| Actively promoting the integrated reform of county-level medical services is an important strategy and approach to solve the current shortage of medical and health resources,low quality,unreasonable structure and layout,and fragmented service systems.As a new model of innovative grass-roots medical reform,the close county-level medical community can achieve Pareto improvement and regional health performance by reconstructing the countylevel medical service system and integrating the resources of the county,township,and village three-level medical service network.At present,many counties in my country have carried out different forms of close medical community construction practices based on different policy environments,resource endowments and population characteristics,and have achieved great results and emerging experiences that can be used for reference,but they are still piloted and promoted in many places.stage,there are still many obstructions in practice.Among them,the integration of the resources of the close county-level medical community involves many parties such as the government,hospitals,and patients.Due to the greater interest adjustment and functional reorganization involved,there are currently many difficulties in the coordination process of all parties,which affects the county and township level medical care.The effectiveness of resource integration and the improvement of the overall medical service level in the county.Therefore,it is of great practical significance to explore the synergistic strategies and incentive-compatible paths for resource integration in a compact county-level medical community.This paper takes the compact county-level medical community in H County,Guangxi as the research object,uses the theory of collaborative governance and incentive compatibility theory as analytical tools,adopts the method of literature research and interviews,and analyzes the existence of the compact county-level medical community in H County in resource integration.The collaborative dilemma is analyzed,and the strategy to solve the dilemma is proposed.First,based on policy documents and field research,the collaborative subject of resource integration of the close county medical community in H County is defined and its interest demands are investigated.Then,the current situation of resource integration,including integration measures and integration effects,is analyzed and sorted out.Secondly,this paper analyzes the collaborative dilemma faced by the resource integration of the tight county medical community in H County from five aspects: loose collaborative organization alliance,weak collaborative guarantee support,weak collaborative subject power,collaborative process capability restriction,and lack of collaborative feedback mechanism.Analysis of its causes.Finally,in response to the above problems,it is proposed from six aspects: building a solid organizational alliance structure,consolidating policy and institutional guarantees,establishing an incentive compatibility mechanism,cross-departmental coordination,stimulating the endogenous power of the development of primary medical care,and enhancing policy awareness and cultural identity.A collaborative strategy for resource integration in a tight county medical community,so as to realize the sustainable development of the medical community. |
