| Objective:In this study,through the collation of medical records about stomachache caused by cold pathogen in modern literature,and with the help of modern data mining technology,a self-made prescription was summarized and used in clinical practice.Clinical observation was carried out under the real world research model,providing new ideas and new prescriptions for clinical treatment,and providing methods for the prevention and treatment of stomachache caused by cold pathogen in the warm area troops fighting in the border area of the acute and alpine regions.Methods:1.Literature review : Literature mining data from January 1976 to now published in China National Knowledge Infrastructure(CNKI),Wanfang database,Superstar Reader database of traditional Chinese medicine in the treatment of cold evil stomach type stomach pain existing literature.The classic expositions of various physicians on stomach pain,especially the stomach-type stomach pain caused by cold pathogen were refined,and the therapeutic prescriptions were screened.Data mining was carried out through the "TCM inheritance calculation platform(TCMICS V3.0)" software to analyze the frequency and regularity of medication,and new prescriptions were obtained through cluster analysis,which were added and subtracted for clinical use.2.Clinical observation : 60 patients with stomachache who were treated in the Ninth 60 th Hospital of the PLA of the Joint Logistics Support Force and the outpatient and ward of the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine from December 2020 to December 2021 were selected and included in the research plan.They were treated with the self-made prescription.Based on the real world research model,the effective rate of the self-made prescription was observed by using the self-control research method.The course of treatment was four weeks.Results:1.Literature mining : A total of 103 prescriptions were included in this study,including 123 kinds of drugs,and the total frequency of all kinds of drugs was 893 times.Among them,the traditional Chinese medicine categories with high frequency were qi regulating drugs,Wenli drugs,Buxu drugs,Jiebiao drugs and Huashi drugs.In the four-gas statistics,the use of warm drugs was the most,400 times(44.79 %),followed by 186times(20.83 %)of flat drugs and 179 times(20.04 %)of thermal drugs.The total frequency of five flavors of traditional Chinese medicine was1410 times,and the medicinal amount of pungent flavor was the largest,which was 623 times(44.18 %).The frequency of bitter drugs was 384(27.23 %);the frequency of sweet medicine was 320 times(22.70 %).The total frequency of meridian attribution of 123 drugs was 2676 times,and the top four viscera meridians were spleen meridian,stomach meridian,lung meridian and liver meridian.The frequencies from high to low were742(27.73 %),494(18.46 %),368(13.75 %)and 273(10.20 %),respectively.When the support was set to 15 and the confidence was set to 0.6,16 data were extracted under the "medication mode",including 10 herbs,and 16 data were extracted under the confidence ≥ 0.6 by "rule analysis".Based on the k-means algorithm and regression simulation,the number of clusters is set to be 5.2.Clinical research : finally included 55 cases,the effective rate was 100 %.After the patients took the self-made prescription,the TCM syndrome score,VAS score,and the quantitative standard score of the reported outcome symptoms of epigastric pain-splenogastric disease doctors were significantly improved(P < 0.01),indicating that the self-made prescription had significant indigenous effect on improving the clinical symptoms of patients.Conclusion:The main etiology and pathogenesis of stomachache caused by cold evil invading the spleen and stomach,blocking qi movement,imbalance of gastric ascending and descending,and pain caused by incompetence are the main pathogenesis of stomachache caused by cold evil invading the spleen and stomach.The treatment should be warming the middle and dispersing the cold,and promoting qi to relieve pain.Through literature mining and cluster analysis,the self-made prescription composed of new prescription addition and subtraction is used in clinical practice,which has obvious effect on improving symptoms of patients with stomachache and cold evil. |
