| Objective:To study the clinical experience and medication rule of traditional Chinese medicine masters in treating arrhythmia through medical record data mining analysis.Methods:using prescriptions written by traditional Chinese medicine masters in the treatment of arrhythmia as the study object,including 122 cases and 242 diagnosis of arrhythmia in total.The contents of medical records collected included general information of patients,diagnosis of Western medicine,Basic disease,complications,cause,clinical symptoms,tongue coating and pulse condition,syndrome elements,traditional Chinese medicine,etc.Created the arrhythmia medical records database of traditional Chinese medicine masters after sorting out the medical records.Using frequency analysis,cluster analysis and association rule analysis of data mining,complex network analysis,this paper discusses the academic thought and medication experience of traditional Chinese medicine masters in treating arrhythmia.Result:1.General information:There were 63 males and 59 females;sex ratio was about 1:1;the average age was 47.1years old.2.Frequency analysisVentricular premature beat was the most common arrhythmia,with a frequency of 56.06%.Ventricular premature beat was the most common arrhythmia,with a frequency of 56.06%.Coronary atherosclerotic heart disease was the most common basic disease,with a frequency of 26.23%.Hypertension was the most common complication,with a frequency of 30.30%.The inducement is mainly emotional and exogenous factors,and the frequency is 38.10%.The inducement is and light red,thin and white tongue coating,the pulse is fine,number,knot,generation and sink.Arrhythmia syndrome elements in different regions are Qi deficiency,blood stasis,yin deficiency and blood deficiency.In different regions,the syndrome elements of arrhythmia are Qi deficiency,blood stasis,yin deficiency and blood deficiency.Qi and yin deficiency are the main syndromes,followed by phlegm turbidity and stasis,and heart and spleen deficiency in two.Drugs were mainly used as tonic drugs,followed by promoting blood circulation and removing blood stasis.The most frequently used traditional Chinese medicine is Ophiopogon japonicus,with a frequency of 57.79%.The use of drugs is cold and warm,and flat and complementary.In the use of medicine flavor,it is mainly sweet,bitter and pungent.In terms of drug meridians,liver meridians are the first in East China,Northeast China,North China and Central South China,lung meridians are the first in Northwest and southwest,and heart,spleen and kidney meridians are the main.3.Association analysisDrug association:a total of 30 high-frequency Chinese medicine pairs were obtained,"Salvia miltiorrhiza and Ophiopogon japonicus〃 have the highest support;and 27 high-frequency three drug combinations were obtained,"Salvia miltiorrhiza,Ophiopogon japonicus and Schisandra chinensis" have the highest support.Three high-frequency four flavor drug combinations were obtained,"Salvia miltiorrhiza,Ophiopogon japonicus,Schisandra chinensis and Astragalus" have the highest support.Western medicine diagnosis was associated with syndrome:Ventricular premature beat and "deficiency of Qi and Yin" have high support.Drug-syndrome association:The Qi Yin deficiency syndrome was associated with high-frequency drugs,"Schisandra chinensis and Salvia miltiorrhiza,drug combination is strongly associated with Qi and yin deficiency syndrome."Sour jujube kernel,Salvia miltiorrhiza,Ophiopogon japonicus","Poria cocos,Salvia miltiorrhiza,Ophiopogon japonicus","Schisandra chinensis,sour jujube kernel and Salvia miltiorrhiza" are strongly associated with Qi and yin deficiency syndrome;The combination of "Astragalus membranaceus,Schisandra chinensis,Ophiopogon japonicus and Salvia miltiorrhiza" has the highest support for deficiency of Qi and Yin.4.Cluster analysisClassified high-frequency Chinese medicines with a frequency of use higher,and obtained 4 types of high-frequency Chinese medicine cluster analysis results.The first kind:Wild jujube kernel,Poria,Cinnamon Twig,Polygala tenuifolia,Poria cocos,Acorus calamus,oyster,keel;The second kind:Angelica sinensis,Radix Paeoniae Alba,radix salviae miltiorrhizae,Radix Polygoni Multiflori rattan,Ligusticum chuanxiong and Radix