Study On The Clinical Experience And Medication Rules Of Traditional Chinese Medicine Master Zhang Lei In Treating Spleen And Stomach Diseases

Objective: Through the study of the medical record and theoretical thought of traditional Chinese medicine master Zhang Lei in treating spleen and stomach diseases,the clinical experience and medication rules of Professor Zhang Lei in treating spleen and stomach diseases were objectively analyzed and summarized.It is expected to provide more ideas and references for the diagnosis and treatment of spleen and stomach diseases with traditional Chinese medicine.Methods: 1.To collect the relevant medical records of spleen and stomach diseases by consulting "Medical Library of Traditional Chinese Medicine Zhang Lei","Zhang Lei’s Medical book","Zhang Lei’s Clinical Experience Collection" and the outpatient medical records of Zhang Lei’s famous medical studio from January 2018 to August 2022.To use Excel to input all medical record data and establish the professor’s medical record database,and input the basic information of patients in the medical record,four diagnostic information and prescription medication.The database of medical records of spleen and stomach diseases was established after classification and screening.2.Using data mining technology to study the medication rules of relevant medical records.Refer to "Symptom Differentiation and Diagnostics of Traditional Chinese Medicine(Second Edition)","the Terminology of Common Clinical Symptoms of Traditional Chinese Medicine","Diagnostics of Traditional Chinese Medicine" to standardize the description of symptoms in medical records.The name of medicine in the prescription was standardized according to "Pharmacopoeia of the People’s Republic of China(2020)" and "traditional Chinese Pharmacology".Excel was used to make frequency statistics of basic and four diagnostic information and medicine.SPSS Modeler 18 was used for association rule analysis.SPSS Statistics 26 was used for system cluster analysis and factor analysis.3.The relevant theoretical and empirical data in Professor Zhang Lei ’s monographs were collated and studied,and the clinical experience of the professor in treating spleen and stomach disease was summarized.Results: 1.A total of 698 medical records,698 patients and 1332 prescriptions were included.There were more female patients than male patients.Age distribution was mainly middle-aged and elderly.The symptoms of anorexia,loose stool and insomnia were the most common.Red tongue,pale red tongue,red tongue and other tongues were the most common.Thin white fur,thin yellow fur,yellow greasy fur,white greasy fur and other tongue fur were the most common.The utilization rate of Pinelliae Rhizoma Praeparatum Cum Alumine(PRPCA)was the highest of traditional Chinese medicine.Banxia Xiexin Decoction was the most commonly used prescription.The four qi were mostly warm,flat,cold and slightly cold.The taste of the medicine was mostly sweet,spicy and bitter.The spleen,lung and stomach meridians were main meridians.The average prescription was about10 herbs.2.The core medicine for stomach distention and fullness were PRPCA,Rhizoma Coptidis,Scutellariae Radix,Massa Medicata Fermentata,Codonopsis Radix,Poria and Glycyrrhizae Radix Et Rhizoma Praeparata Cum Melle.A total of 39 association rules,5 core combinations and 9common factors were obtained.Banxia Xiexin Decoction and Yueju Pill were commonly used.The core medicine for stomachache were Salviae Miltiorrhizae Radix Et Rhizoma,Amomi Fructus,Santali Albi Lignum,PRPCA,Scutellariae Radix,Codonopsis Radix,Bupleuri Radix,Cyperi Rhizoma and Scutellariae Radix.A total of 35 association rules,5 core combinations and 9 common factors were obtained.Danshen Yin,Banxia Xiexin Decoction and Yueju Pill were commonly used.The core medicine for diarrhea were Codonopsis Radix,stir-baked Atractylodis Macrocephalae Rhizoma,Glycyrrhizae Radix Et Rhizoma Praeparata Cum Melle,Poria,raw and stir-baked Crataegi Fructus,raw and stir-baked Plantaginis Semen.A total of 38 