Objective:This study is to observe the clinical effect of acupotomy on the improvement of symptoms of wrist Median nerve entrapment syndrome(MNES)with cold-dampness type.By comparing the relevant scores of the two groups of patients before and after treatment, objectively evaluate the difference of their curative effect of MNES,analyze the clinical mechanism and advantages of acupotomy release therapy for wrist Median nerve entrapment syndrome, improve the timeliness of clinical treatment of wrist Median nerve entrapment syndrome and reduce recurrence. Methods:A total of 66 Patients with cold-dampness type wrist Median nerve entrapment syndrome were admitted to the First Affiliated Hospital of Shandong First Medical University from December 2021 to January 2023.They were randomly divided into the treatment group and the control group, with 33 cases in each group. The treatment group used acupotomy therapy. The release site was the attachment point at both ends of the transverse carpal ligament, that is,the lateral edge of the most convex point of the pea bone, the hook of the hook, the inner lower part of the scaphoid bone node, and the medial edge of the most angular bone node. The treatment was once a week, and one week was a course of treatment; The control group used fine silver needle therapy, and selected acupoints according to the content of "Arthralgia Syndrome"in the " 13th Five Year Plan" teaching material acupuncture and moxibustion therapeutics for higher education in the Chinese medicine industry. The treatment was six times a week, six times a course of treatment, and the interval between courses was one day. After two courses of treatment, the total score of Chinese medicine clinical syndromes and the changes of various scores of Chinese medicine clinical syndromes of the two groups were observed, and the overall clinical efficacy of the two groups was calculated.SPSS 26.0 was used to analyze the changes of VAS score,Levine symptom and function score and the overall clinical efficacy of the two groups of patients before and after treatment,so as to reach the conclusion of this study.Results:1.After the treatment, the VAS pain score of patients was significantly improved compared with that before(P<0.05),and the difference between the two groups was statistically significant(P<0.05),suggesting that both acupotomy therapy and silver needle therapy can effectively relieve the pain of patients, and acupotomy therapy is better than silver needle therapy.2.After the treatment, the Levine symptom and function score of the patients were significantly improved compared with the previous one(P<0.05),and the difference between the two groups was statistically significant(P<0.05),suggesting that both acupotomy therapy and fine silver needle therapy can effectively reduce the patient’s pain symptoms and restore the patient’s wrist function, and acupotomy therapy is better than fine silver needle therapy.3.After the treatment, the total score of the patients’ clinical syndromes of traditional Chinese medicine was significantly improved compared-with that before(P<0.05),and the difference between the groups was statistically significant(P<0.05).The score of numb pain frequency, numb pain degree, swelling degree and joint flexion and extension of patients were significantly improved compared with that before(P<0.05),and the difference between groups was statistically significant(P<0.05).The score of skin cooling、fear of wind chill and morning stiffness was significantly improved compared with that before(P<0.05),and the difference between groups was not statistically significant(P>0.05).It is suggested that the two treatment methods are both effective in improving the symptoms of patients with wrist Median nerve entrapment syndrome, and the acupotomy therapy is better than the fine silver needle in improving the frequency, degree, swelling degree and joint flexion and extension of numbness. There is no significant difference between the two treatments in improving skin cooling, fear of wind and cold, and morning stiffness.4.After the treatment,28 cases in the treatment group were effective,2 cases were ineffective, and the total effective rate was 93.3%,while 24 cases in the control group were effective,6 cases were ineffective, and the total effective rate was 80%,suggesting that the efficacy of acupotomy therapy in the treatment of cold and dampness type of wrist Median nerve entrapment syndrome was better than that of fine silver needle therapy. Conclusion:Acupotomy therapy and fine silver needle therapy are effective in the treatment of wrist Median nerve entrapment syndrome with cold and dampness type;Acupotomy therapy has certain advantages in improving the numbness frequency, numbness degree, swelling degree and joint flexion and extension of patients.Acupotomy therapy has a significant effect on wrist Median nerve entrapment syndrome, which is worthy of further study and clinical promotion.
