The Construction Of Core Competence Evaluation Index System For Domestic Parkinson’s Disease Nurse Specialist

Objective:The purpose is to build a scientific,comprehensive,systematic and practical evaluation index system for the core competence of domestic Parkinson’s Disease Nurse Specialist(PDNS)through qualitative research combined with quantitative research,so as to clarify the requirements of domestic clinical nursing on the core competence of PDNS,provide a scientific reference standard for the setting of PDNS training courses in China,and provide powerful selection Certification and evaluation basis,further promoting the professional and refined development of Parkinson’s Disease(PD)nursing field,improving the quality of PD nursing,and improving the satisfaction of PD patients.Methods:1.Using the literature research method,combined with the theoretical analysis method,relying on the core competence theory and the role theory,and referring to the PDNS practice guide approved by the Netherlands Nursing Association,we analyzed and summarized,and infiltrated the views of PD clinicians,nurses and the main caregivers of PD patients through semi-structured interviews.At the same time,taking into account the characteristics of PD disease,we initially defined the concept of PDNS and its core competence,and formulated the draft indicator system.2.Using the Delphi method,select medical and nursing experts with academic attainments and clinical experience in the field of PD in China according to established inclusion criteria,score the importance of relevant concepts and indicator entries,and provide comments and suggestions.Conduct scientific statistical analysis of correspondence data,and ultimately construct a domestic PDNS core competency evaluation indicator system when the differences in expert opinions are small.3.Using the analytic hierarchy process to calculate the weight values of indicators at all levels in the system,measure their importance,and form a complete domestic PDNS core competency evaluation index system.4.Test the reliability and validity of the research results to evaluate its reliability and stability.Results:1.Based on literature research,theoretical analysis,and semi-structured interviews with 10 PD nurses,4 doctors,and 5 primary caregivers of PD patients,the relevant concepts of PDNS and its core competencies were defined,and the obtained core competency items were analyzed,compared,summarized,and extracted.A preliminary draft of the domestic PDNS core competency evaluation index system was drafted,including 7 first level indicators,27 second level indicators,and 124 third level indicators,Form the foundation for subsequent research.2.In this study,a total of 22 experts from 13 provinces continued to complete two rounds of Delphi correspondence.The effective recovery rates of the two rounds of correspondence questionnaires were 92.00% and 95.65%,respectively,and the expert authority coefficients(Cr)were 0.889 and 0.905,indicating that the experts had a high enthusiasm for this study and provided reliable opinions.The minimum mean of the importance scores of the indicators in the two rounds of correspondence were 3.39 and4.18,the maximum standard deviation was 1.377 and 0.963,the minimum full score ratio was 13.04% and 27.27%,and the maximum coefficient of variation(CV)was 0.396 and0.216,respectively.The Kendall coordination coefficients(s Kendall’ W)of the three level indicators in the two rounds of correspondence were 0.253,0.184,0.208,and 0.407,0.194,0.223(P<001.0),respectively,indicating that the research results were significantly better after the two rounds of correspondence,Expert opinions are more concentrated and coordinated.Finally,a domestic PDNS core competency evaluation index system consisting of 6 first level indicators,16 second level indicators,and 68 third level indicators was formed.3.The weight calculated by the analytic hierarchy process shows that the consistency ratio(CR)of each judgment matrix is less than 0.10,indicating good consistency.The weight of the first level indicators is ranked according to their size as clinical basic nursing ability(0.3087),support and guidance ability(0.3087),communication and cooperation ability(0.1438),critical thinking ability(0.1056),personal development ability(0.0763),and personal characteristics(0.0569).4.In the reliability and validity test results of the indicator system in this study,the Cronbach coefficient(C’ronbachα)of all indicators at all levels is above 0.6,the content validity index(CVII-)at the item level is greater than 0.78,and the overall consistency value and average value in the content validity index(CVIS-)at the scale level are greater than 0.9,indicating that the reliability and validity of this indicator system are good.Conclusion:In this study,the concept of PDNS and its core competence was defined through qualitative and quantitative research methods,and the weight of each indicator was determined by scientific statistical methods.Finally,a systematic,comprehensive and complete evaluation index system of domestic PDNS core competence was constructed,including 6 first-level indicators,16 second-level indicators and 68 third-level indicators.The research results are scientific and reliable,and provide some reference for future managers to the cultivation,selection and management of PDNS.By analyzing the weight coefficient of each index,it can provide focus direction for the training and evaluation of PDNS in the future.
