Analysis Of Professor Zhou Yabin’s Medication Rule In The Treatment Of Patients With Coronary Heart Disease Tachyarrhythmia

Objective:To study the prescription medication of Professor Zhou Yabin in the treatment of tachyarrhythmia of coronary heart disease,analyze the distribution of TCM syndromes,the frequency of drug use,and the association rules between drugs in the included case samples,and summarize the medication rules of Professor Zhou Yabin in the treatment of tachyarrhythmia of coronary heart disease,in order to provide reference for the systematic treatment of the disease in traditional Chinese medicine.Methods:The case data of coronary heart disease tachyarrhythmia diagnosed and treated by Professor Zhou Yabin in the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine from September2019 to September 2022 were collected.A total of 131 medical records that met the inclusion criteria and had complete cases were selected.After extracting and standardizing the relevant data,a database was established.The TCMISS V3.0 was used to count the general data,four diagnostic information,medication rules,medicinal properties,medicinal taste,meridian tropism and efficacy of patients,and association rules and cluster analysis were performed.According to the results,the medication rules of Professor Zhou Yabin in the treatment of coronary heart disease tachyarrhythmia were summarized.Results:1.General situation:Among the 131 cases included,87 were female and44 were male.After syndrome differentiation,they were divided into heart and spleen deficiency and yin deficiency and fire excess syndrome,heart blood stasis syndrome,phlegm fire disturbing heart syndrome,heart deficiency and timidity syndrome,water and qi attacking heart syndrome.2.Symptoms and tongue and pulse conditions:The main symptoms of the131 cases included were palpitations,chest pain,chest tightness,poor sleep,fatigue,upset,anorexia,shortness of breath,hot sweating,white fur,thin fur,pale red tongue,red tongue,fat tongue,weak pulse,thin pulse,string pulse,pulse number,slippery pulse.On the basis of the main syndromes,the syndrome of deficiency of heart and spleen focuses on fatigue,shortness of breath,poor appetite and poor sleep.On the tongue image,the tongue is pink,the coating is thin and white,and the tongue is fat and large,with teeth marks.The pulse condition is mainly weak and thin.On the basis of the main syndromes,the syndrome of yin deficiency and fire excess is most obvious with hot flashes,sweating,hot flashes,dry mouth,upset,night sweats and bitter mouth.The tongue is more red and less moss,and the pulse is mainly pulse count.On the basis of the main syndromes,the pain nature of heart blood stasis syndrome focuses on tingling and is often accompanied by qi stagnation syndromes such as flank distension and abdominal distension.The tongue is multicolored purple,with occasional ecchymosis,and the pulse is often stringy and astringent.On the basis of the main syndromes,the phlegm-fire disturbing heart syndrome focuses on dreaminess,upset,bitter mouth,easy to wake up and irritability.The tongue image is mainly red tongue,yellow and thick greasy fur,and the pulse is slippery or string slippery.On the basis of the main syndromes,the heart deficiency and timidity syndrome is often accompanied by timidity and sweating.The tongue image is pale red tongue,thin white fur,weak or weak string pulse.The syndrome of water-qi lingxin is more common in edema of both lower limbs,dysuria,difficulty in recumbent at night and other symptoms.The tongue image is more common in pale red tongue,white and thick greasy fur or slippery fur,and stringy pulse.3.Prescription medication : 131 prescriptions were included,involving132 traditional Chinese medicines,with a total frequency of 2928 times.The most frequently used traditional Chinese medicines were Pinellia ternata,Astragalus membranaceus,Codonopsis pilosula,Glycyrrhiza uralensis,Poria cocos,Scutellaria baicalensis,Longgu,Oyster,Bupleurum,Cinnamomi Ramulus,Paeoniae Radix Alba,Rhubarb.Among them,89 kinds of traditional Chinese medicines were involved in the syndrome of deficiency of both heart and spleen,and the cumulative frequency of medication was 959 times.The most frequently used traditional Chinese medicines were