Professor Ma Jian’s Analysis And Experience Summary Of The Medication Rules Of Traditional Chinese Medicine In The Treatment Of Simple Obesity

Objective:Based on Professor Ma Jian’s actual clinical cases,the TCM Heritage Computing Platform(V3.0)is used to summarise Professor Ma Jian’s rules and experiences in the use of drugs for the treatment of simple obesity in TCM,with a view to better applying them to clinical practice and making new contributions to the cause of TCM development in China.Method:The medical records of patients with simple obesity who met the inclusion criteria and were treated in the outpatient clinic of Professor Ma Jian of the Department of Endocrinology at the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine from January 2018 to December 2021 were collected,totaling 101 cases.The screened medical records were entered into the TCM Heritage Computing Platform(V3.0)one by one,and analysed including the general information of the patients,the frequency of the herbal medicines applied,the pattern of administration,the characteristics of the four qi and five tastes,the attribution properties and the clustering characteristics,to summarise the rules of herbal medicines applied by Professor Ma Jian in the treatment of simple obesity and his clinical treatment experience and learning methods.Results:(1)Among the 101 cases of simple obesity medical records,the number of male patients’ medical records was 40,accounting for 39.60%,and the number of female patients’ medical records was 61,accounting for 60.40%,with a ratio of 1:1.46.(2)Among the 101 cases of simple obesity medical records,62 symptoms were obtained,among which the symptoms with a frequency greater than 30 were weight gain,weakness,loose stools,snoring and heavy limbs.(3)Among the 101 cases of simple obesity,a total of 8 tongue signs were obtained,with red tongue(101 times),white coating(83 times)and toothed tongue(47 times)predominating,accounting for 84.31% of all tongue signs.(4)In 101 cases of simple obesity,12 types of pulse signs were obtained,of which the main ones were: sunken and slippery pulse(40 times),chordal and slippery pulse(25 times),and sunken and slow pulse(7 times),accounting for 71.28% of the total frequency of pulse signs.(5)Among the 101 prescriptions of Chinese medicine,a total of 80 medicines were used,and the top five medicines used frequently were Poria cocos,Lotus leaf,Semen cassia,Huang Lian and Chen Pi.(6)The four qi of Chinese herbal medicines were mainly cold 469 times(33.99%),warm 453 times(32.83%)and flat 357 times(25.87%),accounting for a percentage of 92.68%.(7)The five tastes of Chinese medicine were mainly bitter 801 times(40.93%),pungent 512 times(26.16%)and sweet 493 times(25.19%),accounting for 92.28% of the total.(8)The properties of the meridians attributed to Chinese herbal medicines totaling more than 300 times include the spleen,lung,stomach,liver,heart and large intestine meridians,accounting for 86.98% of the total.(9)The efficacy of herbal medicines was mainly in the category of clearing heat 402 times,tonifying deficiency 190 times and resolving dampness 153 times,accounting for 53.99% of the total.(10)Based on the association rule,the core drugs can be obtained as follows: Poria,lotus leaf,cassia seed,perilla,Huang Lian,scutellaria,dried ginger,Chen Pi,Semen,and Salvia.(11)Using the k-means algorithm,six core combinations were derived:Group 1: lotus leaf,poria,huang ling,cassia,scutellaria,tansy,dried ginger,pelargonium;Group 2: lotus leaf,semsia,poria,cassia,chenpi,atractylodes,licorice,huang lian;Group 3: huang lian,poria,lotus leaf,semsia,cassia,chenpi,dried ginger,scutellaria;Group 4: huang lian,cassia,pelargonium,lotus leaf,semsia,Poria,Salvia,Ephedra;Group V:Radix et Rhizoma,Lotus Leaf,Poria,Astragalus,Scutellaria,Radix et Rhizoma,Semen,Salvia;Group VI: Radix et Rhizoma,Radix et Rhizoma,Lotus Leaf,Poria,Scutellaria,Rhizoma,Pericarpium,Radix et Rhizoma.Conclusion:(1)The herbs used by Professor Ma Jian in the treatment of simple obesity are mostly used for clearing heat,tonifying deficiency and resolving dampness,with the core prescription drugs being: Poria,lotus leaf,cassia seed,perilla,Huang Lian,scutellaria,dried ginger,Chen Pi,Semen,and Salvia,with the most commonly used pair being lotus leaf-cassia seed.(2)The herbs applied by Professor Ma Jian in the treatment of simple obesity are mostly cold,warm and flat in the four qi,bitter,sweet and pungent in the five tastes,and bitter in the spleen meridian,lung meridian,stomach meridian and liver meridian.(3)Professor Ma Jian believes that the main causes of simple obesity are dietary disorder,internal injury to the emotions,and prolonged sitting with little labour.This disease is mostly a deficiency of the root and a deficiency of the symptoms.The deficiency of the root is mainly due to deficiency of the spleen and deficiency of qi;the deficiency of the symptoms is mainly due to phlegm and turbidity,and may be accompanied by pathological products such as blood stasis,qi stagnation and water-dampness.(4)In treating simple obesity,Professor Ma Jian divided it into three types of evidence: evidence of heat and dampness in the stomach,evidence of deficiency and dampness in the spleen,and evidence of phlegm and dampness within.(5)When treating simple obesity,Professor Ma Jian focuses on examining the etiology and pathogenesis of the disease and treating it with evidence.The basic principle of treatment is to strengthen the spleen,harmonize the stomach and resolve dampness and phlegm,while at the same time using methods such as clearing dampness and heat,regulating qi and invigorating blood.
