Objective To observe the expression changes of tumor necrosis factor-α(TNF-α)and interleukin-1β(IL-1β)in tears in patients with dry eye treated with sphenopalatine ganglion acupuncture,and to explore whether TNF-α and IL-1β can objectively evaluate the efficacy of single acupuncture of sphenopalatine ganglion on dry eye.Methods 100 patients diagnosed as dry eye in our hospital were selected and classified as moderate and severe.After screening,the patients were randomly divided into treatment group and control group,with 50 patients in each group.The treatment group was treated with acupuncture at sphenopalatine ganglion,and the control group was treated with artificial tears "Ally-0.3%sodium hyaluronate" based on grading treatment.① The levels of TNF-α and IL-1β in tears of patients were detected and measured by the samples collected before and after treatment;②Individual patient scores were evaluated,including the Schirmer I test(SIT),rupture time of tear film(BUT)and fluorescein staining score of cornea(fluorescien,FL),Visual Analogue Scale(VAS)and the other 5 most common symptoms were scored.;③The total effective rate was statistically compared;④Correlation analysis of TNF-α and IL-1β expression levels;⑤ROC analysis was performed on the expression levels of TNF-α and IL-1β to determine whether they could be used as evaluation indicators.Results ① After treatment,the expression levels of TNF-α and IL-1β in tears of the whole group,moderate group and severe group were significantly decreased(P<0.05);Further analysis of the data after treatment showed that the expression levels of TNF-α and IL-1β in tears in treatment group(whole group,moderate group and severe group)were lower than those in control group(P<0.05).② After treatment,the levels of SIT and BUT in 2 groups(whole group,moderate group and severe group)were significantly increased(P<0.05),while FL,VAS and 5 symptom scores were significantly decreased(P<0.05);Further analysis of the data after treatment showed that the levels of SIT and BUT in the treatment group(whole group,moderate group and severe group)were significantly higher than those in the control group(P<0.05),while FL,VAS and 5 symptom scores were significantly lower than those in the control group(P<0.05).③ The total effective rate of treatment group(94.00%)was significantly higher than that of control group(76.00%),and the difference was statistically significant(x2=6.353,P<0.05).④Pearson correlation analysis showed that TNF-α and IL-1β Level had strong correlation(r=0.794,P<0.05).⑤ROC curve analysis showed that the AUC values of TNF-α and IL-1β were 0.804 and 0.835,respectively,and the sensitivity was 88.05%and 84.35%,suggesting that both TNF-α and IL-1β had certain diagnostic value.Conclusion ①After treatment,the level of patients in SIT and BUT acupuncture group was significantly higher than that in control group,and FL,VAS and 5 symptom scores were significantly lower than that in control group,suggesting that acupuncture group was better than control group.After treatment,the levels of TNF-α and IL-1β in tears of both groups were decreased,and the decreased levels in the acupuncture group were lower than those in the control group,suggesting that the acupuncture group was more effective in inhibiting inflammation than the control group.② Compared with graded treatment based on artificial tear,acupuncture of sphenopalatine ganglion has significant clinical efficacy and higher cost performance in the treatment of moderate and severe dry eye.③ Detection of TNF-α and IL-1β levels in tears not only has a certain value in the diagnosis of dry eye,but also can be used for objective and scientific evaluation of the post-treatment effect and prognosis. |
