Objective: To observe the clinical effect of fumigation and washing with traditional Chinese medicine combined with acupuncture on postoperative analgesia of damp-heat mixed hemorrhoids.Method: Study period: September 2021 to September 2022.Location: Department of Proctology,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine.Subjects: Patients with damp-heat mixed hemorrhoids meeting inclusion requirements.Sample size: 90 cases were randomly divided into observation group 1,observation group 2and control group,with 30 cases each.Research intervention measures: After external stripping and internal ligation operation,patients in 3 groups were given routine postoperative care.Observation group 1 received electroacupuncture treatment,observation group 2received traditional acupuncture treatment,and control group received conventional treatment.Treatment duration: 7 days.Research points: Upper,secondary,middle,lower and Chengshan points.The quantitative score(VAS)of the pain at the 6h,1st,3rd,5th and 7th days after surgery and the score of the traumatic edge edema at the 1st,3rd,5th and 7th days after surgery were recorded,the time of pain relief after the first dressing change(min),the time of first postoperative defecation and urination,the dose of postoperative pain medication,the length of hospital stay and the time of complete wound healing were recorded.After 7 days of treatment,the wound healing area,clinical effective rate,and the patient’s loss.Statistical software R 4.05 was used for statistical analysis of the research data.Results:1.Statistically,there was no significant statistical difference in the baseline data of the three groups of patients(p >0.05),so it could be compared.2.Variance analysis was performed on the incision pain scores of the 3 groups after surgery,and there was no statistical difference between the postoperative 6h and postoperative 7th day,p > 0.05.On the 1st,3rd and 5th day after surgery,p <0.05,there were statistical differences among the 3 groups.The differences in pain scores of the 3 groups were different or not completely the same.LSD-t test was used for further comparison,and p <0.05 was observed between group 1 and 2,group 1 and control group,and group 2 and control group.Postoperative anal pain scores in the same period of observation group 1 < observation group2 < control group(p <0.05).3.The first postoperative dressing change pain duration was compared(p < 0.05),and the difference between the three groups was different or not completely the same.Further comparison by LSD-t test showed that p values were all <0.05 between observation group 1and 2,group 1 and control group,and group 2 and control group.The pain duration of patients in the three groups after the first dressing change was observed in Group 1 <observation group 2 < control group.4.After the variance analysis on the scores of anal edema of patients in the 3 groups after mixed hemorrhoids surgery,it was found that on the 1st and 7th day after surgery,there was significant p > 0.05,which could be considered that there was no statistical difference in the scores of anal edema among the 3 groups.The comparison of edema scores on the 3rd and 5th days after surgery showed a significant p < 0.05,which could be considered as statistically significant,indicating that the differences of edema scores in the 3 groups were generally different or not completely the same.LSD-t test was used to compare edema scores on the 3rd and 5th days after surgery in pairs.Observation group 1 < observation group 2 < control group(p <0.05).5.Variance analysis was performed on the first postoperative defecation and urination time of the patients,p < 0.05,and the differences between the three groups of data were generally different or not completely the same.LSD-t test was used,and the first defecation time and first urination time of observation group 1 < observation group 2 < control group(p <0.05).6.Variance analysis was performed on the total dose of postoperative analgesics,p <0.05,the difference between the three groups was statistically significant,and the difference between the three groups was generally different or not completely the same.LSD-t test was used to observe the dose of postoperative analgesics in group 1 < Group 2 < control group(p <0.05).7.Variance analysis was performed on the days of postoperative hospitalization for patients with mixed hemorrhoids(p >0.05).The average days of hospitalization of patients with mixed hemorrhoids surgery in observation group 1 and group 2 was less than that of patients in control group,but there was no statistically significant difference between the three groups.8.Comparison of wound healing time,Chi-square test showed that p >0.05,and there was no statistical significance in the difference between the three groups.It could be considered that the wound healing time of the three groups was generally the same.9.After 7 days of treatment,the wound healing area of the three groups was compared,and the Chi-square test showed that p >0.05.There was no statistical significance in the difference between the three groups,which suggested that the wound healing area of the three groups was the same after treatment.10.After revisit and telephone follow-up,the clinical cure rate of the three groups was 100%,and all patients were cured eventually.11.During the experiment,no obvious adverse reactions occurred in patients in observation group 1 and control group,and only 1 case of mild needle sickness occurred in observation group 2.With the timely treatment and communication of physicians,the patient successfully completed the follow-up treatment,and no case of loss or withdrawal was reported in the 3groups.Conclusion:1.TCM fumigation combined with electroacupuncture and traditional acupuncture can play an effective analgesic role in the postoperative treatment of damp-heat mixed hemorrhoids.2.In terms of reducing postoperative pain score of mixed hemorrhoids,reducing postoperative edema score,reducing postoperative analgesic drug usage and first postoperative defecation time,the treatment of traditional Chinese medicine fumigation and washing combined with electroacupuncture has more advantages than traditional Chinese medicine fumigation and washing combined with traditional acupuncture.
