Clinical Effect Observation Of Timing Auricular Scraping On Idiopathic Tinnitus Patiets With Spleen And Stomach Weakness

Objective:To explore the clinical treatment effect of auricular scraping on idiopathic tinnitus patients with spleen and stomach weakness,and under the guidance of midnight-noon ebb-flow theory,analyze the therapeutic advantages of timing auricular scraping.In order to enrich the traditional Chinese medicine nursing therapy of idiopathic tinnitus with spleen and stomach weakness,and make clear the best time to carry out the auricular scraping operation,and provide theoretical and practical basis for the promotion and application of auricular scraping technology.Methods:In this study,convenient sampling was used,according to the inclusion criteria and exclusion criteria,132 idiopathic tinnitus patients with spleen and stomach weakness were selected who admitted in the otolaryngology department of a tertiary hospital in Harbin from January to December 2022.The study subjects were randomly divided into four groups and the intervention was as follows.The control group received basic treatment and usual hospital care.The non-timing group received auricular scraping at any time except Chenshi and Sishi on the basis of the control group.The Chenshi group received auricular scraping at Chenshi(7:00 ～9:00)on the basis of the control group.The Sishi group received auricular scraping at Sishi(9:00～11 :00)on the basis of the control group.Each auricular scraping lasted for 30 minutes.The frequency was 3 days once,4 times for a course of treatment,a total of 1 course of intervention.Before and after the intervention,patients filled in the Tinnitus Handicap Inventory(THI),TCM syndrome score,Pinsburgh Sleep Quality Index(PSQI),and accepted Pure tone audiometry(PTA).Clinical efficiency was determined according to the recovery degree of tinnitus severity.Data collected from the clinical trial were double checked by two people and entered in the Excel form.Statistical analysis was performed using the SPSS 26.0 software.Results:1.Before the intervention,the THI scale score,TCM syndrome score,PSQI scale score and PTA results of the four groups were compared,and no differences were statistically significant(all P>0.05),indicating that the baseline levels of the four groups were basically consistent and comparable.2.Before and after the intervention,the THI scale score,TCM syndrome score,PSQI scale score and PTA results of the four groups were compared,and all four groups were better after the intervention than before the intervention,with statistically significant differences(all P<0.05).3.After the intervention,the THI scale score,TCM syndrome score,PSQI scale score and PTA results of the four groups were compared,the differences were statistically significant(all P<0.05).Further comparison revealed that: The control group compared with the other three groups,the differences in the THI scale score,TCM syndrome score,PSQI scale score and PTA results were statistically significant(all P<0.05).The non-timing group compared with the Chenshi group and the Sishi group,there’s no statistically significant difference in the THI scale score and PTA results(all P>0.05),and the differences in TCM syndrome score and PSQI scale score were statistically significant(all P<0.05).There was no significant difference between the Chenshi group and the Sishi group(all P>0.05).4.The total effective rate in the four groups were 80.65 %,84.38%,90.90%,and 90.90%,with statistically significant difference(P<0.01).Further comparison revealed that: the control group compared with the non-timing group,Chenshi group and Sishi group,the differences were statistically(P<0.01).There was no statistically significant difference between the remaining groups(all P>0.05).Conclusions:1.Auricular scraping has a certain clinical effect for patients with idiopathic tinnitus with spleen and stomach weakness,which can improve the severity of tinnitus,Chinese medicine syndrome and sleep quality,and reduce hearing loss.2.T iming auricular scraping on the basis of midnight-noon ebb-flow theory has certain clinical therapeutic advantages.The effect of improving TCM syndrome and sleep quality in idiopathic tinnitus patients with spleen and stomach weakness is more significant operating at the time of Chenshi(7:00～9:00)and Sishi(9:00～11:00).
