| Objective: Understand elderly patients with hypertension stress,loneliness and attachme nt status,clear elderly patients with hypertension loneliness,attachment and resistance,analyze t he influence of resistance factors,explore the influence of loneliness and attachment against r esistance,develop effective intervention measures for clinical nursing staff,improve the patient’s resistance to provide reference materials.Methods: This study is a descriptive study in non-experimental studies.From May to O ctober 2021,320 elderly hypertension patients who met the inclusion and exclusion criteria i n a third-class general hospital in Jilin province were selected as the research subjects,and t he general data questionnaire,loneliness scale,adult attachment scale and stress stress scalew ere used for questionnaire survey.Data collation and analysis were performed through the SP SS26.0 Chinese version statistical software package,including frequency,percentage,meanand standard deviation,t-test,one-way analysis of variance,Pearson correlation analysis andmultiple linear regression analysis,with P<0.05.Results:(1)The stress stress score of elderly hypertensive patients was 38.16±10.23(23~84),which was lower than domestic65.4±13.9(P<0.05).The adverse stress was statistically different indi fferent gender,age,occupation,family per capita monthly income,marital status,educational statu s,disease duration,residence status,type of chronic diseases,type of medical insurance,and majo r economic sources(P<0.05).(2)The loneliness score of elderly patients with hypertension was48.25±11.16(15~66),am ong which50(15.6%),207(64.7%)and63(19.7%),respectively.Loneliness was statistically different in different occupations,blood pressure control status,types of chronic diseases,types of medi cal insurance,and major economic sources(P<0.05).(3)The attachment avoidance score of elderly hypertensive patients was2.78±0.70(1.00~4.75),and the attachment anxiety score was3.49±0.72(1.33~4.50).Attachment avoidance was stat istically different by gender,occupation,marital status,residence status,and major economicori gin(P<0.05).Attachment anxiety was statistically different by gender,age,occupation,maritalstat us,hospitalizations in the last year,disease duration,residence,and major economic sources(P<0.05).(4)Loneliness(r=-0.462,P<0.05);attachment avoidance and attachment anxiety(r=0.415,P<0.05;r=-0.450,P<0.05);loneliness and attachment avoidance and attachment anxiety(r=-0.584,P<0.05;r=0.586,P<0.05).(5)The factors affecting stress in elderly hypertensive patients are gender,age,residence,ch ronic diseases,loneliness and attachment anxiety.The general demographic characteristics expla ined 15.8%(F=4.515,P<0.001),18.6%(F=10.306,P<0.001),5.2%(F=11.446,P<0.001),and the total variation was 39.6%(F=11.446,P<0.001),with the largest interpretation rate of loneliness.Conclusions:(1)Elderly patients with hypertension have low stress resistance and need to be improve d.Women,over 80 years old,unemployed,family per capita monthly income of less than 1000 yuan,divorced,primary school and below,course of disease for 1 year,living alone,living wit h 1 chronic disease,medical insurance for urban and rural residents,income sources for childr en and subsistence allowance and patients have low stress resistance.(2)About 80% of elderly mild and moderate lonely patients account for hypertension,an d medical staff should pay attention to it in clinical care.Farmers,patients with substandardb lood pressure control,two chronic diseases,medical insurance for urban and rural residents,a nd spouses of the main economic origin had a high level of loneliness.(3)Attachment avoidance for elderly hypertensive patients is low in women,enterprises a nd institutions,widowed,cohabitation with children,main economic sources for children and su bsistence relief;attachment anxiety among men,80 and above,farmers,divorce,hospitalization,<1year,living alone and main source of pension.(4)The higher the level of loneliness,the lower the level of stress resistance,the higherth e level of attachment avoidance and the higher the attachment anxiety.(5)The influencing factors of stress stress in elderly hypertensive patients are gender,age,living alone,type of chronic diseases,loneliness and attachment anxiety,among which lonelines s has the largest explanatory rate. |