Huangqi,Dangshen,Banxia,Gancao,Fuling,Fushen,Jiegeng,Shengma,Yuanzhi and Wuweizi.There were 74 kinds of traditional Chinese medicines involved in Yin deficiency and fire hyperactivity syndrome,with a total frequency of 787 times.The most frequently used traditional Chinese medicines were Rehmannia glutinosa,Lily,White peony root,Oyster,Keel,Scutellaria baicalensis,Bupleuri,Pinellia ternata,Cinnamon Twig and Codonopsis pilosula.Heart blood stasis syndrome involves 70 kinds of traditional Chinese medicine,with a cumulative frequency of 411 times.The most frequently used traditional Chinese medicine is Salvia miltiorrhiza,Pinellia ternata,Ligusticum chuanxiong,Glycyrrhiza uralensis,Corydalis yanhusuo,Bupleurum chinense,Scutellaria baicalensis,Oyster,Keel,Poria cocos.The syndrome of phlegm-fire disturbing the heart involves 72 kinds of traditional Chinese medicine,with a cumulative frequency of 286 times.The most frequently used traditional Chinese medicine is Pinelliae Rhizoma,Glycyrrhizae Radix et Rhizoma,Scutellariae Radix,Oyster,Ramulus Cinnamomi,Keel,Paeoniae Radix Alba,Bupleurum,Rhubarb and Chrysanthemum.There are 57 kinds of traditional Chinese medicine involved in the syndrome of heart deficiency and timidity,and the cumulative frequency of medication is 272 times.The most frequently used traditional Chinese medicine is oyster,keel,magnet,dragon tooth,pinellia,poria cocos,white peony root,cassia twig,scutellaria baicalensis and bupleurum.There were 64 kinds of traditional Chinese medicines involved in the syndrome of water-qi Lingxin,and the cumulative frequency of medication was 213 times.The most frequently used traditional Chinese medicines were Astragalus membranaceus,Codonopsis pilosula,Glycyrrhiza uralensis,Tinglizi,Polygonum aviculare,Qumai,Dafupi,Danshen,Maidong and Wuweizi.The commonly used drugs are mainly warm drugs,followed by cold drugs and flat drugs.The top three sweet medicines,bitter medicines and pungent medicines with the highest frequency of use are mainly lung,spleen and heart meridians.The top three drug categories used frequently were :tonifying deficiency,tranquilizing mind and clearing heat.Through correlation analysis and cluster analysis,Professor Zhou Yabin’s 24 commonly used drug combinations for the treatment of coronary heart disease tachyarrhythmia(support 65,confidence 0.9)involved 9 drugs.There were 20 commonly used drug combinations for the treatment of heart and spleen deficiency syndrome(support 22,confidence 0.9),involving 6drugs.There were 143 commonly used drug combinations(support 16,confidence 0.9)in the treatment of yin deficiency and fire excess syndrome,involving 19 drugs.There were 22 commonly used drug combinations for the treatment of heart blood stasis syndrome(support 9,confidence 0.9),involving 11 drugs.There were 152 commonly used drug combinations(support 6,confidence 0.9)in the treatment of phlegm-fire disturbing heart syndrome,involving 13 drugs.There were 21 commonly used drug combinations for the treatment of patients with heart deficiency and timidity syndrome(support 6,confidence 0.9),involving 8 drugs.There were 43 commonly used drug combinations(support 5,confidence 0.9)in the treatment of patients with water-qi lingxin syndrome,involving 10 drugs.(k= 3)Conclusion:1.The common syndromes of Professor Zhou Yabin in the treatment of coronary heart disease tachyarrhythmia are heart and spleen deficiency syndrome,yin deficiency and fire hyperactivity syndrome,qi stagnation and blood stasis syndrome.2.Professor Zhou Yabin ’s commonly used drugs for the treatment of coronary heart disease tachyarrhythmia are Pinellia,Astragalus,Codonopsis,licorice,Poria,Scutellaria,keel,oysters,Bupleurum,Guizhi,white peony,rhubarb.3.The common medicinal properties of Professor Zhou Yabin in the treatment of tachyarrhythmia of coronary heart disease are mainly warm drugs,cold drugs and neutral drugs.The flavor of the medicine is mainly sweet,bitter and pungent.The meridian tropism is mainly attributed to the lung,spleen and heart meridians;commonly used drugs are tonifying deficiency,tranquilizing and clearing heat.4.Professor Zhou Yabin often uses the method of tonifying deficiency and reducing excess,strengthening the body and dispelling evil in the treatment of coronary heart disease tachyarrhythmia.
