Objective:In this study, the expression levels of serum thyroid related hormones and antibodies were compared between patients with benign and malignant breast tumors, and then the relationship between thyroid related hormones and clinical characteristics of breast cancer was further discussed to clarify the correlation between thyroid function and the risk of breast cancer,so as to provide new ideas for the pathogenesis and clinical prevention and treatment strategies of breast cancer. Methods:A total of 200 female patients with benign and malignant breast tumors were selected from the Department of breast and thyroid tumor surgery of the Affiliated Hospital of Qinghai University from June 2016 to June 2021.The general data of the two groups were collected. After obtaining informed consent one week before operation, the fasting serum levels of Total Triiodothyronine(TT3),Total Thyroxine(TT4),Free Triiodothyronine(FT3),Free Thyroxine(FT4),Thyroid Stimulating Hormone(TSH),Immunoreactive parathyroid hormone(i PTH),Thyroglobulin(TG)and Thyroid globulin antibody(Anti-TG)and Thyroid peroxidase antibody(Anti-TPO)levels.The breast cancer group was the observation group, and the benign breast tumor group was the benign control group. All patients were diagnosed pathologically after operation.Estrogen Receptor(ER)status,Progesterone Receptor(PR)status, expression of Human epidermal growth factor receptor-2(HER-2),Nuclear proliferation antigen marker(Ki67)and Epidermal Growth Factor Receptor(EGFR)were detected in patients with breast cancer. The data were collected and sorted out.The general data(age, height, weight, menarche age, menopausal status and number of births),thyroid related hormones and antibody levels(TT3,TT4,FT3,FT4,TSH,i PTH,TG,Anti-TG and Anti-TPO)of the two groups were retrospectively compared and analyzed,The statistically significant thyroid hormone or antibody indexes and the clinicopathological characteristics of breast cancer were further statistically analyzed.According to TNM stage, the thyroid hormone levels of different course of breast cancer were statistically compared and analyzed. The statistically significant indexes in univariate analysis were again included in the multivariate binary logistic regression model for analysis,The correlation between thyroid related indexes and the risk of invasive breast cancer was obtained. Results:By comparing the general data(age, body mass index, menarche age, menopausal status and number of births)of patients with benign and malignant breast tumors, there was no significant difference between the two groups(P>0.050).The comparative analysis of thyroid related hormone levels between the two groups showed that the serum TT3 content of patients with breast cancer was 1.02(0.88,1.44)ng/ml,which was higher than that of the control group of benign breast tumors by 0.96(0.89,1.05)ng/ml(P=0.012);The positive rate of serum Anti-TPO expression in breast cancer group(62%)was lower than that in benign control group(76%)(P=0.032).There was no significant difference in TT4,FT3,FT4,TSH,i PTH,TG and Anti-TG between the two groups(P>0.050).Through variance analysis of thyroid hormones in patients with different courses of breast cancer,it was found that the TT3 content in patients with stage I breast cancer was1.53 ± 0.60ng/ml,that in patients with stage II breast cancer was 1.34 ± 1.49ng/ml,that in patients with stage III breast cancer was 0.93 ±0.12ng/ml,and that in patients with stage IV breast cancer was 0.81 ± 0.27ng/ml.The serum TT3 level in breast cancer patients gradually decreased with the progress of the disease, and that in patients with stage I,stage III There was significant difference between stage Ⅳ breast cancer(P<0.010).There was no significant difference in serum TT3 content in different age, tumor diameter,ER,PR,HER-2,Ki67,axillary lymph node metastasis and EGFR expression in breast cancer group(P>0.050).The expression of Anti-TPO in serum of patients with breast cancer had no significant difference with age, tumor diameter, clinical stage,ER,PR,HER-2,Ki67,axillary lymph node metastasis and EGFR expression(P>0.050).Multivariate binary logistic regression analysis showed that Anti-TPO positive was an independent factor affecting the incidence of breast cancer(P<0.050).Conclusion:Compared with the benign control group, the higher serum TT3 hormone level was associated with the incidence of breast cancer, and the TT3 level decreased gradually with the progression of breast cancer; The high positive rate of Anti-TPO is an independent protective factor for breast cancer.Monitoring the positive rate of thyroid peroxidase antibody has certain guiding significance for the prevention and treatment of breast cancer and the prognosis.
