| Objective: Based on the theory of five evolutive phases and six climatic changes,through literature analysis of the pathogenic factors of knee osteoarthritis and clinical retrospective study,to explore the correlation between the regional climate characteristics of knee osteoarthritis and related pathogenic factors.Methods:1.The database of CNKI,Wanfang,Weipu,etc.was searched by computer to collect relevant literature on the pathogenesis of knee osteoarthritis.The search time was from May 2002 to May 2022.After the researchers screened the literature,extracted data and evaluated the quality of methodology included in the study,Stata12.0 and Win BUGS1.4.3software were used for statistical score.Rev Man5.2 software was used to conduct a meta-analysis of the included literature.2.During the period from May 2012 to May 2022,1500 inpatients with knee osteoarthritis diagnosed in the Orthopedics and Traumatology Department of the Affiliated Hospital of Chang chun University of Traditional Chinese Medicine were collected and sorted out.The name,age,sex,menstruation,occupation,BMI,time of onset,inducement of onset and other items of the patients were recorded.The six gas data were calculated according to the theory of traditional Chinese medicine.The collected factors and number of cases were analyzed by multifactorial conditional logistic regression analysis based on six qi variables to analyze the relationship between five evolutive phases and six climatic changes and the incidence of knee osteoarthritis.Results:1.The literature analysis finally included 18 Chinese literatures,7510 patients in the case group and 13020 people in the control group;Meta analysis results showed that age(OR=2.79,95% CI=2.52~3.14),sex(OR=2.48,95% CI=2.23~2.95),BMI(OR=2.46,95% CI=2.17~2.83),sports injury(physical work)(OR=6.85,95% CI=3.76~9.79),living environment(moist+dark)(OR=2.60,95% CI=2.33~3.21),history of knee joint trauma(OR=3.99,95% CI=2.24 ~ 6.26),genetic factors(OR=2.37,95% CI=2.35 ~ 2.47),Osteoporosis(OR=2.11,95% CI=3.58~8.79)Diabetes(OR=3.24,95% CI=2.09~3.45),hypertension(OR=1.01,95% CI=2.67 ~ 6.22),sensitivity analysis and publication bias evaluation results show that the included literature does not have publication bias,and the results are relatively reliable.2.The clinical analysis finally included 1038 cases in the case group and 1038 cases in the control group.The analysis results showed that age: 40-64 years(OR=11.69,95%CI=2.5 ~ 11.97),≥ 65 years(OR=4.87,95% CI=1.6 ~ 61.65),gender: female(OR=5.26,95% CI=1.04 ~ 18.03),menopause(OR=9.04,95% CI=2.59 ~ 8.46),BMI:24-27.9(OR=12.35,95% CI=1.28~7.62),≥ 28(OR=2.62,95% CI=3.08~18.66),Manual workers(OR=2.86,95%CI=1.5~23.59),family history(OR=1.24,95% CI=43.49~ 65.24),knee joint trauma history(OR=1.45,95% CI=5.78 ~58.73),osteoporosis(OR=1.69,95%CI=1.59~17.93),glycolipid metabolic diseases(OR=3.86,95% CI=2.68~27.58),living environment(OR=5.26,95% CI=7.64~43.11);The analysis results of the six qi show that the initial qi(OR=3.69,95% CI=3.64 ~ 15.37),the two qi(OR=0.17,95% CI=1.58 ~4.25),the three qi(OR=0.38,95% CI=1.31~9.03),the four qi(OR=5.35,95% CI=2.77~10.42),the five qi(OR=0.71,95% CI=5.35~9.53),the six qi(OR=9.52,95% CI=4.18~22.01),and the initial qi,the four qi,and the six qi have obvious effects on the onset of knee osteoarthritis.Conclusion: The clinical research results are basically consistent with the literature analysis results.Age,gender,BMI,sports injury(physical work),living environment(damp+dark),history of knee joint trauma,family history,osteoporosis,diabetes,hypertension and other factors are the risk factors for the onset of knee osteoarthritis.The clinical research shows that the initial qi of six qi,the qi of four qi The three periods of six qi have obvious influence on the onset of knee osteoarthritis. |
