OBJECTIVE: Through the collection of clinicopathological results of postmenopausal women with uterine space occupying lesions, the correlation between clinical characteristics, imaging characteristics and postoperative pathological diagnosis results of postmenopausal women was analyzed,so as to guide more standardized clinical treatment. Moreover, malignant lesions can be found more timely and accurately to prevent missed diagnosis and misdiagnosis,so as to provide effective personalized medical services for patients. Thus,it can provide certain evidence and reference for clinical prevention and diagnosis, and achieve early detection of lesions, timely clinical diagnosis and effective treatment for high-risk groups as soon as possible.METHOD: A total of 278 postmenopausal women admitted to the First Affiliated Hospital of Hebei North University(provincial Grade A comprehensive hospital)from December 2020 to December 2022 underwent transvaginalultrasound(TVUS)to suggest uterine space occupation and further hysteroscopy. There were 13 cases of malignant lesions and 265 cases of benign lesions.Statistical analysis was performed using SPSS26.0 software package.t test was used for normal distribution data, rank sum test was used for non-normal distribution data, and chi-square test was used for counting data.Logistic regression analysis was used to analyze the independent influencing factors of the malignant group.P < 0.05 at the test level indicated that the difference was statistically significant.RESULT: The mean age of the 278 patients was(56.02±4.89)years,and the mean menopause time was(6.57±5.01)years.Pathological results showed that 265(95.32%)of 278 postmenopausal patients had benign lesions and 13(4.68%)had malignant lesions. The distribution of postoperative pathological results was mainly benign lesions, and there were statistically significant differences in age, menopause age and BMI(Body Mass Index)between benign and malignant groups(P <0.05).The family history of hypertension, diabetes and malignant tumor were statistically significant between benign group and malignant group(P < 0.05).There were no significant differences in menopause years, pregnancy times, birth times and personal history of malignant tumors between benign group and malignant group(P > 0.05).There were statistically significant differences in the history of vaginal bleeding and drainage between the benign group and the malignant group(P < 0.05).There were significant differences in endometrial thickness and uterine space occupying size between benign and malignant groups(P < 0.05).There was no significant difference in uterine mass echo between benign group and malignant group(P > 0.05).Age,BMI,hypertension,family history of malignant tumor, history of vaginal bleeding, and uterine space occupying size were independent risk factors for malignant group.CONCLUSION:1.endometrialpolyps(EP),submucosal myoma and endometrial hyperplasia are mainly benign lesions in postmenopausal women.2.Age,menopause age,BMI,hypertension, abnormal postmenopausal bleeding and drainage, family history of malignant tumor, hormone replacement therapy history, endometrial thickness and uterine space occupying size are high-risk factors for the development of malignant uterine space occupying lesions in postmenopausal women.3.Age,BMI,hypertension,family history of malignant tumor, history of vaginal bleeding and size of uterine space were independent risk factors of malignant group.
