Analysis Of Anxiety Status And Influencing Factors Of Rural Chronic Disease Patients In Youjiang District Under Epidemic Prevention And Control

Objective:To understand the anxiety status of rural Chronic Disease patients in Youjiang District,Baise City,Guangxi under epidemic prevention and control,and analyze related psychological risk factors,so as to provide scientific basis for the management of rural patients with Noninfectious chronic disease(hereinafter referred to as"chronic disease").Methods:A total of 1073 residents suffering from chronic diseases in three rural areas of Youjiang District,Baise City,Guangxi from February 2022 to May 2022 were selected as the objects of this survey by the multi-stage cluster sampling method.The demographic and sociological data,family data,chronic disease data and epidemic related data of the patients were collected.According to Generalized anxiety disorder 7(GAD-7),patients with chronic diseases can be divided into chronic anxiety group(GAD-7 score≥5points)and chronic non-anxiety group(GAD-7 score<5 points).SPASS25.0 software was used to compare the differences between the two groups in gender,age,address,marriage,debt,family structure,type of chronic diseases,course of disease,random blood sugar,random blood pressure,knowledge of chronic diseases,standard vaccination of novel coronavirus vaccine,and acceptance of knowledge of epidemic prevention and control.The possible risk factors of anxiety were analyzed by logistic regression model.Results:A total of 1073 patients with chronic diseases were included in this study.After data collection and analysis,it was found that:(1)There were 485 patients(45.2%)in the anxiety group,including 324 patients(30.2%)with mild anxiety,160 patients(14.9%)with moderate anxiety,and 1 patient(0.1%)with severe anxiety.There were 588 cases(54.8%)in the non-anxious group.(2)According to the analysis of family structure and anxiety and non-anxiety,174 cases(44.5%)of compound family,173 cases(39.1%)of lineal family,60cases(44.1%)of nuclear family,78 cases(75.0%)of incomplete family had anxiety.After comparison,The difference was statistically significant(χ2=47.243,P<0.05),indicating that the prevalence of anxiety in patients with direct family was lower,while the prevalence of anxiety in patients with incomplete family was significantly higher than that in other family structures.(3)According to the types of chronic diseases and the analysis of anxiety and non-anxiety,460 cases(44.2%)of patients with only 1 chronic disease accompanied by anxiety,25 cases(78.1%)of patients with≥2 chronic diseases accompanied by anxiety,after comparison,the difference between the two groups was statistically significant(χ2=14.435,P<0.05).This indicates that the prevalence of anxiety in patients with≥2 chronic diseases is higher than that in patients with 1 chronic disease.(4)Patients in the anxious group and the non-anxious group were compared.The results showed that:(1)The age between the two groups(χ2=21.037,P<0.001),the difference was statistically significant(P<0.05);There was no significant difference in gender(χ2=3.550,P=0.060)(P>0.05).(2)There were significant differences in address(χ2=21.037,P<0.001),marriage(χ2=35.547,P<0.001),debt(χ2=143.979,P<0.001)and family structure(χ2=47.243,P<0.001)between the two groups(all P<0.05).(3)The types of chronic diseases between the two groups(χ2=14.435,P<0.001),random blood glucose(Z=-6.268,P<0.001),random systolic blood pressure(Z=-13.329,P<0.001),random diastolic blood pressure(Z=-12.055,P<0.001),and knowledge of chronic diseases(χ2=11.120,P=0.004),The differences were statistically significant(all P<0.05);There was no significant difference in disease duration(χ2=0.243,P=0.886)(P>0.05).(4)The difference between the two groups was statistically significant in the standard vaccination of novel coronavirus vaccine(χ2=14.495,P=0.009),while there was no statistically significant difference in the acceptance of epidemic prevention and control knowledge education(χ2=2.892,P=0.235)(P>0.05).(5)Among chronic patients,anxiety score was positively correlated with marriage,debt,random blood glucose,random systolic blood pressure and random diastolic blood pressure(marriage r=0.229,debt r=0.396,random blood glucose r=0.191,random systolic blood pressure r=0.429,random diastolic blood pressure r=0.387;P<0.05);It was negatively correlated with age and knowledge level of chronic diseases(age=-0.073,knowledge level of chronic diseases r=-0.112;P<0.05).(6)The results of multivariate logistic regression analysis showed that:Have debts(OR=8.521,95%CI:4.876-14.894),≥2 chronic diseases(OR=2.825,95%CI:1.068-7.474),random elevated blood glucose(OR=1.110,95%CI:1.057-1.168),random elevated systolic blood pressure(OR=1.036,95%CI:1.024-1.048),increasedrandomdiastolicbloodpressure(OR=1.026,95%CI:1.010-1.043),all of which were the risk factors for anxiety in rural chronic disease patients in Youjiang District under epidemic prevention and control(all P<0.05).Conclusion:(1)Rural patients with chronic diseases in Youjiang District,Baise City,Guangxi are more prone to anxiety under epidemic prevention and control,but mild anxiety is more common,which should be paid attention to in chronic disease management.(2)Family resources play an indispensable role in the management of patients with chronic diseases.General practitioners can use family resources to manage chronic diseases.(3)Patients with chronic diseases are more likely to suffer from anxiety if they have more diseases and poor control of blood sugar and blood pressure.Grassroots general practitioners should achieve early prevention,advance screening and timely necessary psychological intervention.
