Research On Differentiative Medication Rules And Academic Thoughts Of Professor Deng Yunming’s Treatment Of Gout Based On The Traditional Chinese Medicine Inheritance Computing System V3.0

Objective: To summarize differentiative rules and academic thoughts of professor Deng Yunming’s treatment of gout by using the traditional Chinese medicine inheritance calculation system V3.0,and provide clinical basis for the intervention of traditional Chinese medicine in the disease.Methods: From January 1st,2019 to December 31 st,2020,Professor Deng Yunming,Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine,the clinical patients of gout who met the standard of case inclusions had been recorded in the forms of photos(basic information,diagnosis,clinical manifestation,external hospital auxiliary examinations,handwritten prescription sheets),prints(prescription medication and auxiliary examination of our hospital).The data had been input into the system such as the basic information,prescriptions,symptoms,tongue images and pulse images,and then conducted the statistical analysis and cluster analysis.Results: The 254 standard items of gout differentive by Pro.Deng Yunming had been analysis that the most common symptoms were first metatarsophalangeal joint redness and heat pain,high skin temperature,local acid swelling tenderness,limited movement,or loose stools,yellowish or reddish urine,night imsomnia;the most common tongue and pulse manefesitions were tongue red with yellow greasy fur,soft and rapid pulse;the most common pathogenesis types were accumulation of damp-heat pathogen,cold-dampness obstruction syndrome,deficiency of liver and kidney and phlegm-turbidity stagnation Syndrome;the most common pathogenesis: dampness and heat accumulation as branch,spleen and soil trapped for the root,the disease of the deficiency of the standard solid,the inclusion of virtual reality;the most common treatment: clearing heat and dampness,tonifying liver and kidney,warming Yang and water,invigorating spleen and dampness,expelling phlegm and lowering turbidity and so on;and the most commonly used drugs of which had accounted for 43.54% such as Cyathnla capitata Moq.,Atractylodes macrocephala,Appendiculate cremastra pseudobulb,Semen coicis,Bran-processed Rhizoma Atractylodis,Phellodendron chinensis,Pycnostelma paniculatum K.Schum,Rhizoma smilacis glabrae,Paris polyphylla,Amomum cardamomum,etc.By K-means algorithm and regression model cluster analysis,three core combinations were extracted: the first one was “Bran-processed Rhizoma Atractylodis,Rhizoma smilacis glabrae,Rhizoma dioscoreae hypoglaucae,Cyathnla capitata Moq.,Semen coicis”;the second was “Phellodendron chinensis,Semen coicis,Atractylodes macrocephala,Cyathnla capitata Moq.,Appendiculate cremastra pseudobulb” and the last was “Bran-processed Rhizoma Atractylodis,Atractylodes macrocephala,Cyathnla capitata Moq.,Appendiculate cremastra pseudobulb,Paris polyphylla”.Conclusions:1.By using data mining technology,the medical cases and priscriptions of famous old Chinese medicine could be objectively analyzed on many aspects as well as angles,and guide the clinical work.2.Combining the systematic statistics with the experience of professor Deng’s treatment in gout,the second combination-“Phellodendron chinensis,Semen coicis,Atractylodes macrocephala,Cyathnla capitata Moq.,Appendiculate cremastra pseudobulb”is the fundamental herbs in treating gout.3.Professor Deng’s treatment of gout pays more attention to adjusting middle-jiao,and the main principle of acute stage is“excess in superficiality”,which is clearing heat and dampness,as well as fixing middle-jiao,thus,Professor Deng is accustomed to heavily apply the medicine for its application such as Cyathnla capitata Moq.,Atractylodes macrocephala,Appendiculate cremastra pseudobulb,Semen coicis,Bran-processed Rhizoma Atractylodis,Phellodendron chinensis,Paris polyphylla and add or subtract herbs on this basis.However,during chronic and intermittent periods,professor Deng focuses on “deficiency in origin”,and the treatment is mainly about “Resolving Phlegm and reducing turbidity”,“tonifing liver and kidney”,“warm Yang and dissipate cold”,and adjust the spleen soil in the middle-jiao.
