Clinical Study On Acupuncture And Moxibustion Treatment Of Neurogenic Tinnitus Of Weak Spleen And Stomach Type Guided By The Theory Of Qi Movement Ascending And Descending

Objective: To observe the clinical efficacy of acupuncture and moxibustion treatment of neurogenic tinnitus of weak spleen and stomach type guided by the theory of qi movement ascending and descending,and compare it with conventional acupuncture treatment to discuss the new ideas and explore a better scheme for acupuncture treatment of this disease.Methods: Sixty patients with neurogenic tinnitus of weak spleen and stomach type who met the inclusion criteria and exclusion criteria were randomly assigned and equally divided into 30 patients each of the observation group and the control group.The observation group was treated with acupuncture combined with grain moxibustion under the guidance of the theory of qi movement ascending and descending: 1.Filiform acupuncture: acupoints:Zhongwan,Tianshu,Yinlingquan,Gongsun,Zusanli;Baihui,Qihai,Guanyuan,Hegu,Taichong;Ermen,Tinggong,Tinghui and Yifeng of the suffering side;Operation: after needle injection and getting qi,apply the method of leveling,reinforcing and reducing,and kept the needle for 30 minutes,during which the needle was injected once every 10 minutes;2.Grain moxibustion: acupoints: Zhongwan,Qihai,Guanyuan,Tianshu and Ermen,Tinggong,Tinghui,Yifeng of the suffering sides;operation: place the grain-sized moxa cone at the acupoint and ignite it,when the patient has burning pain,replace the moxa cone,each acupoint moxibustion is 3 strong.The control group was treated with routine acupuncture combined with grain moxibustion: 1.Filiform acupuncture: acupoints: Ermen,Tinggong,Tinghui and Yifeng of the suffering side,match the Pishu,Zusanli,Qihai,the operation is the same as the observation group;2.Grain moxibustion:acupoints and the operation are the same as the observation group.Course of treatment: both groups were treated once a day,one week as a course of treatment,and one day off on Sunday.The curative effect was observed after four consecutive courses of treatment.Evaluation method: Before and after treatment,the efficacy was evaluated by using the tinnitus evaluation questionnaire(TEQ),tinnitus handicap inventory(THI),hospital anxiety and depression scale(HADS),tinnitus loudness visual analogue scale(VAS),and TCM syndrome score,and the relevant data were statistically analyzed by SPSS version 26.0 software.Result: 1.Before treatment,there was no significant difference between the two groups in terms of gender,age,course of disease,TEQ score,THI score,HADS score,tinnitus loudness VAS score,and TCM symptom score(P>0.05),which was balanced and comparable.2.After treatment,the total effective rate of the observation group was93.10%,and the total effective rate of the control group was 86.21%.The clinical efficacy of the two groups(cured,markedly effective,effective and ineffective)was tested by rank sum test,the treatment effect of the observation group was better than that of the control group,with a statistically significant difference(P<0.05).3.After treatment,intra-group comparison showed that the TEQ score,THI score,HADS score,tinnitus loudness VAS score and TCM syndrome score of the two groups were significantly lower than those before treatment,with a statistically significant difference(P<0.05).4.After treatment,the reduction effect of TEQ score,THI functional and emotional score,HADS subscale score and total score,tinnitus loudness VAS score,and TCM syndrome score in the observation group was significantly better than that in the control group,with a statistically significant difference(P<0.05);There was no significant difference between the two groups in reducing the severity score of THI(P>0.05).Conclusion: Based on the guidance of the theory of ascending and descending of qi,acupuncture combined with grain moxibustion treatment spleen and stomach weakness type nerve tinnitus has definite clinical curative effect,the therapy in improve the overall curative effect,shorten the tinnitus time,reduce tinnitus,reduce the influence of psychological and sleep,improve the overall feeling of tinnitus patients have more advantages compared with conventional acupuncture.
