ObjectiveTo construct a scientific and systematic core competence evaluation index system for ECMO specialist nurses,in order to provide references for the training,assessment and ability evaluation of ECMO specialist nurses.Meantime,it promotes the development of ECMO specialized nursing.Methods1.With “extracorporeal membrane oxygenation”,“nurs*” and “core competence/competency” as the search terms,the research group searched for the literature in domestic and foreign databases,and formed a pool of core competence items of ECMO specialist nurses preliminarily.2.Using qualitative research methods,the research team conducted semi-structured interviews with related interviewees in tertiary grade A polyclinic from Shandong province,including nurses,ICU physicians,perfusionists and head nurses who managed ECMO independently.The interview data were analyzed by content analysis to determine the needs of core competences of ECMO specialist nurses.Based on the results of literature review and semi-structured interview,the core competence evaluation index system for ECMO specialist nurses was initially formed.3.30 experts from all over the country were selected to participate in the two rounds of expert consultation.The experts were asked to put forward to suggestions on the revision of the indexes and rate the importance of the proposed indexes.The core competence evaluation index system for ECMO specialist nurses was finally constructed.4.The weights of indicators were determined,and consistency ratio was calculated by analytic hierarchy process.Results1.The pool of core competence items of ECMO specialist nurses was initially formed by literature review.2.7 themes were extracted from the semi-structured interviews,including professional knowledge,clinical practice ability,critical thinking ability,professional development ability,communication and coordination ability,management ability,and personal traits.3.25 experts from 6 provinces in China completed two rounds of consultation.The effective recovery rates of the two rounds of expert consultation were 90.00% and92.59% respectively,the expert authority coefficient was 0.886,and the Kendall harmony coefficient was 0.205(P<0.001)in the second round.The final core competence evaluation index system for ECMO specialist nurses includes 7 first-level indexes,19second-level indexes and 70 third-level indexes.The first-level indexes include: clinical practice ability,professional knowledge,communication and coordination ability,critical thinking ability,professional development ability,management ability and personal traits.4.The weights of the first-level indexes were clinical practice ability(0.273),professional knowledge(0.223),communication and coordination ability(0.162),critical thinking ability(0.128),professional development ability(0.094),management ability(0.077),and personal traits(0.042).All consistency ratio were less than 0.1,which indicated the setting of indexes weights was reasonable.ConclusionsThe core competences of ECMO specialist nurses are comprehensive.It is necessary to pay attention to and strengthen these seven core competences of ECMO specialist nurses so as to meet the needs of ECMO specialized nursing development.With high enthusiasm and authority of experts and consensus of experts,the core competence evaluation index system for ECMO specialist nurses in this study was scientific and reliable,which can provide valuable references for the training,assessment and ability evaluation of ECMO specialist nurses.
