Study On Clinical Experience Of Associate Professor Zhai Ying In The Treatment Of Stable Angina Pectoris In Coronary Heart Disease And Discussion On The Mechanism Of Empirical Formula

Objective: To explore the clinical experience of Associate Professor Zhai Ying in the treatment of stable angina pectoris in coronary artery disease,and to summarise the core formulae and discuss their mechanisms of action in the treatment of this disease.Methods:Collect information on cases related to the treatment of stable angina pectoris in coronary heart disease from the tutor,screen,collate and upload these information to the Ancient and Modern Medical Case Cloud Platform,derive statistical analysis information on the mentor’s cases and prescriptions for the treatment of this disease,then,using the hierarchical cluster analysis module within the platform to obtain party groups and select core empirical parties,screen the herbal components and targets of action of core formulae from TCMSP database,screen and correct target names in Uni Prot database,searche in OMIM,Drug Bank,Gene Cards databases to derive stable angina disease targets for coronary artery disease,screen the intersection of the two targets as potential targets of action,construct the core formula-component-potential target-disease network through Cytoscape 3.9.1 software and perform topological analysis to identify the key active components of the core formula,submit intersecting targets to STRING 11.5 to obtain targets’ interaction networks to derive key potential targets,submit potential targets to DAVID knowledgebase for GO gene function and KEGG pathway enrichment analysis to explore their mechanisms of action.Results: In this study,a total of 226 cases were collected,including 226 prescriptions,of which 93 were male and 133 were female,with an overall age distribution of 40-90 years;the cases are dominated by Qi deficiency with phlegm and stasis evidence,and Qi and Yin deficiency with stasis evidence,the elements of Chinese Medicine Identification Includes Qi deficiency,Yin deficiency,blood stasis,phlegm,Qi stagnation,etc.The prescriptions use a total of 149 kinds of Chinese herbs,which are mainly warm,flat,sweet,pungent and bitter,and mainly enter the heart and lung Meridians;based on hierarchical clustering analysis,the core empirical formulae were obtained: Gua Pou,Han Xia,Chen Pi,Chuan Xiong,Dan Shen,Dang Shen,Fu Ling and Gan Cao.All the above core formulae were selected and imported into the TCMSP database,and 192 active ingredients of the core formulae were screened,with 273 predicted gene targets.A total of 1125 targets related to stable angina pectoris in coronary artery disease were screened in various disease databases,then 115 predicted potential targets were obtained;the core formula herbal-component-potential target-disease network was constructed and 46 key active ingredients were derived;obtain high-confidence interaction information of potential target proteins and derive key potential targets: IL6,TNF,AKT1,IL1 B,VEGFA;the results of GO gene functional analysis: response to hypoxia,positive regulation of smooth muscle cell proliferation,positive regulation of cell proliferation,positive regulation of nitric oxide biosynthesis,inflammatory response,positive regulation of angiogenesis,positive regulation of cellular response to hypoxia,negative regulation of lipid storage.response to hypoxia,negative regulation of lipid storage,lipid metabolic processes,regulation of blood pressure,etc.;the results of KEGG pathway enrichment analysis: lipids and atherosclerosis,fluid shear stress and atherosclerosis,relaxin signaling pathway,etc.Conclusion: Supervisor Associate Professor Zhai Ying treats stable angina pectoris in coronary artery disease,using mainly warm and flat herbs,complemented by slightly cold or cold products.The taste of medicine is mostly sweet,pungent and bitter,and the treatment is to remove phlegm and eliminate blood stasis,benefit Qi and nourish blood to relieve pain.The herbs in the core formula mainly act by regulating lipid metabolism,reducing lipid storage,positively regulating angiogenesis,promoting smooth muscle cell proliferation,eliminating inflammatory responses,actively improving the internal state of the body’s lack of oxygen,regulating blood pressure,improving haemodynamics,and affecting the relaxin signalling pathway.
