Objective Through the old age femoral intertrochanteric fractures of postoperative patients with limb swelling with peach siwu decoction medicine orientation,observe limb swelling,pain,inflammation,and under the bed for the first time after time and the influence of the hip joint function and so on,to evaluate peach siwu tang directed through medicine for elderly patients with femoral intertrochanteric fracture of limb swelling effect,provide the basis for clinical application.Methods According to the inclusion and exclusion criteria,90 cases of elderly patients with femoral intertrochanteric fractures and swelling and pain of affected limbs after surgery in the Department of Orthopedics and Traumatology,People’s Hospital Affiliated to Fujian University of Traditional Chinese Medicine from January 2021 to December 2021 were selected,and randomized using SPSS 22.0 Divided into intervention group and control group with 45 cases in each.The control group received conventional treatment and nursing,and the intervention group received taohong Siwu Decoction directed permeation at huantiao and Xuehai acupoints,intervention once a day,20 minutes each time,starting from the first day after the operation,continuous intervention for 14 days.Before intervention,3 days after intervention,7 days after intervention,and 14 days after intervention,the swelling,pain level and inflammation level of the affected limbs in the two groups were evaluated.The time of getting out of bed for the first time after surgery was recorded.Hip function was assessed before,14 days after,and follow-up 30 days after surgery.SPSS 22.0 was used for statistical analysis,and P<0.05 was considered as statistically significant difference,in order to evaluate the influence of taohong Siwu Decoction directional permeation on the swelling and pain of the affected limb in elderly patients with intertrochanteric fracture after surgery.Results1 Completion of the study A total of 90 patients were included in this study,and the final 83 patients completed the trial.Among them,2 patients in the intervention group were excluded,2 patients dropped out,and a total of 41 patients were completed;3 patients in the control group were dropped out,and a total of 42 patients were completed.2 Baseline data situation Before intervention,there was no statistical significance in age,gender,height,weight,education level,marital status,medical expense payment method,escort status,history of underlying diseases,surgical side,cause of injury and fracture type between the two groups(P > 0.05).3 Comparison of the evaluation indicators of the two groups of patients before and after the intervention3.1 Comparison of the swelling of the affected limb between the two groups of patients3.1.1 Circumferential diameter difference of the next circumference(the greater trochanter of the femur and the root of the thigh)(1)Comparison before intervention There was no significant difference in lower extremity circumference difference(the greater trochanter of the femur and the root of the thigh)between the two groups before intervention(P >0.05),which were comparable.(2)Comparison within groups After 3,7 and 14 days of intervention,the difference of lower limb circumference between the two groups was all P <0.05 compared with before intervention,and the difference was statistically significant.The difference of lower limb circumference between the two groups was larger than before intervention after 3 days and 7days of intervention.After 14 days of intervention,the difference of lower limb circumference between the two groups was smaller than before.After 7 days of intervention,the difference of lower limb diameter between the two groups was smaller than that after 3 days of intervention(P<0.05),and after 14 days of intervention,the difference of lower limb diameter between the two groups was smaller than that after 3 days of intervention and 7 days of intervention(P<0.05).(3)Comparison among groups After 3 days of intervention,7 days of intervention,and14 days of intervention,the difference in circumference of the next week in the intervention group were smaller than those in the control group,and the differences were statistically significant(P<0.05).3.1.2 The difference in the circumference of the next circumference(15cm above the patella)(1)Comparison before intervention There was no significant difference in lower extremity circumference difference(15cm above the patella)between the two groups before intervention(P>0.05),which was comparable.(2)Comparison within groups After 3,7 and 14 days of intervention,the difference of lower limb circumference between the two groups was all P <0.05 compared with before intervention,and the difference was statistically significant.After 3 days of intervention,the difference of lower limb circumference between the two groups was larger than before intervention.After 7 days of intervention and 14 days of intervention,the difference of lower limb circumference between the two groups was smaller than before intervention.After 7 days of intervention,the difference of lower limb diameter between the two groups was smaller than that after 3 days of intervention(P<0.05),and after 14 days of intervention,the difference of lower limb diameter between the two groups was smaller than that after 3 days of intervention and 7 days of intervention(P<0.05).