Observation On Efficacy And The Influence Of Serum VCAM-1 Of Zhibai Dihuang Decoction And Siwu Decoction Combined With Laser Therapy On DR Of Yin Deficiency With Blood Stasis Type

Objective:This subject through evaluating the clinical efficacy of combination therapy of Zhibo Dihuang Decoction and Siwu Decoction combined with laser photocoagulation and laser therapy alone for pre and post-treatment of DR patients with Yin deficiency with stasis type,clinical syndrome scores and serum VCAM-1 levels,have observed the clinical efficacy of the former,and explored the related mechanism by which its efficacy exerts.Methods:A total of 113 eyes 60 DR patients of Yin deficiency with stasis were collected,including.They were randomly divided into the treatment group(30 cases,57 eyes)and the control group(30 patients,56 eyes),and the control group underwent laser photocoagulation treatment alone,once every 7 days for 4 this basis.The treatment group received concomitant oral administration of Zhibai Dihuang decoction combined with Siwu Decoction on this basis within the course of 28 days.BCVA,fundus photography,CMT,FFA were observed before and on the first day after the end of the treatment period.The overall efficacy and TCM clinical syndrome scores and serum VCAM-1 levels were compared before and after the treatment.Result:1.Before the treatment,there was no significant difference in sex,age,degree of lesion,BCVA,CMT,TCM clinical syndrome score and serum VCAM-1 level between the two groups(P<0.05).2.Clinical efficacy analysis:After the treatment,BCVA,Retinal hemorrhage,exudation area,microvascular tumor,CMT and FFA were all ameliorated compared with prior treatment,and the difference was statistically significant(P<0.05);the efficacy of the treatment group was better than that of the control group,The difference was statistically significant(P<0.05);Total effective rate of treatment group and control group was 89.5% and76.8%,respectively,the treatment group was better,and the difference was statistically significant(P<0.05).3.The scores of TCM clinical syndromes in both groups were decreased,and the treatment group was better than the control group.Both of two groups in self-control pre and post the treatment and comparison between the two groups after the treatment had statistical difference(P<0.05).The effective rate was 90.0% in the treatment group and 13.3% in the control group,with significant difference(P<0.05).4.The level of serum VCAM-1 decreased in both groups,while the treatment group was significantly lower.Both of two groups in self-control pre and post the treatment and comparison between the two groups after the treatment had statistical difference(P<0.05).Conclusion:1.Compared with laser photocoaculation treatment alone,Zhibai Dihuang Decoction and Siwu Decoction combined with laser therapy has a better curative effect on DR of yin deficiency with stasis type.It can better improve visual acuity and the condition of fundus microangioma,hemorrhage and exudation,promote the regression of macular edema,and reduce the formation of non-perfusion area and neovascularization.2.Compared with laser photocoaculation treatment alone,Zhibai Dihuang Decoction and Siwu Decoction combined with laser therapy can better improve the clinical symptoms of TCM and reduce the serum VCAM-1 level of DR patients of yin deficiency with stasis type,so as to improving the therapeutic effect of DR.
