Clinical Study Of Wangshi Yangluan Decoction In The Treatment Of PCOS Anovulatory Infertility With Kidney Deficiency And Blood Stasis

Objective: To explore scientific and effective therapeutic ideas and plans for the treatment of this disease by observing the clinical efficacy of Wangshi Yangluan Decoction in the treatment of PCOS anovulatory infertility with kidney deficiency and blood stasis.Methods: Sixty cases meeting the diagnostic criteria of Chinese and Western medicine for PCOS anovulatory with kidney deficiency and blood stasis were selected and randomly divided equally into an observation group and a control group,with 30 cases in each group.Excluding the detached cases,a total of 57 patients actually participated in the clinical observation,including 29 cases in the observation group and 28 cases in the control group.In the control group,letrozole was given orally on day 5 of the menstrual cycle(or withdrawal bleeding)at 2.5 mg per day for 5 days;in the observation group,Wangshi Yangluan Decoction was added to the treatment in the control group,and was given orally on day 5 of the menstrual cycle(or withdrawal bleeding)at 1 dose per day,warmed twice in the morning and evening,for 7days.Patients in both groups were treated for three consecutive menstrual cycles.The TCM syndrome scores and sex hormone levels before and after treatment,and the number of ovulatory cycles and pregnancy cases after treatment were observed and recorded in both groups,and the overall clinical efficacy,TCM symptom efficacy,ovulation rate and pregnancy rate in both groups were calculated and counted.Results:1.There were no significant differences in age,duration of infertility,as well as sex hormone level and Chinese medicine symptom score before treatment between the two groups(P > 0.05),which were comparable.2.After treatment,comparing the comprehensive clinical efficacy of the two groups,it was found that the total clinical efficiency of the observation group and the control group were 89.7% and 78.6% respectively,and both groups achieved a high total clinical efficiency.Compared with the control group,the total clinical effective rate of the observation group was higher.Although there was no statistically significant difference between the two groups(P > 0.05),the level of sexual hormones,the rate of ovulation and pregnancy in the observation group were better than those in the control group(P < 0.05).It can be seen that both Wangshi Yangluan Decoction combined with letrozole and letrozole alone can effectively improve sex hormone levels,promote follicle maturation and discharge,and increase the chance of conception,but the former is more effective.3.After treatment,the TCM symptom scores of both groups decreased significantly compared with those before treatment(P < 0.01),and the TCM symptom scores of the observation group were significantly lower than those of the control group,with a very significant difference(P < 0.01).The total TCM efficiency of the observation group and the control group were 89.7% and46.4% respectively,and the total TCM efficiency of the observation group was higher,with a very significant difference(P < 0.01).Therefore,the combination of Wangshi Yangluan Decoction with letrozole and letrozole alone can effectively improve the traditional Chinese medicine symptoms of kidney deficiency and blood stasis in patients,but the former has better curative effect.4.After treatment,the LH,T and LH/FSH levels in both groups decreased significantly compared with those before treatment,and the difference was statistically significant(P < 0.05);FSH level in both groups increased compared with those before treatment,but the difference was statistically significant only in the observation group(P < 0.05).After treatment,LH,T,and LH/FSH levels in the observation group were significantly lower than those in the control group,and the difference was significant(P < 0.05);FSH level in the observation group was higher than that in the control group,but the difference was not statistically significant(P > 0.05).It can be seen that the sex hormone levels of the two groups improved after treatment compared with those before treatment,but the data of the observation group were better than those of the control group,indicating that Wangshi Yangluan Decoction combined with letrozole has better efficacy in regulating hormone levels compared with single letrozole.5.After treatment,the ovulation rate of the observation group was 77.9%and the pregnancy rate was 62.1%;the ovulation rate of the control group was63.3% and the pregnancy rate was 32.1%.The ovulation rate and pregnancy rate of the observation group were better than those of the control group,with significant differences(P < 0.05).Therefore,compared with single letrozole,Wangshi Yangluan Decoction combined with letrozole can more effectively improve the ovulation rate and pregnancy rate in patients with PCOS anovulatory infertility.6.During the treatment period,no serious adverse effects were observed in both groups.Conclution: The combination of Wangshi Yangluan Decoction and letrozole is more effective than letrozole alone in the treatment of PCOS anovulatory infertility with kidney deficiency and blood stasis,which can significantly improve the symptoms of kidney deficiency and blood stasis and regulate the sex hormone level,resulting in higher ovulation rate and clinical pregnancy rate.
