A Study On The Performance Evaluation Of The National Organization’s Policy Of Centralized And Volume Procurement Of Drugs

The implementation of the policy of organizing centralized drug procurement at the national level is an important part of the new medical reform in the past four years,and it is a Policy Innovations and optimization of the past centralized drug procurement at the provincial,municipal,county and regional levels.From the implementation of the new policy at the end of2018 to November 2022,the country organized a total of seven batches of centralized and volume procurement of drugs.The construction of evaluation indicators and performance evaluation for the implementation of this new policy series is one of the important contents of public management.This article takes the policy of centralized procurement of drugs by national organizations as the research object.Through the "structure process results" model,combined with relevant theories such as the "4E" theory and economies of scale theory,the performance evaluation indicators of this policy are extracted from policy requirements,work objectives,and literature review.The research method uses expert scoring and Analytic Hierarchy Process to construct the performance evaluation indicator system of centralized procurement of drugs by national organizations.The target layer is the performance evaluation of the national organization’s drug centralized procurement policy,while the criterion layer includes six items:policy input,policy participation,execution management,economic benefits,social benefits,and satisfaction.The evaluation index layer includes nineteen items:procurement frequency,included drug varieties for centralized procurement,enterprise participation,and drug price reduction.Construct a consistency judgment matrix using Analytic Hierarchy Process(AHP),obtain matrix values through expert scoring,and then use the sum product method to calculate the weights of each indicator one by one.Then,perform consistency testing to obtain the comprehensive weights of various indicators at all levels.Using the above evaluation index system to evaluate the performance level of the national organization’s drug centralized procurement policy,a total performance score of 3.87 points was obtained,indicating a good evaluation.Among them,evaluation of excellent indicators such as organizational security,participation of pharmaceutical enterprises,conservation of medical insurance funds,and drug price decline rate;The evaluation of moderate or below includes indicators such as the number of centrally purchased drug varieties,supply guarantee,regulatory effectiveness,reducing public medical expenses,and enterprise sales costs.Through the research and analysis of the performance evaluation of this policy,the following issues have been found:(1)the public’s awareness and participation are not high;(2)Insufficient supply guarantee capacity for centralized procurement of drugs;(3)The participation of multiple subjects is relatively low;(4)Information technology construction needs to be improved;(5)Lack of accuracy in reporting volume by medical institutions;(6)The effect of controlling patient medical expenses is not significant;(7)The regulatory mechanism is not sound.Propose corresponding countermeasures:(1)Increase investment in policy promotion;(2)Strengthening supply guarantee capabilities;(3)Enhancing the participation enthusiasm of diverse subjects;(4)Promote the informatization construction of the pharmaceutical equipment procurement platform;(5)Optimize the "quantity" of procurement with quantity;(6)Expand the coverage of national drug collection products;(7)Establish and improve supervision and management mechanisms.