(3)Comparison among groups After 3 days of intervention,7 days after intervention,and 14 days after intervention,the difference in circumference of the next week in the intervention group was smaller than that in the control group,and the differences were statistically significant(P<0.05).3.2 Comparison of pain between two groups of patients(1)Comparison before intervention There was no significant difference in VAS scores between the two groups before intervention(P>0.05),which was comparable.(2)Comparison within groups After 3,7 and 14 days of intervention,VAS scores of the two groups were lower than before intervention(P<0.05),and the difference was statistically significant.After 7 days of intervention,VAS scores of 2 groups were significantly lower than3 days of intervention(P<0.05),and after 14 days of intervention,VAS scores of 2 groups were significantly lower than 3 days of intervention and 7 days of intervention(P<0.05).(3)Comparison among groups After 3 days of intervention,7 days after intervention,and 14 days after intervention,the VAS scores of patients in the intervention group were lower than those in the control group,and the differences were statistically significant(P<0.05).3.3 Comparison of inflammation levels between the two groups of patients3.3.1 Serum CRP level(1)Comparison before intervention There was no statistical significance in serum CRP levels between the two groups before intervention(P>0.05),which was comparable.(2)Comparison within groups After 3,7 and 14 days of intervention,the serum CRP level of 2 groups were all P <0.05 compared with that before intervention,and the difference was statistically significant.After 3 days of intervention,the serum CRP level in both groups was higher than before intervention.After 7 days of intervention and 14 days of intervention,serum CRP levels in both groups were lower than before intervention.After 7 days of intervention,the serum CRP level in both groups was lower than that after 3 days of intervention(P<0.05),and after 14 days of intervention,the serum CRP level in both groups was lower than that after 3 days of intervention and 7 days of intervention(P<0.05).(3)Comparison among groups After 3 days of intervention and 7 days after intervention,the serum CRP levels in the intervention group were lower than those in the control group,and the difference was statistically significant(P<0.05).After 14 days of intervention,there was no significant difference in serum CRP levels between the two groups(P>0.05).3.3.2 ESR level(1)Comparison before intervention There was no statistical significance in serum ESR levels between the two groups before intervention(P>0.05),which was comparable.(2)Comparison within groups After 3,7 and 14 days of intervention,THE ESR levels in 2 groups were all P <0.05 compared with those before intervention,and the difference was statistically significant.ESR levels in both groups were higher than before intervention 3 and 7days after intervention.After 14 days of intervention,ESR levels in both groups were lower than before.After 7 days of intervention,ESR level in both groups was higher than that after 3days of intervention(P<0.05),and after 14 days of intervention,ESR level in both groups was lower than that after 3 days of intervention and 7 days of intervention(P<0.05).(3)Comparison among groups After 3 days of intervention,ESR levels in the two groups were not statistically significant(P>0.05).After 7 days of intervention and 14 days after intervention,ESR levels in the intervention group were lower than those in the control group,and the difference was statistically significant(P<0.05).3.4 Comparison of the first time to get out of bed after surgery between the two groups The time of first getting out of bed in the intervention group was shorter than that in the control group,and the difference was statistically significant(P<0.05).3.5 Comparison of hip joint function between two groups of patients(1)Comparison before intervention There was no significant difference in Harris hip function score(HHS)between the two groups before intervention(P>0.05),which was comparable.(2)Comparison within groups After 14 days of intervention and the follow-up 30 days after operation,HHS of the intervention group and the control group was higher than before intervention(P<0.05),and the difference was statistically significant.The HHS score of the intervention group and the control group was significantly higher than that of the intervention group at the follow-up 30 days after operation(P<0.05).(3)Comparison among groups After 14 days of intervention,the HHS in the intervention group was higher than that in the control group,and the difference was statistically significant(P<0.05).There was no statistical significance in HHS between the two groups during follow-up 30 days after surgery(P>0.05).3.6 Safety Evaluation No adverse reactions related to this study occurred during the intervention period.Conclusion Taohong Siwu Decoction can improve the swelling of the affected limb in elderly patients after intertrochanteric fracture surgery to a certain extent,relieve postoperative pain,reduce the level of postoperative inflammatory response,shorten the first time to get out of bed after surgery,and improve the hip It has certain safety and is worthy of clinical application and promotion. |
