methods:78 patients with cardiac polyps diagnosed by BLI endoscopy in the past 5years.Mucosal patterns were classified into four categories: small round,prolonged,villous or ridge,and unclear patterns,and micro vascular patterns were classified into five categories: honey comb,dense vascular,fine net work,core vascular,and unclear patterns.According to endoscopic features of the cardiac polyp by ME-BLI,its properties were determined,and compared with pathological diagnosis,counted the coincidence rate of endoscopic diagnosis and pathological.And the sensitivity and specificity of endoscopic signs in the diagnosis of different types of polyps.Results:1.Patients with cardiac polyps have the coincidence rate between endoscopic diagnosis and pathology was 88.16%.2.The micro structures for predicting IP was prolonged(sensitivity 89.29%,specificity79.17%),and the micro vascular was fine net work(sensitivity 75.00%,specificity87.50%).The micro structures for predicting FGP was small round(sensitivity 90.00%,specificity 93.94%),and the micro vascular was honey comb(sensitivity 80.00%,specificity 98.48%).The combined patterns for predicting neoplastic polyps were villous or ridge+ core vascular and unclear+unclear(sensitivity 100.00%,specificity 96.92%).Conclusion:The micro structures and micro vascular of cardiac polyps by using magnifying BLI have closely related to histopathology,and provides meaningful information for predicting the histopathology of cardiac polyps. |
