Objective To observe the clinical efficacy of electroacupuncture in the treatment of Neck type Cervical Spondylosis and its effect on the Surface Electromyography(i EMG)of upper trapezius muscle,analyze the relationship between the difference of bilateral upper trapezius muscle tension and the symptoms of Neck Type Cervical Spondylosis(NTCS),so as to provide clinical evidence for electroacupuncture in the treatment of NTCS.Methods68 patients with NTCS meeting the standard were randomly assigned to the treatment group and the control group,with 34 samples in each group.Due to the problems of case shedding and elimination,63 cases were actually completed,including 32 cases in the treatment group and 31 cases in the control group.Both groups were given daily life guidance,and i EMG of bilateral upper trapezius muscles were measured before the first treatment.According to the detection results of surface electromyography before the first treatment,the control group selected C4-C6 Cervical Jiaji Points,Tianzhu(BL10),Bilateral Houxi(SI3),Shenmai(BL62)and Xuanzhong(GB39)on the side with high i EMG score for routine acupuncture treatment.The treatment group,which was on the basis of the control group,added electroacupuncture dense wave at C4 and C6 Cervical Jiaji Points on this side.Both groups were treated for 30 minutes,once every other day,three times a week,and three weeks as a course of treatment.The curative effect was determined after one course of treatment.The changes of the Northwick Park neck pain questionnaire(NPQ)table,Short-Form Mc Gill Pain Questionnaire(SF-MPQ)table and i EMG of upper trapezius were observed before and after treatment.Results SPSS25.0was used for statistics,and P < 0.05 was considered statistically significant.Results1.Both groups were effective after treatment.The total effective rate of the treatment group was 93.75% and the total effective rate of the control group was 83.87%.The curative effect of the treatment group was better than that of the control group(P<0.05).2.The scores of NPQ in both groups decreased significantly after treatment(P<0.01).The treatment group was significantly better than the control group in reducing NPQ scores(P<0.01).3.PRI,VAS,PPI scores and the total SF-MPQ scores in the two groups after treatment were significantly lower than those before treatment(P<0.01).The treatment group was significantly better than the control group in reducing PRI,VAS,PPI scores and the total scores of SF-MPQ(P<0.01).4.Compared with before treatment,both groups could significantly reduce the i EMG difference of bilateral upper trapezius muscle(P<0.01).And the treatment group was significantly better than the control group(P<0.01).After treatment,there was no significant difference in i EMG value of bilateral upper trapezius muscle in the treatment group(P>0.05),but there was still significant difference in i EMG value of bilateral upper trapezius muscle in the control group(P<0.01).5.The correlation analysis between i EMG difference of bilateral upper trapezius muscle before treatment and NPQ score before treatment showed that there was a significant positive correlation between them(P<0.01).In addition,there was a significant positive correlation between the changes of bilateral i EMG difference before and after treatment and the changes of NPQ score(P<0.01).Conclusions1.Both electroacupuncture and conventional acupuncture can improve the pain state and quality of life of NTCS patients,and electroacupuncture is significantly better than conventional acupuncture.2.Both electroacupuncture and conventional acupuncture can adjust the muscle tension of neck muscles and improve the state of "dynamic imbalance",and electroacupuncture is significantly better than conventional acupuncture.3.The i EMG difference of bilateral superior trapezius muscle can reflect the severity of symptoms of patients with cervical spondylosis to a certain extent.There is a positive correlation between them.The greater the i EMG difference,the more serious the symptoms of NTCS patients. |
