Objective To compare the efficacy of butorphanol or sufentanil combined with ropivacaine in epidural labor analgesia and the effects on maternal and infant outcomes.Methods 90 singleton and full term primiparas who would like to accept labor analgesia were randomly divided into three groups.There were 30 participants in each group.20 ug/ml butorphanol and 0.115% ropivacaine were given in group B.0.4 ug/ml sufentanil and 0.115% ropivacaine were given in group S.0.115% ropivacaine were given in group L.After epidural puncture and catheterization,the first dose of 6ml solution was given respectively,and patient-controlled analgesic pumps were set and run in three groups.The analgesic effect after medication was observed and recorded in the three groups: Analgesia onset time(VAS ≤ 3 points),first PCA time,dosage,number of effective compressions,analgesia duration,VAS score 30 min after analgesia and Ramsay sedation score after analgesia onset.Vital signs,delivery mode,neonatal Apgar score and adverse reactions of the three groups of women were observed and recorded.Results Analgesic effect: There were statistically significant differences in analgesic onset time,first PCA time and effective compression times among the three groups(P<0.05).While there were no statistically significant differences in drug dosage and analgesic time among the three groups(P>0.05).Compared with group L,the analgesic onset time in groups B and S was significantly shortened,and the first PCA time was significantly prolonged(P< 0.05).Compared with group L,the number of effective compressions in group B was significantly reduced(P<0.05).There was no significant difference in analgesia between group B and group S(P>0.05),and there was statistically significant difference in Ramsay sedation score among the three groups(P<0.05).Adverse reactions: There were 4cases of adverse reactions in group B with an incidence of 13.33%,11 cases of adverse reactions in group S with an incidence of 36.67%,and 16 cases of adverse reactions in group L with an incidence of 53.33%.The difference was statistically significant(P<0.05).Compared with group S,adverse reactions in group B were significantly reduced,and the difference was statistically significant(P<0.05).There was no significant difference in delivery mode and Apgar score among the three groups(P>0.05).Conclusion Compared with ropivacaine group,the analgesic effect of combined opioids is better,and the incidence of adverse reactions was lower.Butorphanol 20 ug/ml combined with 0.115% ropivacaine had the same analgesic effect as Sufentanil 0.4 ug/ml combined with 0.115% ropivacaine.But the incidence of adverse reactions was significantly reduced in butorphanol group.
