Study On The Construction Of Evaluation Index Of Nurse Core Competence In Anesthesia Resuscitation Room

Objective: To construct a set of evaluation indexes of the core competence of nurses in anesthesia resuscitation room,which are suitable for the development of anesthesia nursing in China and have special characteristics,and to provide theoretical basis and reference for the training,assessment and use of nurses in anesthesia resuscitation room.Methods:(1)Through literature review,expert interview and research group discussion,the evaluation index framework of anesthesia resuscitation room nurse core competency was developed;Draft evaluation index items of core competence of nurses in anesthesia resuscitation room were formed,and Delphi expert correspondence questionnaire was formulated accordingly.(2)Delphi method was used to carry out expert letter consultation.A total of 26 experts from five grade-III,Grade-A general hospitals in Jiangsu Province were selected for two rounds of letter consultation.Indicators were deleted,modified and selected,and evaluation indicators of the core competence of nurses in the resuscitation room were established.(3)The third Round of Expert Correspondence: Using the analytic hierarchy process(ahp)construct the anesthesia resuscitation room nurse core competency evaluation index hierarchy model,create two indexes comparative judgment matrix,prepare and issue 3 rounds of enquiry for questionnaire,compare two importance between the two indicators,calculate and determine the weight coefficient of index,at the same time for the construction of the two indicators more consistency of judgment matrix.Results: Established the anesthesia resuscitation room nurse core competency evaluation index including seven primary index(professional theoretical knowledge,professional clinical practice ability,communication and coordination ability,critical thinking ability,nursing management ability,professional development ability and attitude and personality),27 secondary indexes,74three-level index,using the analytic hierarchy process(ahp),calculate and determine the weight coefficient of index.Expert enthusiasm: the effective recovery rate of the questionnaire in the three rounds of expert letter consultation was 100%,indicating high expert enthusiasm.Expert authority: expert judgment coefficient is 0.873,familiarity coefficient is 0.823,and authority coefficient is 0.848>0.70,indicating high expert authority.Degree of expert coordination: in the first round,the Kendal harmony coefficient of the first,second and third indexes was 0.158,0.265 and 0.179,and the total Kendal harmony coefficient was 0.195.In the second round,the Kendal harmony coefficient of the first,second and third indexes was 0.308,0.308 and 0.362,and the total Kendal harmony coefficient was 0.363.The difference was statistically significant(P<0.001),and the expert coordination degree was high.Degree of concentration of expert opinions: in the first Round of Delphi letter consultation,the mean value of importance assigned to indicators ranged from 3.846 to 4.962,and the standard deviation ranged from 0.196 to 1.156.In the second round of Delphi letter consultation,the mean value of importance assignment of indicators ranged from 3.923 to 5,and the standard deviation ranged from 0 to 1.067.In addition,the overall index met "mean value of importance assignment ≥3.5 and coefficient of variation ≤0.25",indicating a high degree of concentration of expert opinions.Conclusion: The evaluation index of the anesthesia resuscitation room nurse core competency constructed in this study is scientific,reliable,specific,comprehensive and systematic,and has guiding significance for the training,assessment and use of nurses in anesthesia resuscitation room.
