Clinical Observation Of Therapeutic Effects Of Yishen Huoxue Formula Combined With L-Carnitine On Astheno-Teratozoospermia Manifesting Kidney Deficiency And Blood Stasis Syndrome

ObjectiveTo compare the effects of Yishen Huoxue Formula combined with levocarnitine,LCarnitine and Yishen Huoxue Formula on semen parameters,sperm DNA fragment index and sex hormone level of idiopathic Astheno-teratozoospermia manifesting kidney deficiency and blood stasis syndrome,to observe the advantages and disadvantages of Yishen Huoxue Formula combined with L-Carnitine in the treatment of idiopathic Asthenoteratozoospermia manifesting kidney deficiency and blood stasis syndrome.MethodsCollecting cases from reproductive medicine department and andrology department of the First Affiliated Hospital of Guangzhou University of Chinese Medicine from August,2020 to December,2020,a total of 90 patients with idiopathic Astheno-teratozoospermia manifesting kidney deficiency and blood stasis syndrome were included.Excluding loss of follow-up and shedding cases,82 cases finally were included,27 cases in combination group,27 cases in Chinese medicine control group,28 case in western medicine control group.Patients in Chinese medicine control group were given the oral administration of Yishen Huoxue Formula in the way of twice a day in one dose a day for 12 weeks,the patients in western medicine control group were given 1g L-Carnitine oral liquid three times a day for 12 weeks,the patients in combination group were given Yishen Huoxue Formula and L-Carnitine oral liquid at the same time,and Yishen Huoxue Formula was taken first,and L-Carnitine oral liquid was taken half an hour later,the method of taking traditional Chinese medicine and western medicine same as traditional Chinese medicine control group and western medicine control group.The main parameters of sperm quality,sperm DNA fragment index,and the level of follicle-stimulating hormone,luteinizing hormone and testosterone of the patients before and after treatment were statistically analyzed within and between the groups after the 12-week treatment course.Results1.The baseline level of patients in three groups before treatment: There were no statistically significant differences in age,duration of infertility,semen parameters,sperm DNA fragment index,follicle-stimulating hormone(FSH),luteinizing hormone(LH)and testosterone(T)among all groups(P<0.05).2.The sperm concentration,semen volume,progressive motility sperm rate were significantly improved in three groups respectively after 12 weeks of treatment(P<0.05).The sperm DNA fragment index was reduced in three groups respectively after 12 weeks of treatment(P<0.05).The normal sperm morphology rate was significantly improved in combination group and Chinese medicine control group after 12 weeks of treatment(P<0.05).The non-progressive motility sperm rate,and the level of FSH,LH and T were not significantly improved in three groups respectively after 12 weeks of treatment(P>0.05).There were significantly statistical differences in semen volume,progressive motility sperm rate among three groups after 12 weeks of treatment(P<0.05),on the contrary,the sperm concentration,non-progressive motility sperm rate,and the level of FSH,LH and T were not significant improved among three groups after 12 weeks of treatment(P>0.05).There was no statistical difference in efficacy among the three groups(P>0.05).3.In terms of safety,one patient in the combination group and one patient in the traditional Chinese medicine control group developed mild diarrhea during the treatment of first week,and then recovered spontaneously,and no obvious adverse reactions were found in the rest of the patients.ConclusionThe combination of Yishen Huoxue Formula and L-Carnitine can improve sperm quality and lower the sperm DNA fragment index in patients with idiopathic Asthenoteratozoospermia manifesting kidney deficiency and blood stasis syndrome.The combination of Yishen Huoxue Formula and L-Carnitine was preferable to the control groups in improving sperm concentration,semen volume and progressive motility sperm rate,with statistical significance.The combination of traditional Chinese medicine and western medicine exists more advantages in treating idiopathic male infertility.It can be used as one of the treatment methods for male infertility to improve sperm quality and sperm DNA fragment index.