association rules,6 core combinations and 8 common factors were obtained.Shanqian decoction,Tongxie prescription and Xiangsha Liujunzi decoction were commonly used.The core medicine for abdominal pain were stir-baked Atractylodis Macrocephalae Rhizoma,Poria,Angelicae Sinensis Radix,Paeoniae Radix Alba,Scutellariae Radix,PRPCA and Chuanxiong Rhizoma.A total of 22 association rules,5 core combinations and 9 common factors were obtained,and Danggui Shaoyao Powder was commonly used.The core medicine for constipation were Rhei Radix Et Rhizoma,stir-baked Cannabis Fructus,Paeoniae Radix Alba,Magnoliae Officinalis Cortex,Armeniacae Semen Amarum and Aurantii Fructus Immaturus.A total of 28 association rules,4 core combinations and 6 common factors were obtained,Maziren Pill and Xuefu Zhuyu Decoction were commonly used.The core medicine for vomiting were PRPCA,Citri Reticulatae Pericarpium,Codonopsis Radix,Scutellariae Radix,Bambusae Caulis In Taenias and Bupleuri Radix.A total of 20 association rules,4 core combinations and 6common factors were obtained.Banxia Xiexin Decoction,Dachaihu Decoction and Jupi Zhuru Decoction were commonly used.The core medicine for hiccups were PRPCA,Codonopsis Radix,Glycyrrhizae Radix Et Rhizoma,Kaki Calyx,Ophiopogonis Radix,Jujubae Fructus,Citri Reticulatae Pericarpium and Paeoniae Radix Alba.A total of 24 association rules and 4 core combinations were obtained.Banxia Xiexin Decoction,Zhuye Shigao Decoction and Sanhuang Xiexin Decoction were commonly used.The core medicine for dysphagia were Gekko,Notoginseng Radix Et Rhizoma,PRPCA,Glycyrrhizae Radix Et Rhizoma and Codonopsis Radix.A total of 32 association rules and 5 core combinations were obtained.Xiangsha Liujunzi Decoction,Huqi Powder and Qianjin Weijing Decoction were commonly used.3.Professor Zhang Lei believes that the pathogenesis of spleen and stomach disease is often mixed with cold and heat,and long-term disease is often accompanied by qi stagnation and blood stasis.He puts forward that ’the treatment of spleen disease lies in transportation,and the treatment of stomach disease lies in unblocking’.The key to treatment is to transport the spleen to move stagnation,unblock the stomach and regulate qi activity.The prescription is ingenious and the curative effect is remarkable.Conclusion: 1.The common pathogenesis of spleen and stomach diseases in this study is characterized by cold and heat,deficiency in origin and excess in superficiality.The common treatment methods are acrid opening and bitter descending,cold and heat regulating,soothing the liver and regulating the spleen,promoting blood circulation and removing blood stasis,strengthening the body resistance and eliminating pathogenic factors,commonly used Banxia Xiexin Decoction,Xiaoyao Powder,Chaihu Shugan Powder.Commonly used PRPCA,Codonopsis Radix,Poria,Scutellariae Radix,Glycyrrhizae Radix Et Rhizoma,Scutellariae Radix,stir-baked Atractylodis Macrocephalae Rhizoma,Glycyrrhizae Radix Et Rhizoma Praeparata Cum Melle,Bupleuri Radix and Paeoniae Radix Alba.2.The medication of Stomach distention and fullness medication reflects the characteristics of transportation.Stomachache medication was reflected activating without stagnation.Diarrhea medication was reflected regulating yin and yang.Abdominal pain medication was reflected dredging stagnation.Constipation medication was reflected rigid and soft,Qi and blood treatment.Vomiting medication was reflected reconciliation and descending.Hiccup medication was reflected cold and heat balance,dispelling evil and strengthening the body.Dysphagia medication was reflected removing turbidity,dispelling evil and strengthening healthy qi.3.Professor Zhang Lei’s treatment of spleen and stomach diseases has the characteristics of distinguishing the pathogenesis,treating the disease and seeking the root,flexibly using the ancient prescriptions,and being good at establishing new prescriptions.It embodies the characteristics of transportation,regulating qi movement,refining medication and remarkable curative effect.
