Objective: To study the relationship between the ratio of apo A1(apolipoprotein A1)/apo B(apolipoprotein B),brachial artery flow-mediated endothelial dilation(FMD)and the degree of coronary artery stenosis,so as to provide a new idea for the diagnosis and treatment of coronary artery disease.Methods: A total of 124 patients who underwent coronary angiography(CAG)for the first time in the Department of Cardiology of the affiliated Hospital of Qinghai Universit y from November 2020 to January 2022 were enrolled in the observation group and 61 patients in the control group.The observation group was divided into single-vessel disease group(n= 50),two-vessel disease group(n=33)and three-vessel disease group(n= 41).Blood lipids,blood glucose and other tests were perfected within 24 hours after admission.General data,FMD and coronary angiography data were collected.Gensini scores were calculated according to coronary angiography data.Al l the collected research indicators were statisticall y anal yzed by statistical software.Results: 1.There was no significant difference in sex,smoking,diabetes,hypertension,TC and LPa between CHD group and non-CHD group,but the age,apo B,BM I,LDL-C and TG in CHD group were higher than those in non-CHD group,while apo A1,apo A1/apo B,HDL-C and FMD in CHD group were lower than those in non-CHD group.2.Binary multivariate Logistic regression analysis showed that BMI(OR=2.012,95%C I:1.004-4.032),apo A1/apo B(OR=0.006,95%C I:0.634-1.538,0.024)and apo A1(OR=0.782,95%C I:0.710-1.426)were independent risk factors for coronary heart disease.3.The comparison between coronary heart disease subgroups showed that apo A1/apo B and FMD in single-vessel disease group wer e lower than those in two-vessel disease group and three-vessel disease group,apo A1/apo B and FMD in two-vessel disease group were lower than those in three-vessel disease group,and there were significant differences in apo A1/apo B and FMD among single-vessel disease,double-vessel disease and three-vessel disease group.4.Spearman correlation anal ysis of Gensini score showed that apo A1/apo B and FMD were negativel y correlated with Gensini score.5.The multiple linear regression of Gensini integral showed that t here were significant differences between FMD(Baudelaire 15.376,95% CIRue 18.410,-12.341),apo A1/apo B(Bevermore 17.208,95% C IRV 24.382)and Gensini(10.033).6.Draw the ROC curve of apo A1/apo B to coronary heart disease,the area under the curve of apo A1/ apo B is 0.843,the confidence interval of 95% of II is 0.780 to 0.905,the cutoff point of apo A1 apo B is 1.380,the specificit y is 72.1%,and the sensitivit y is 80.6%.The ROC curve between FMD and coronary heart disease was drawn.The 95% confidence inter val under the FMD curve was 0.751-0.877,the cutoff point was 6.656,the specificit y was 80.3%,and the sensitivit y was 71.8%.Conclusions:1.FMD and apo A1/apo B are the risk factors of coronary heart disease.2.The ratio of apo A1/apo B and FMD decreased with the severit y of coronary artery stenosis,and negativel y correlated with the severit y of coronary artery stenosis.3.When FMD < 6.656% or apo A1/apo B < 1.380,the risk of coronary heart disease is higher.
