A Clinnical Study Of Self-gravity Curvature Traction Combined With Exercise Therapy To Treat The Neck Type Of Cervical Spondylosis

Objective:To evaluate the efficacy and safety of the self-gravity curvature traction combined with exercise therapy that treated neck type of cervical spondylosis and look for a new solution which is simple and effective among the clinical treatment of the disease,it compared the differences of treatment neck type of cervical spondylosis in improving cervical physiological curvature and neck muscle function between the self-gravity curvature traction combined with exercise therapy and conventional traction combined with exercise therapy through clinical randomized controlled study.Methods:86 subjects who met the inclusion criteria for neck type of cervical spondylosis were divided into experimental group (43 cases) and control group (43 cases)in accordance with the principle of randomization,experimental group was given the therapy of self-gravity curvature traction combined with exercise therapy,control group was treated with the therapy of conventional traction combined with exercise therapy;ten days was taken as a theraputic course, All the patients got two courses with two-day rest between each course. Before the treatment,after one course, after two course, Short-Form McGill Pain Questionnaire(SF-MPQ),cervical physiological curvature by X-ray measurement, integrated electromyography, median frequency and other targets were tested respectively to observe the change of the two groups in pretherapy and post-treatment.Results:1. Clinical efficacy:Two kinds of solution for treating the neck type of cervical spondylosis have a certain clinical curative effect and the total effective rate was no significant difference, but self-gravity curvature traction combined with exercise therapy is superior to conventional traction combined with exercise therapy in the aspect of clinical cure rate (P<0.05).2. SF-MPQ score:intra-group comparison,SF-MPQ score had significant difference among the two groups in pretherapy and post-treatment (P<0.01);Comparison between groups, two groups had no significant difference in the pain score after the treatment of first course(P> 0.05), but the treatment group was better than control group after the second course of treatment,and the difference had statistically significant (P<0.01).3. The physiological curvature of cervical vertebra through X-ray measurement: intra-group comparison, two groups all had improved in terms of cervical physiological curvature compared with pre-treatment, the difference was statistically significant (P<0.01);Comparison between groups,there was significant difference among two groups (P<0.01), the treatment group was better than the control group.4. Integrated electromyography(iEMG):intra-group comparison,the integrated electromyography of trapezius muscle and sternocleidomastoid of two groups had difference with statistically significant at different stages(P<0.01);Comparison between groups, after first course of treatment, the integrated electromyography of trapezius muscle and sternocleidomastoid of two groups had no statistical significance(P>0.05);after second course of treatment, the integrated electromyography of trapezius muscle and sternocleidomastoid of two groups were statistically significant (P<0.05),and the experimental group was better than the control group.5. The median frequency(MF):intra-group comparison,the median frequency of trapezius muscle and sternocleidomastoid of experimental group had difference with statistically significant at different stages(P<0.01);The median frequency of trapezius muscle in control group had significant difference at different stages(P<0.05);The median frequency of sternocleidomastoid of control group had significant difference after first course compared with prior treatment (P<0.01) and there were no significant difference after second courses compared with first course of treament (P>0.05).Comparison between groups, after first course of treatment, the median frequency of trapezius muscle and sternocleidomastoid of two groups had no statistical significance(P>0.05);after second course of treatment, the median frequency of trapezius muscle and sternocleidomastoid of two groups were statistically significant (P<0.05),and the experimental group was better than the control group.6. Correlation analysis:the correlation analysis of cervical physiological curvature and SF-MPQ score in neck type of cervical spondylosis patients that Pearson correlation coefficient was 0.683,suggest that there were a significant negative correlation each other (P< 0.01);Compared with the correlation analysis of trapezius muscle and sternocleidomastoid iEMG that Pearson correlation coefficient was 0.243 and 0.406,suggest that there were positive correlation each other (P< 0.05);Compared with the correlation analysis of trapezius muscle and sternocleidomastoid MF that Pearson correlation coefficient was 0.396and 0.353, suggest that there were significant positive correlation between (P< 0.01).Conclusion:l.Two methods of treatment of neck type of cervical spondylosis all have certain effect, but the clinical cure rate of self-gravity curvature traction combined with exercise therapy is higher, especially in improving cervical physiological curvature and the neck muscle iEMG and MF is obviously better than the conventional traction combined with exercise therapy.2.Self-gravity curvature traction combined with exercise therapy to treat neck type of cervical spondylosis can correct cervical internal and external mechanical balance and improve the stability of the cervical spine through recovering the cervical curvature sequence, improving the function of the neck muscles, strengthening the neck muscles strength,increasing the ability of muscle fatigue,so as to so achieve the goal of treatment.3.Self-gravity curvature traction combined with exercise therapy is a new method which is safe, effective, economic and convenient,it is worth clinical application and promotion to treat neck type of cervical spondylosis...
