Effect Of Compound Tianma Antihypertensive Granule On Essential Hypertension And Left Ventricular Diastolic Function

Objective: The aim of this study was to investigate the clinical efficacy and effect on left ventricular diastolic function of compound Tianma antihypertensive granules in the treatment of patients with essential hypertension(Yin deficiency and Yang hyperactivity),and to further investigate the role of compound Tianma antihypertensive granules in target organ damage of heart.Methods: A total of sixty patients with yin-deficiency and yang-hyperactivity type essential hypertension were randomly divided into control group and treatment group,with 30 patients in each group.Among them,The control group received oral amlodipine benzoate and/or valsartan tablets,while the treatment group added Tianma antihypertensive granules on the basis of western medicine,After 12 weeks of treatment,both groups were compared before and after treatment for incident blood pressure(CBP),24 h ambulatory blood pressure(24h SBP,24 h DBP,DSBP,DDBP,NSBP,NDBP),cardiac color Doppler related indexes(LVDd,IVST,LVPWT,LVMI,E,A,E,E/A,E/E),Ang II(angiotensin II),TGF before and after treatment-β1(Transformation growth factor β1),TNF-a(tumor necrosis factor a),curative effect of TCM syndrome,integral of TCM syndrome,safety evaluation and other indicators,and systematically evaluate its curative effect.Results: 1.Blood pressure: After the course of treatment,the levels of CBP,24 h SBP,24 h DBP,DSBP,DDBP,NSBP and NDBP were lower in both groups than before treatment P<0.05),and the efficacy of the treatment group was significantly higher than that of the control group(P<0.05).2.In terms of cardiac color Doppler index: After the course of treatment,LVDd,IVST,LVPWT,LVMI,A and E/E’ decreased to different degrees in both groups,and the decrease was more obvious in the treatment group(P<0.05),which was statistically significant;E,E’and E/A increased to different degrees in both groups,and the increase was more obvious in the treatment group(P<0.05),which was statistically significant.3.Regarding the efficacy and TCM symptom scores: TCM symptom scores in both groups decreased after treatment compared to before treatment,with(6.76±2.24)scores in the treatment group and(9.73±2.05)scores in the control group,and the decrease in the treatment group was significantly higher than the control group(P<0.05),which was statistically significant.The overall performance score in the treatment group was93.33% and in the control group was 76.66%,which was better than the control group and was statistically significant(P<0.05).4.Relevant laboratory indicators: after the treatment course,Ang II,TGF-β1 and TNF-a values decreased to different degrees in both groups,and the reduction effect was more significant in the treatment group compared with the control group(P<0.05),which was statistically significant.5.Safety indicators: Safety indicators: There was one case of mild loss of appetite in the treatment group during the study period,and there was no significant change in the safety indicators of liver and kidney function before and after the treatment(P>0.05).Conclusion: In patients with essential hypertension with yin-deficiency and yang-hyperactivity type,the effect of adding compound Tianma antihypertensive granule tothe treatment of western medicine is more obvious in reducing blood pressure and improving clinical symptoms,and can delay ventricular remodeling and improve left ventricular diastolic function.The mechanism may be related to the reduction of AngⅡ and TGF-β1.TNF-a level is related,clinical application is safe,and has promotion value.
