Background:In recent years,the treatment of locked intramedullary nail has been gradually applied in the treatment of some eligible clavicle fractures,with less periosteal stripping,less impact on blood supply,and smaller incision than plate fixation,which is in line with people’s pursuit of beauty.Although it has some advantages,it still has some drawbacks.In view of this,this team used Mimics21.0 software and CAD2022 software to measure the clavicle anatomy.According to the internal characteristics and structural characteristics of interlocking bone marrow cavity,we designed a new interlocking bone marrow nail instrument.which has biological properties such as stability,firmness,good compatibility with the human body,with good elastic fixation effect on the fracture and enhanced anti-rotation ability.Objective:To measure the anatomical number of clavicle by mimics 21.0 software and CAD software,and to provide scientific guidance for the design and parameter determination of self-developed intramedullary nail.Methods:A total of 93 CT data of patients with clavicle fracture and shoulder CT from 2019 to 2021 in our hospital were collected,more males than females,and measured by MIMCS21.0 and CAD software.SPSS 25.0 software was used for statistical analysis of the measurement results.Normally distributed continuous data were expressed as mean plus or minus standard deviation.Independent sample t-test was used for comparison between the two groups.Test level a=0.05.Results:All the selected samples were adults from north China,with male age of 49.65±15.21 years and female age of 51.31±16.54 years.The total length of the clavicle was 139.24mm-160.78mm,including 146.6mm-163.12mm for males and 133.01mm-153.61mm for females.The average length of the clavicle in males was 11.55mm longer than that in females(P<0.05).The overall width of the distal clavicle was 16.95mm-24.41mm,the middle width was 9.2mm-14.56mm,and the proximal width was 20.29mm-27.15mm.The width of the sternum and acromion ends was larger than that of the middle part,and the width of the proximal part was larger than that of the distal part.The proximal width of the male clavicle was 21.03mm to 27.89mm,the middle width was 9.76mm to 15.46mm,and the distal width was 18.36mm to 25.3mm.The proximal width of the female clavicle was 19.5mm-25.9mm,the middle width was 8.8mm-12.96mm,and the distal width was 15.56mm-22.62mm.The width of three parts of clavicle axial plane in male was larger than that in female(P<0.05).The overall range of internal curvature radius was 51.82-78.08mm(56.78mm-79.96mm of the male and 46.45mm-74mm of the female),the male radius was larger than the female radius by 8.22mm(P<0.05).The overall range of external curvature radius is 23.87-41.45mm,among which,male is 26.18mm-42.28mm and female is 21.05mm-39.88mm,male is larger than female 3.75mm(P<0.05).The inner angle ranged from 143.97° to 158.19°,the male angle was 144.95° to 159.93°,and the female angle was 143.02° to 155.34°.The difference between males and females was about 3.26°(P<0.05).The lateral Angle ranged from 142.35° to 160.27°,the male angle was 144.94° to 161.1°,and the female angle was 139.29-158.61°,the male angle was larger than the female angle with a difference of about 4.07°(P<0.05).The clavicular cavity and cross-sectional area gradually decreased from the proximal end to S4 and S5 of the middle,which thickness of the cortical bone increased,but the increase was uneven.After S5,the cross-sectional area gradually increased again,but the cross-sectional area of the proximal clavicle was still larger than that of the distal end.The medullary cavity approximately rotates counterclockwise,from the beginning which the height is greater than the width to that the height is less than the width.The male medullary cavity was generally larger than the female(P<0.05).According to the measured anatomical data,we designed six types of intramedullary nails according to the length of the clavicle and the width of the medullary cavity.The length of the intramedullary nails was designed to be 130-155mm,and every 5mm was a specification.According to the length,the radian of the nail body was 150°-175°,and every 5°was a specification.Conclusion:In this experiment,CT data of adult clavicle in northern China were measured,including clavicle length,width,distal and far radius radius of curvature,inner and lateral Angle,etc.Moreover,the morphology of the clavicular cavity was measured,and the adult intramedullary locking nail was designed and patented according to the imaging characteristics of the clavicular bone. |
