Study On The Problems And Countermeasures Of J Provincial Government’s Promoting The Construction Of A Close-knit County Medical Community

The 19 th National Congress of the Communist Party of China made a major decision to implement the “Healthy China” strategy,raising the maintenance of people’s health to a new level of national strategy.In October 2016,the “Healthy China 2030”Planning Outline issued by the CPC Central Committee and the State Council proposed to “comprehensively build an integrated medical and health service system with a complete system,a clear division of labor,complementary functions,close collaboration and efficient operation”.As a result,the pilot project for the construction of close-knit county medical and health communities(abbreviation: close-knit county medical communities)across the country was officially launched.As an important exploration to further deepen the reform of China’s medical and health system in the new era,the construction of a close-knit county medical community has been highly valued by the Party Central Committee.General Secretary Xi Jinping has repeatedly stressed that “it is necessary to intensify the construction of public medical and health institutions,promote the construction of county medical communities,improve the conditions of grassroots infrastructure,implement the treatment of rural doctors,and enhance the ability of grassroots disease prevention and treatment and health management”.As a relatively typical economically underdeveloped province in China,J province has multiple identities such as an old industrial base,an important national commodity grain production base,a pilot province for national ecological construction,and a frontier province.In recent years,the medical service capacity of primary medical and health institutions(township health centers,community health centers,village clinics,etc.)in J province have stagnated,and the county’s medical service resources have become weaker.From the perspective of the reform of the medical and health system,the in-depth integration of medical and health resources and the integration of medical and health services at the county level will help J province alleviate prominent problems such as insufficient total medical resources,uneven distribution,lack of grassroots medical and health personnel,and weak medical service capabilities.Accelerating the construction of a close-knit county medical community will help J province improve the county medical treatment rate and patient satisfaction,and better provide economical,high-quality and convenient integrated medical and health services to the people.In 2019,J province officially launched the pilot construction of a province-wide close-knit county medical community and the community will be fully rolled out across the province in 2022.In terms of organizational leadership,the construction of a close-knit county medical community has leading institutions,arrangements for deployment,and measures.In terms of practical effects,by the end of August 2022,10 pilot counties had reached the national standard for judging the construction of close-knit county medical communities.At present,there are the following outstanding problems in the construction of a close-knit country medical community in J province:First,the implementation of policies is not in place,and the overall progress is lagging behind;Second,the integration of resources is insufficient and the degree of compactness is insufficient;Third,the operating mechanism is not perfect and lacks sustainable development planning;Fourth,the public’s willingness to seek medical treatment at the grassroots level is not strong,and the hierarchical diagnosis and treatment has not been comprehensively promoted.For the J provincial government,it is urgent to solve the above outstanding problems to ensure the smooth completion of the pilot construction of the close-knit county medical community in the province and the sustained and stable development of the close-knit county medical community.Through the analysis of the causes of the prominent problems in the construction of the close-knit county medical community in J province,and combining with the advanced experience and mature practices of the construction of the close-knit county medical communities in Shanxi,Zhejiang and Fujian provinces,the J provincial government further promotes the construction of the close-knit county medical community,which will focus on the following four aspects.First,strengthen the government organization and leadership to further unify thinking,raise awareness,have the courage to take responsibility in the reform,dare to delegate power,scientifically transmit pressure,and strengthen assessment.Second,promote the formulation of supporting policies,especially increase financial investment,implement the reform of the salary system,and promote the reform of medical insurance policies.Third,to promote the integration of a close-knit county level medical community,we must focus on completing internal management integration to facilitate the allocation of medical resources;at the same time,we will strengthen the integration of external services and promote hierarchical diagnosis and treatment.Fourth,increase the intensity of policy publicity,ensure that construction measures are open and transparent,and that construction progress is reported in a timely manner,and strive to win the understanding of the cadres and workers of the leading units and member units and gain the support of the people at the county level.
