Clinical Experience And Medication Rule Of Professor Shi Zhenggang In The Treatment Of Intellectual Retardation

Ojective:Based on data mining technology,this paper analyzes the characteristics of syndrome,treatment and medication of Professor Shi Zhenggang’s treatment of Mental retardation(MR)in children.To summarize the medication rule and clinical treatment experience of Professor Shi in the treatment of MR in children.so as to provide more accurate ideas and reference for the clinical treatment of MR.Methods:Through Professor Shi Zhenggang,the outpatient counselor,collected and sorted the cases that met the diagnostic criteria of intelligent developmental delay received by the supervisor in the outpatient department of the Affiliated Hospital of Gansu University of Chinese Medicine from January 2019 to October 2022,and referred to the relevant requirements of the TCM inheritance assistance platform V3.0 system.Create prescription database with Microsoft Office Excel 2019,proofread and enter data.General information such as age and gender,syndrome type,high-frequency drugs involved in prescription and their sex and taste normalization were systematically analyzed.IBM SPSS modeler 18.0 and IBM SPSS Statistics 27.0 were used for association rules and cluster analysis,respectively.Combined with the clinical application,this paper analyzed and summarized the medication rules of Professor Shi Zhenggang in the treatment of MR,and summarized the clinical experience of Professor Shi Zhenggang in the treatment of MR.Results:1.Descriptive analysis: This study included 47 cases,Total prescription 149,which involved the taste of Chinese medicine 138.The male to female ratio of cases was 2.1:1,with a majority of children aged 4-6.There are 9 types of syndrome involved: Deficiency of spleen and kidney(55.70%),Weakness of spleen and stomach(20.13%),The spleen deficiency and the accumulation of food(11.41%),Spleen deficiency and liver stasis(4.7%),Depression in wind and phlegm(2.01%),Deficiency of heart and spleen(2.01%),Weakness of liver and kidney(1.34%),Phphonic stasis blocked(1.34%),Phlegm and food stagnation(1.34%).The viscera are mainly involved in the spleen,kidney,heart,liver and other viscera,and the pathological factors are mainly phlegm,blood stasis and food accumulation.The top 10 Medication frequency of the Medication is from high to low sort in sequence: Hawthorn,Chuan Niu Knee,Grain Bud,Wheat Bud,Yu Jin,Acorus Calamus,Yuan Zhi,Cistanches,Betel Nut,Boneset.The Medication is classified as a tonic for deficiency,an eliminator of food,an activator of blood circulation and stasis,a purgative,and a purgative,damp-clearing drug,qi-regulating drugs,tranquilizer,liver and wind soothing medicine.The main meridians of traditional Chinese medicine were spleen,stomach,liver and kidney.The four qi of Chinese medicine involved were mainly warm medication,accounting for about half of the medication used,followed by cold and flat medication,but not for hot medication.The five tastes of the medication used were mainly sweet,bitter and Xin,with less sour and salty.2.Analysis of formula formation rules based on association rules: A total of 57 drug association rules were obtained,including 23 second-order association rules and 34 second-order association rules.The commonly used drugs were mainly the combination of hawthorn,grain sprout,malt,calamus,polygalea,rhizome,rhizoma buccana,Cistanche,psoralen,areca nut,Radix astragalus,poria cocos and fructus aurantii.3.Cluster analysis was performed on the top 21 tastes of high-frequency drugs with frequency ≥ 45,which could be divided into 6 clusters:The first kind: Hawthorn;The second category: Radix achyranthes,Yu Jin,calamus,Radix polygala,Cistanche,deerhorn cream,Xu Duan,Caulis Spatholobi;The third category: grain bud,malt,Shen Qu;The fourth category: Areca nut,Poria cocos,Fructus aurantii,Radix Astragali,selfheal,Atractylodis,Radix pseudostellariae;The fifth type: psoralen;The sixth type: Schisandra.Based on the analysis and screening of the prescriptions obtained from the above cluster analysis,combined with Professor Shi Zhenggang’s clinical experience and clinical drug characteristics,the following core prescriptions can be obtained:(1)Yu Jin,Acorus calamus,Yuan Zhi,Radix achyranthes,Bonesetter,Cistanches,Cistanches,Sequins,Radix achyranthes,Roasted Astragalus,Poria,Atractylodes,Hawthorn,Radix achyranthes,Grain Bud,Malt;(2)Yu Jin,Acorus calamus,Yuan Zhi,Radix achyranthes,Roasted Astragalus,Poria,Atractylodes,Citrus Aurantium,Betel nut,Zedoary,Xia Ku Cao,Gardenia,Xian He Cao,Hawthorn,Grain Bud,Malt.Conclusions:1.Professor Shi Zhenggang believed that the mental retardation of children was mainly attributable to the spleen,stomach,liver and kidney,and The main pathogenic factors are "sputum,blood stasis and Food lag".The pathological basis is deficiency of spleen and kidney,brain and pulp dystrophy,and obstruction of sputum and blood stasis is the main pathological factor.The mainly clinical syndrome types are spleen and kidney deficiency,spleen and stomach weakness,accumulation of spleen deficiency,liver depression and deficiency of spleen,depression in wind phlegm,heart-spleen weakness,liver and kidney weakness,phlegm and blood stasis,stagnation of phlegm and food.2.Professor Shi Zhenggang always insists on prescribing drugs based on physiological and pathological characteristics of children in the treatment of mental retardation,treating from the spleen and kidney,tonifying the kidney and transporting the spleen,giving consideration to the viscera and distinguishing the primary and secondary.Spleen,kidney,liver three viscera,cold and warm.The main therapeutic principles are warming the kidney and filling the lean pulp to cure its root,eliminating phlegm and removing blood stasis and opening the body to cure its symptoms,and nourishing the acquired spleen and stomach to strengthen its root.Based on the spleen and kidney deficiency,taking into account phlegm,blood stasis,food accumulation,both symptoms and root causes of treatment,elimination and supplement.Fuzheng and exorcism are used together to supplement deficiency without obstructing evil,dispelling evil without hurting positive.The core drug groups of Professor Shi Zhenggang’s treatment of MR Include calamus calamus,turpentine,Radix polygonae,Radix butanae,cistanche,psoralea,processed astragalus,poria,paraphylla,hawthorn,grain bud and malt,especially the use of kidney toning and spleen transporting drugs in clinical syndrome.3..Through the V3.0 system of TCM inheritance assistance platform,we analyzed and summarized Professor Shi Zhenggang’s prescription medication rules and clinical diagnosis and treatment experience for intelligent retardation,which can truly and accurately reflect the characteristics of Professor Shi Zhenggang’s clinical prescription medication,and provide relatively accurate data support and theoretical basis for the inheritance of tutor’s experience and the treatment ideas of this disease.
