Study On The Construction Of Core Competence Index System Of Hospice Nursing Specialist Nurses

【Objective】This study is based on the literature review of the current situation of core competence of hospice care specialist nurses,construct a peace of a systematic and scientific care specialist nurse core competence system,aimed at promoting the construction of the standardization of hospice care specialist nurses,improve the quality of nursing personnel of hospice care,improving the quality of life at the end of the patient’s survival.【Method】1.Document research method and qualitative research--semi-structured interview method were used to collect data to preliminarily construct the item framework of core competence of nursing specialists in hospices;2.Use that modified Delphi method,expert consultation were carried out.According to the expert opinions and suggestions,the core competence index system of hospice nursing specialist nurses was determined;3.Optimal order graph method and average distribution method were used to determine the weight of indicators at all levels;4.A questionnaire survey was conducted among 80 nurses in palliative medicine department and oncology department to preliminarily test the reliability and validity.【Results】1.According to literature research and semi-structured interview results,combined with theoretical analysis,the index system of core competence of hospice nursing specialist nurses was preliminarily constructed,including first-level indexes(5 dimensions),second-level indexes(23 items)and third-level indexes(98 items).2.Pre-consultation with 4 experts,including 3 females and 1 male,aged 45～57(51.75 ± 5.74)years old,working years 19～36(29.50 ± 8.58)years.According to the expert opinions,5 first-level indicators,23 second-level indicators and 83 third-level indicators were included after the modification.3.After two rounds of Delphi correspondence,the recovery rate and effective rate of the questionnaire were 90% and 100%,respectively.The basis of judgment was 0.861,the familiarity was 0.867,and the authority coefficient(CR)was 0.864,indicating that the experts were highly active and authoritative.The coordination coefficient was between0.300 and 0.515(P < 0.01),and the coordination was good.Finally,6 first-level indicators,22 second-level indicators and 73 third-level indicators were determined.4.The weights of first-level indicators determined by the order chart method were:clinical nursing(0.2056),psychological nursing(0.1944),career development(0.0500),supportive care(0.1722),narrative nursing(0.1944),ethics and law(0.1852).5.Through the questionnaire survey results show,S-CVI is 0.863～0.945,S-CVI/Ave is 0.960,the correlation coefficient between the first-class index score and the total scores of all indexes is 0.759～0.947(P < 0.05),and the correlation coefficient between the second-class index score and the total scores of all indexes is 0.607～0.835(P < 0.05),which shows good content validity and structure validity.Cronbach’s α coefficient of the whole index system is 0.964,which has good reliability.【Conclusion】This study determined the hospice care specialist nurses,hospice care specialist nurse two core competence concept,forming a set of system,science,hospice care specialist nurses index system of core competence,including 6 first-level indicators,22 secondary indicators and 73 tertiary indicators,to the talent training and discipline development of hospice care to provide theoretical reference and improve the quality of life at the end of the patient’s life.
