| Objective Infertile men and normal men were tested for anti-sperm antibody(AsAb),sperm DNA fragmentation index(DFI),sperm acrosomal enzyme activity,and semen routine parameters.To explore the effects of AsAb on sperm DFI,sperm acrosomal enzyme activity,semen routine parameters and male age,and further analyze the correlation between the above related indicators,so as to provide better fertility guidance for male infertility patients with fertility needs.Methods A total of 156 infertile male patients treated in the reproductive medicine outpatient department of Qingdao Women and Children’s Hospital from January 2020 to June 2021 were selected as the experimental group,and 115 normal males in the physical examination center at the same time were selected as the control group.The basic demographic data and related detection indicators of the study subjects were collected.Enzyme linked immunosorbent assay(ELISA)was used to detect the plasma AsAb of the study subjects.According to the detection results,the experimental group was divided into AsAb-positive group(31 cases)and AsAb-negative group(125 cases).The subjects in the control group were divided into AsAb-positive group(3 cases)and AsAb-negative group(112 cases).The number of AsAb-positive cases in the control group was too small to be statistically significant.Therefore,part of the data analysis of the AsAb-positive group in the control group was subsequently cancelled.According to the age,the samples in AsAb-positive group and AsAb-negative group were divided into three subgroups: <30 years old,30-39 years old,and ≥40 years old groups.Sperm DFI,acrosin activity and routine parameters(sperm concentration,sperm motility,and normal morphological sperm proportion)were assessed by sperm chromatin structure assay(SCSA)and substrate enzyme method and computer-assisted sperm analysis(CASA).SPSS 22.0 software was used for statistical analysis,χ2 test was used to compare AsAb,sperm DFI,sperm acrosomal enzyme activity and semen routine parameters between the experimental group and the control group,and two-tailed t test was used to analyze the semen indexes of AsAb-negative and positive patients in the experimental group.One-way ANOVA was used to compare semen indexes among three age subgroups in AsAb-negative group and positive group respectively.Two-way ANOVA was used to analyze the semen indexes of three age subgroups of AsAbnegative group in experimental group and control group,and two-way ANOVA was also used to compare semen indexes between AsAb-positive and AsAb-negative age subgroups in the experimental group.Pearson correlation was used to analyze the correlation of semen indexes in AsAb-negative group and positive group in the experimental group respectively,as well as the correlation of semen indexes in the AsAb-negative group in the control group,P<0.05 was considered statistically significant.Results(1)The percentages of abnormal semen parameters in the experimental group were higher than those in the control group,and the differences among AsAb,sperm motility and proportion of normal sperm were significant(P<0.05).(2)The sperm concentration,sperm motility and proportion of normal sperm in AsAb-positive group were lower than those in AsAb-negative group in the experimental group(P<0.05).There were no significant differences in sperm DFI,sperm acrosin activity and age of infertile males between AsAb-positive and AsAb-negative groups(P>0.05).(3)Among the three age subgroups of AsAb-positive group in the experimental group,the proportion of normal sperm in <30 years old group was significantly higher than that in 30-39 years old group and ≥40 years old group(P<0.05).The difference of sperm DFI between the 30-39 years old group and the ≥40years old group was statistically significant(P<0.05),while the difference between the <30years old group and the 30-39 years old group was not significant(P>0.05).(4)Among the three age subgroups of AsAb-negative group in the experimental group and the control group,sperm motility in <30 years old and 30-39 years old group was significantly higher than that in ≥40 years old group(P<0.05).The proportion of normal sperm in <30 years old group was significantly higher than that in 30-39 years old group and ≥40 years old group,and that in 30-39 years old group was higher than that in ≥40 years old group,the differences were statistically significant(P<0.05).The sperm DFI of <30 years old group and 30-39 years old group was significantly lower than that of ≥40 years old group(P<0.05).(5)The effects of AsAb groups on sperm concentration,sperm motility,proportion of normal sperm and sperm DFI were significant(P<0.05),but the effects on acrosomal enzyme activity were not significant(P>0.05).Age groups had significant effects on sperm concentration,sperm motility and proportion of normal sperm(P<0.05),but had no significant effects on sperm DFI and acrosomal enzyme activity(P>0.05).The interaction between AsAb group and all age groups had no significant effect on semen index parameters(P>0.05).(6)In the experimental AsAb-positive group,age was positively correlated with sperm DFI(P<0.05).There was no correlation between age,sperm DFI and acrosomal enzyme activity(P>0.05).(7)In the experimental AsAb-negative group,age was positively correlated with sperm DFI,and negatively correlated with sperm motility and proportion of normal sperm(P<0.05).There was a negative correlation between sperm DFI and sperm motility and the proportion of normal morphologic sperm(P<0.05).There was no correlation between age and sperm concentration and acrosomal enzyme activity(P>0.05).There was no correlation between sperm DFI and sperm concentration or acrosomal enzyme activity(P>0.05).There was no correlation between acrosomal enzyme activity with sperm DFI,sperm concentration,sperm motility and proportion of normal sperm(P>0.05).(8)In the AsAb-negative group of the control group,age was positively correlated with sperm DFI(P<0.05),and negatively correlated with sperm motility,proportion of normal sperm and acrosomal enzyme activity(P<0.05).Sperm DFI was negatively correlated with sperm motility and the proportion of normal sperm(P<0.05).There was no correlation between age and sperm concentration(P>0.05).There was no correlation between sperm DFI and sperm concentration and acrosomal enzyme activity(P>0.05).There was no correlation between acrosomal enzyme activity and sperm DFI,sperm concentration,sperm motility and proportion of normal sperm(P>0.05).Conclusion(1)AsAb reduce sperm motility,sperm concentration and the proportion of normal sperm,early treatment for patient might increase the success rate of conception.(2)After the age of 40,the sperm motility and the proportion of normal sperm decreased significantly,and the sperm DFI increased significantly,which resulted in the decrease of fertility.(3)It is recommended to perform sperm DFI examination as early as possible for infertile men more than 40 years of age. |
