Objective: This study established the Croton oil-induced anal edema model of rats,studied and discussed the anti-inflammatory and detumescence effect of compound Nepeta herb fumigation on this model,and analyzed the inflammatory response of compound Nepeta herb fumigation on mixed hemorrhoids and the inhibitory effect of inflammatory factors combined with clinical and postoperative case tissues.Methods:1.Experimental research By observing the morphological changes of anal swelling in SD rats induced by Croton oil,it is suggested that the anal edema model of SD rats is consistent with the changes of acute inflammation,which is similar to the inflammatory changes during the acute attack of mixed hemorrhoids.The contents of IL-6,TNF-α and iNOS were significantly different between the normal group and the model group,indicating that the croton oil-induced rat anal edema model was successfully established.There were significant differences in IL-6,TNF-α and iNOS between Nepeta formula group and Indomethate Supposal group.2.Clinical research The patients were screened according to the specified diagnostic criteria,inclusion criteria and exclusion criteria,and 126 cases were finally obtained.All enrolled patients were randomly assigned cards selected according to the principle of simple randomization,and the attending physicians were divided into blank group,indomethacin Shuan group and Nepeta formula group according to the serial number on the cards,with 42 cases in each group.Blank group received direct surgical treatment,treatment group was given Nepeta formula fumigation and sit-bath 3 days before surgery,once a day,and mixed hemorrhoidectomy 3 days later.Patients in Indomethacin suppository group were given indomethacin suppository to relieve anal cavity,once a day,each time,3 days before surgery.Mixed hemorrhoidectomy was performed 3 days later.The contents of IL-6,TNF-α and iNOS in hemorrhoids were determined by enzyme linked adsorption method after operation to objectively reflect the anti-inflammatory effect of Schizonepesta formula.Results:1.Experimental research By observing the morphological changes of anal swelling in SD rats induced by Croton oil,it is suggested that the anal edema model of SD rats is consistent with the changes of acute inflammation,which is similar to the inflammatory changes during the acute attack of mixed hemorrhoids.The contents of IL-6,TNF-α and iNOS were significantly different between the normal group and the model group(P<0.01),indicating that the croton oil-induced rat anal edema model was successfully established.There were significant differences in IL-6,TNF-α and iNOS between Nepeta formula group and Indomethate Supposal group(P<0.05).2.Clinical research There were no significant differences in gender and age among the three groups before medication,which were comparable.Compared with blank group,the contents of IL-6,TNF-α and iNOS in hemorrhoids tissue in Nepeta formula group and indomethate Suppositories group were decreased.There was no significant difference in IL-6 content between Nepeta formula group and Indomethate Supposal group(P>0.05),but there were significant differences in TNF-α and iNOS contents between them(P<0.05).Conclusion:1.Compound Nepetra herb lotion can significantly reduce the levels of IL-6,TNF-α and iNOS in perianal and anal tissues of rat hemorrhoids model induced by croton oil and in hemorrhoids tissues of clinical patients with mixed hemorrhoids,and has significant anti-inflammatory,swelling and analgesic effects.2.The mechanism of anti-inflammatory detumescence and analgesic action of compound Nepeta herb fumigation and lotion may be related to reducing the expression of IL-6 and TNF-α,inhibiting the activation of iNOS and reducing the release of NO,so as to achieve the purpose of anti-inflammatory detumescence and analgesic action,and provide ideas for further exploring the anti-inflammatory,analgesic and analgesic mechanism of compound Nepeta herb fumigation and lotion.3.No adverse reactions were found in the animal experiment and clinical observation of compound Schizonepeta fumigation lotion,and the efficacy is safe and stable,worthy of clinical promotion.
