Objective: Through the systematic collation and analysis of the prescription data of oral traditional Chinese medicine for the treatment of Failed back surgery syndrome,the rules of medication composition were found,the drug matching relationship was explored,and the core prescription was explored.Through network pharmacology,the core prescription drug "Astragalus-Angelica-Oxknee-Chuanxiong" was established for the treatment of Failed back surgery syndrome,and the mechanism of action of core drugs in the treatment of Failed back surgery syndrome was predicted from the molecular level by combining the disease target genes.Methods: Study 1: Retrieved from the literature database to collect clinical research literature on the treatment of Failed back surgery syndrome,and extracted Chinese medicine prescriptions.Through the software of the TCM inheritance computing platform(V3.0),the frequency analysis,association rule analysis and cluster analysis of prescription Chinese medicines were carried out,and the efficacy,sexual taste,attribution,high-frequency drugs and combinations of all prescription Chinese medicines were statistically obtained,and the core drug pairs were refined and core prescriptions were mined.Study 2: Query the active ingredients and targets of core prescription drugs through databases such as TCMSP and TCMID;The Gene Cards database was used to screen out the disease targets of;Cytoscape3.9.1 software was used to construct a TCM-compound-target network diagram;Venny was used to intersect drug targets with disease targets to obtain core drugs and disease co-targets;and a common target PPI network was constructed through the STRING online platform for reuse Cytoscape3.9.1 software performs network topology analysis to screen out core gene targets,and uses the DAVID platform to perform GO enrichment analysis and KEGG pathway analysis on drug and disease common targets.Predict the core targets and pathways of traditional Chinese medicine in the treatment of Failed back surgery syndrome.Results: Study 1: A total of 113 articles were included in this study,113 prescriptions were extracted,and a total of 237 drugs were obtained.The top10 Chinese medicines used frequently are angelica,Chuanxiong,ox knee,astragalus,safflower,earth dragon,peach kernel,licorice,white peony,myrrh;The medicinal properties are mostly warm,flat and cold,and warm drugs are the mainstay;The medicinal taste is mostly sweet,bitter and spicy;Drugs are mostly attributed to the liver,spleen,heart,and kidneys;The efficacy of the drug is mainly to activate blood circulation and remove stasis,replenish deficiency,dispel rheumatism,calm the liver and quench wind,etc.Based on the analysis of association rules,38 pairs of high-frequency traditional Chinese medicine combinations were obtained,and angelica + Chuanxiong,angelica + safflower,angelica + astragalus were the top three drug combinations with frequent occurrence.There were 18 core drug combinations,Chuanxiong + red peony + angelica,Chuanxiong + astragalus+ ground dragon + angelica,red peony + angelica as the top three combinations of confidence;The core drug combination is similar to 4 groups,and the core drug 4 flavors,respectively,angelica,astragalus,oxknee,and Chuanxiong.Study 2: The core prescription obtained from Study 1 extracted4 core Chinese medicines of angelica,astragalus,ox knee and Chuanxiong for network pharmacological analysis,and the active ingredients of the drug were queried through the TCMSP database,and a total of 49 active ingredients were obtained,including 20 astragalus,2 angelica,20 ox knee and 7Chuanxiong;The active ingredients correspond to 234 targets of astragalus,50 angelica,207 ox knees,31 Chuanxiong,and 231 drug active ingredient targets after combined deduplication;Cytoscape3.9.1 was used to perform topological analysis on the network diagram of the active ingredient target,and the top three core compounds were quercetin,β-sitosterol and kaempferol.In Gene Cards,the disease targets of Failed back surgery syndrome were queried,and 708 disease-related targets were obtained after combined with deduplication,and 58 common targets were obtained by intersecting the targets of the disease and the active ingredient.Using cyto Hubba analysis,the core genes of the top 10 scores were IL6,VEGFA,TP53,TNF,AKT1,IL1 B,ESR1,EGF,MMP9,and PTGS2.The results of GO functional enrichment analysis were inflammatory response,immune response,oxidative stress response,cell proliferation and body response to drugs.KEGG signaling pathway includes various cancer-related signaling pathways,PI3K/AKT signaling pathways,cellular aging signaling pathways,TNF signaling pathways,VEGF signaling pathways,C-type lectin receptor signaling pathways,HIF-1 signaling pathways,IL-17 signaling pathways,etc.Conclusion(s): The Failed back surgery syndrome with traditional Chinese medicine is mainly based on replenishing qi and blood,strengthening liver and kidney,and dissolving blood stasis.High-frequency drugs include deficiency medicines,mainly blood circulation and stasis removal drugs,supplemented by rheumatism,liver and wind extinguishing drugs.High-frequency Chinese medicines are angelica,Chuanxiong,ox knee,astragalus,safflower,earth dragon,peach kernel,licorice,white peony,myrrh,etc.;The high-frequency combination is angelica + Chuanxiong,angelica +safflower,angelica + astragalus;The core prescription drugs are angelica,astragalus,oxknee,and Chuanxiong.In the core prescription,angelica and astragalus are the king,and the medicine composition of angelica blood soup is composed of qi and blood,"qi is the handsome of blood" and "blood is the mother of qi",qi is actually bloody,blood is stasis,stasis is vein,qi is bloody,qi is bloody,blood is stagnant,and qi is strong and blood is self-eliminating;Supplemented by Chuanxiong,ox knee through the meridians,Chuanxiong focuses on activating blood and relieving pain,ox knee nourishes liver and kidney,and strengthens muscles and bones.When all drugs are used together,qi and blood,foot and blood stasis,liver and kidney are strong,muscles and bones are strong.The network pharmacological analysis of the core drug group "angelica,astragalus,oxknee,Chuanxiong" in the treatment of Failed back surgery syndrome was carried out,and it was concluded that its core drug effect was through quercetin,kaempferol,β-sitosterol and other active ingredients,acting on core targets such as IL6,AKT1,TP53,TNF,VEGFA,etc.,and its core targets mainly regulated various cancer-related signaling pathways by participating in the body’s inflammatory response,immune response,oxidative stress response,cell proliferation and the body’s response to drugs.PI3K/AKT signaling pathway,cellular aging signaling pathway,TNF signaling pathway,VEGF signaling pathway,C-type lectin receptor signaling pathway,HIF-1 signaling pathway,etc.play a role in the treatment of Failed back surgery syndrome.It reflects the characteristics of multi-component,multi-target and multi-pathway of traditional Chinese medicine compound treatment of diseases. |
