Observation On The Clinical Curative Effect Of The Treatment Of DKD (Qi And Blood Deficiency) Based On The "State-target Strategy" With The Decoction Of Angelicae Sinensis For Enriching Blood And Jingui Shenqi Pill

Objective:This study was based on the academic concept of "state-target strategy".By observing the clinical curative effect of Decoction of Angelicae Sinensis for Enriching Blood combined with Jingui Shenqi Pill in the treatment of patients with stage G1-G3 b type 2 diabetic kidney disease with deficiency of both qi and blood,we objectively evaluated its clinical curative effect and safety,and provided new ideas to slow down the progression of diabetic kidney disease.Methods:Sixty four patients with stage G1-G3 b type 2 diabetic kidney disease with deficiency of both qi and blood who met the inclusion criteria were collected from the outpatient clinic and ward of the Department of Endocrinology,Shandong Province Hospital of Traditional Chinese Medicine,and randomly divided into a control group and a treatment group,with 32 cases in each group.The control group was given general treatment combined with conventional western medicine,while the treatment group was treated with Decoction of Angelicae Sinensis for Enriching Blood combined with Jingui Shenqi Pill on top of the control group.All treatment courses are 8 weeks.In this study,we observed and recorded the renal function:urinary albumin/creatinine ratio(UACR),serum creatinine(Scr),blood urea nitrogen(BUN),estimated glomerular filtration rate(e GFR),Retinol binding protein(RBP),micro-albumin(MALB);Glucose metabolism: fasting blood glucose(FPG),glycosylated hemoglobin(HbAl C);Lipid metabolism: total cholesterol(TC),triglyceride(TG),high density lipoprotein(HDL-C),low density lipoprotein(LDL-C);blood pressure: systolic blood pressure(SBP),diastolic blood pressure(DBP);safety indexes and TCM syndrome score before and after treatment in both groups.Finally used SPSS 26.0 to statistically analyze the above indexes data to evaluate the efficacy and safety of Decoction of Angelicae Sinensis for Enriching Blood combined with Jingui Shenqi Pill.Results:1.There were no statistically significant differences in gender,age,BMI,disease duration,glucose and lipid metabolism,blood pressure,renal function and TCM syndrome score between patients in the treatment and control groups before treatment(P>0.05),which were comparable.2.Glucose metabolism: Compared with before treatment,FPG and HbA1 c in the two groups was significant improved after treatment(P <0.01);Compared with the differences between treatment and control groups,the HbA1 c were significantly improved(P < 0.01),The FPG were no statistically significant difference(P>0.05).3.Lipid metabolism: Compared with before treatment,TC,TG,LDL-C in the two groups and HDL-C in the treatment group was significant improved after treatment(P<0.01),HDL-C in the control group were no statistically significant difference(P>0.05).Compared with the differences between treatment and control groups,the LDL-C,HDL-C were significantly improved(P<0.01),The TC,TG were no statistically significant difference(P>0.05).4.Blood pressure:Compared with before treatment,SBP in the two groups and DBP in the control group was significant improved after treatment(P<0.01),DBP in the treatment group were no statistically significant difference(P>0.05).Compared with the differences between treatment and control groups,the SBP,DBP were no statistically significant difference(P>0.05).5.Renal function: Compared with before treatment,Scr,BUN,MALB,RBP in the two groups and e GFR in the treatment group was significant improved after treatment(P<0.01),UACR in the two groups and e GFR in the control group was improved after treatment(P<0.05).Compared with the differences between treatment and control groups,the Scr,BUN,e GFR were significantly improved(P<0.01),The RBP,MALB,UACR were no statistically significant difference(P>0.05).6.TCM treatment efficiency: TCM symptom efficiency of the treatment group(86.67%)was better than that of the control group(73.33%),and differences were statistically significant(P<0.05).7.TCM symptom scores: the improvement of TCM symptom scores in the treatment group was better than that in the control group,and there were statistical differences within the group(P < 0.01)and between the groups(P < 0.05).Conclusion:1.The Decoction of Angelicae Sinensis for Enriching Blood and Jingui Shenqi Pill combined with conventional western medicine is significantly effective in treating patients with stage G1-G3 b type 2diabetic kidney disease with deficiency of both qi and blood,and has better effect in improving the efficacy and alleviating the clinical symptoms of patients compared with the control group.2.After treatment,FPG,HbA1 c,TC,TG,HDL-C,LDL-C,SBP,Scr,BUN,SBP,MALB,e GFR,and RBP were all improved by combining the Decoction of Angelicae Sinensis for Enriching Blood and Jingui Shenqi Pill with conventional western medicine,suggesting that the Decoction of Angelicae Sinensis for Enriching Blood and Jingui Shenqi Pillhas definite efficacy in regulating Glucose and lipid metabolism,lowering blood pressure,and improving renal function.3.The combination of the Decoction of Angelicae Sinensis for Enriching Blood and Jingui Shenqi Pill with conventional western medicine treatment could better improve HbA1 c,HDL-C,LDL-C,Scr,BUN and e GFR levels compared with western medicine treatment alone.4.During this clinical study,no significant toxic side effects were found with the combination of the Decoction of Angelicae Sinensis for Enriching Blood and Jingui Shenqi Pill,which is safe and worthy of clinical promotion.