Paeoniae Rubra;The third kind:Roasted licorice,jujube,Codonopsis pilosula,pseudoginseng,American ginseng,Ophiopogon japonicus,Rehmannia glutinosa,Astragalus membranaceus,Atractylodes macrocephala,Schisandra chinensis;The Fourth kind:Pinellia ternata,Amomum villosum,tangerine peel,Fructus aurantii,Coptis chinensis,Sophora flavescens,turmeric,Trichosanthes,Zhuru,raw licorice;5.Complex network analysisBased on the regional division,six core prescriptions and addition and subtraction compatibility of Chinese medical masters for the treatment of arrhythmia Qi and yin deficiency syndrome are obtained:the core prescriptions in Northeast China are Astragalus membranaceus,Ophiopogon japonicus,Schisandra chinensis,Yuzhu,Rehmannia glutinosa and licorice.The core prescriptions in North China are American ginseng,Ophiopogon japonicus,Schisandra chinensis,Astragalus membranaceus,Salvia miltiorrhiza,Poria cocos and wild jujube kernel.The core prescriptions in East China are:Radix Pseudostellariae,Ophiopogon japonicus,Coptis chinensis,Borneo,Amomum villosum,Sophora flavescens,Salvia miltiorrhiza,oyster and keel.The core prescriptions in Northwest China are:Radix Pseudostellariae,Ophiopogon japonicus,radix salviae miltiorrhizae,Sophora flavescens,Poria cocos,wild jujube kernel,roasted licorice,Trichosanthes,hawthorn,Radix Scrophulariae and deer grass.The core prescriptions in Southwest China are:Salvia miltiorrhiza,Ophiopogon japonicus,Schisandra chinensis,Radix Pseudostellariae,roasted licorice,Rehmannia glutinosa,astragalus,Atractylodes macrocephala and poria cocos.The core prescriptions in central and southern China are:ginseng,Astragalus membranaceus,Ophiopogon japonicus,Schisandra chinensis,Zhuru,orange red,raw licorice,Salvia miltiorrhiza,Cinnamon Twig and crane grass.6.Network pharmacology analysis:It is found that the key components of core drugs for the treatment of arrhythmia are quercetin,which may act on functional pathways such as PI3K-Akt signal pathway,cAMP signal pathway and relaxin signal pathway through targets such as STAT3,mapkl and AKT1,and participate in the regulation of oxidative stress,inflammatory response,blood vessel formation and other mechanisms to regulate arrhythmia.Conclusion1.Chinese medical masters believe that arrhythmia is based on deficiency of both qi and Yin and stagnation of heart pulse.The treatment is based on Supplementing Qi and nourishing yin and promoting blood circulation and removing blood stasis.In terms of treatment,combined with the living and eating habits of local people,geographical and climatic environment,it varies from person to person,adapts measures to local conditions,and treats according to syndrome differentiation.2.On arrhythmia Qi and yin deficiency syndrome,Chinese medical masters in different regions are based on the addition and subtraction of Shengmai Powder to nourish yin and Qi.And each has its own characteristics.Northeast China pays attention to promoting blood circulation and Qi.North China pays attention to promoting blood circulation and resolving phlegm.East China pays attention to promoting blood circulation and removing blood stasis and treating both heart and spleen.Northwest China pays attention to clearing heat and cooling blood.Southwest China pays attention to supplementing qi,nourishing yin and dredging pulse.Central and southern regions attach importance to eliminating phlegm and turbidity.3.Network pharmacological analysis shows that the drug combination of"Salvia miltiorrhiza,Schisandra chinensis,Ophiopogon japonicus and Astragalus membranaceus" can act on diseases through multiple components,multiple targets and multiple ways,which provides a molecular biological basis for the so-called"different treatment of the same disease and the same treatment of different diseases" in traditional Chinese medicine.It can be used as the direction of exploring the mechanism of action in the next step.4.Through data mining technology,this study excavates the potentially useful information and knowledge of traditional Chinese medicine information,which can objectively and comprehensively reflect the experience and academic thought of Chinese medical masters,and complement and confirm each other with the traditional methods of following teachers. |
