Study Of Professor Liu Weiming’s Prescription Rules For Diarrhea-type Irritable Bowel Syndrome Based On The TCM Inheritance Auxiliary System

Objective Using the TCM inheritance auxiliary platform(V2.5)to tap and analyze the prescription rules of Professor Liu Weiming in treating diarrhea-type irritable bowel syndrome(IBS-D),and to summarise and refine Professor Liu Weiming’s academic thinking for IBS-D.Materials and methods In this study,220 cases of patients diagnosed with IBS-D in the outpatient clinic of Professor Liu Weiming of the Department of Spleen and Stomach Diseases of Jinan Hospital of TCM,affiliated to Shandong University of TCM from December 2021 to December 2022 were collected.The contents of the cases included patients’ basic information,information of the diagnoses and prescription of medication.A standard database was established and data mining was performed using the TCM inheritance auxiliary platform(V2.5),and to visualize the results,and to compare and analyze the data.Results1.Basic information:Among 220 cases,78 were male and 142 were female,with a male to female ratio of approximately 1:1.82;and the age distribution ranged from 19 to 81 years old,with the majority of the group aged 30 to 59 years old.2.Four diagnostic information:79 symptoms were counted,and the top three in order of frequency were loose stools,abdominal pain and loss of appetite;and the most frequent tongue and pulse appearances were light red tongue,thin white moss and weak pulse.3.Distribution of evidence types:79 cases of spleen deficiency and dampness,66 cases of liver depression and spleen deficiency,43 cases of spleen and kidney yang deficiency,22 cases of mixed cold and heat,and 10 cases of spleen and stomach dampness and heat.4.Frequency statistics:91 herbs were used 3636 times,and those with a frequency greater than 100 were Largehead atractylodes rhizome,Chinese yam,Codonopsis pilosula,Saposhnikovia divaricate,Coix seed,Dried tangerine peel,Plantain seed,Villous amomum fruit,Debark peony roots,Astragalus mongholicus,Medicated leaven,Pericarpium arecae,Kudzuvine root,Hyacinth bean,Gordon euryale seed.5.Taste,stringency and efficacy statistics:the tastes of the drugs are mainly sweet,pungent and bitter;the properties of the drugs are mainly warm and flat;the astringency of the drugs are mainly the spleen,lung,stomach,liver and kidney;the efficacy of the drugs is mainly deficiency medicine.6.Association rules:The high-frequency drug combinations are"Largehead atractylodes rhizome,Chinese yam","Largehead atractylodes rhizome,Saposhnikovia divaricate","Codonopsis pilosula,Largehead atractylodes rhizome",etc.The top three combinations of association rules with confidence are"Dried tangerine peel,Saposhnikovia divaricate→Largehead atractylodes rhizome","Saposhnikovia divaricate,Coix seed→Largehead atractylodes rhizome"and"Chinese yam,Saposhnikovia divaricate→Largehead atractylodes rhizome".7.Cluster analysis:A total of four groups of potential new formula combinations were obtained,namely(1)Chinese yam,Hyacinth bean,Coptis chinensis,Gordon euryale seed,Coix seed;(2)Medicated leaven,Plantain seed,Villous amomum fruit,Fructus psoraleae,Nutmeg;(3)Plantain seed,Villous amomum fruit,Fructus psoraleae,Saposhnikovia divaricate,Schisandra chinensis,Cornus;(4)Radix curcumae,Liquorice root,Astragalus mongholicus,Kudzuvine root,Pericarpium arecae,Radix bupleuri.Conclusion1.The core medicines used by Prof.Liu Weiming in the treatment of IBS-D are:Largehead atractylodes rhizome,Chinese yam,Codonopsis pilosula,Saposhnikovia divaricate,Coix seed,Dried tangerine peel,Plantain seed,Villous amomum fruit,Debark peony roots,Astragalus mongholicus,Medicated leaven,Pericarpium arecae,Kudzuvine root,Hyacinth bean,Gordon euryale seed.The high-frequency drug combinations are:"Largehead atractylodes rhizome,Chinese yam","Largehead atractylodes rhizome,Saposhnikovia divaricate","Codonopsis pilosula,Largehead atractylodes rhizome"2.Prof.Liu Weiming’s understanding of the pathogenesis of IBS-D include:(1)the middle jiao is not conducive to the ascending and descending of the clear and turbid;(2)the lesser yang is not smooth and the wood is depressed and the earth cannot be evacuated;(3)the vital energy is greatly weakened and the lower part of the body is deficient while the yin fire is strong.3.Prof.Liu Weiming’s principles for treating IBS-D include:(1)sweet and warm to benefit,pungent and bitter to transport;(2)dredge the liver to reach wood,strengthening the spleen and earth;(3)tonify deficiency and dipping fire,and wind medicine to point the eyes.
