| ObjectiveTo explore the clinical value of Zhongyi ointment combined with sodium bicarbonate cold wet compress in the treatment of gout with dampness and heat accumulation,and to provide a new intervention method for rehabilitation of gout with dampness and heat accumulation.MethodsThis study was a randomized controlled clinical trial.84 patients with damping-heat aggregate gout who met the inclusion criteria were enrolled through open recruitment from January 2020 to February 2021.On the basis of voluntary participation,the patients were randomly divided into control group(n=42)and observation group(n=42)by number table method.On the basis of conventional treatment,the control group was treated with external application of Zhongyi ointment,while the observation group was treated with cold and wet application of Zhongyi ointment combined with sodium bicarbonate.After 5 days of treatment,index analysis and efficacy evaluation were performed in both groups.VAS scale was used to evaluate the pain degree of patients,SF-36 scale was used to evaluate the quality of life of patients,and T test was used to analyze the data.The TCM syndrome symptom score table was used to evaluate the clinical syndrome symptoms,and the rank-sum test was used to analyze the data.CRP and ESR indicators were detected,and rank-sum test was used for data analysis.The relief time of pain,redness and UA were detected,and the data were analyzed by T test.Clinical efficacy and subjective satisfaction were analyzed by Wilcoxon Mann-Whitney U rank-sum test.SPSS26.0 was used for statistical analysis of the data.The test level α=0.05 was specified,and a two-sided test was used,with P<0.05 indicating statistically significant differences.ResultsIn this study,79 patients with damp-heat aggregate gout completed the test,including 40 cases in the observation group(1 case of shedding,1 case of elimination)and 39 cases in the control group(2 cases of shedding,1 case of elimination).During the study,no allergic phenomena of external application drugs were found in the two groups of patients,and there was 1 case of dizziness,1 case of gastrointestinal discomfort and other reactions in the observation group.There was no significant difference in general data and clinical data between 2groups(P>0.05).1.TCM syndrome symptom score and clinical efficacyIntra-group comparison: After intervention,the scores of traditional Chinese medicine syndrome symptoms such as joint redness,joint swelling,joint fever,joint pain,upset,thirst,short red urine decreased significantly in the two groups compared with before intervention,with statistical significance(P<0.01);Inter-group comparison: After intervention,the scores of TCM syndromes and symptoms in the observation group were significantly lower than those in the control group,with statistical significance(P<0.05);Clinical efficacy: after intervention,the effective rate of symptom improvement in the observation group was 95.00%,significantly better than 79.49% in the control group,and the difference between the two groups was statistically significant(Z=-2.110,P=0.035).2.Pain levelIntra-group comparison: After intervention,the pain score of patients in the two groups decreased significantly compared with that before intervention,with statistical significance(P<0.01);Inter-group comparison: after intervention,the pain score of the observation group(1.27±0.63)was significantly lower than that of the control group(1.70 ± 0.96),the difference was statistically significant(P<0.05).3.Relief time of pain and rednessAfter intervention,the pain relief time(23.80 ± 10.65)h and the swelling relief time(54.05 ± 18.08)h in the observation group were shorter than those in the control group(29.51±12.59)h and the swelling relief time(70.56±29.28)h,and the difference was statistically significant(P<0.05).4.Quality of life scoreIntra-group comparison: After intervention,the quality of life score of patients in the two groups increased significantly,with statistical significance(P<0.01);Inter-group comparison:After intervention,the body pain score of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05),but there was no significant difference between the observation group and the control group in terms of overall health,mental health,emotional function,social function,physiological function,physiological function and vitality(P>0.05).5.Changes of CRP,ESR and UAIntra-group comparison: CRP and ESR in 2 groups decreased significantly after intervention,with statistical significance(P<0.01).UA decreased in both groups after intervention,but the decrease was not significant(P>0.05).Inter-group comparison: After intervention,CRP and ESR in the observation group were significantly lower than those in the control group,with statistical significance(P<0.05).There was no significant difference in UA between the observation group and the control group before and after intervention(P>0.05).6.Subjective satisfactionAfter intervention,the subjective satisfaction of patients was analyzed,which was divided into satisfied,general and dissatisfied.The results show: After intervention,31 cases in the observation group were satisfied(77.50%),6 cases in the control group were satisfied(61.54%),7 cases in the control group were satisfied(17.95%),8 cases in the control group were dissatisfied(20.51%).After statistical analysis,There was no significant difference between the two groups(Z=-1.671,P=0.095).ConclusionZhongyi ointmen combined with sodium bicarbonate cold and wet compress has a good effect on gout with damp-heat accumulation,which can effectively improve the pain,inflammatory response and clinical symptoms of patients with gout,improve their daily activity ability and improve their quality of life.It is a simple and effective gout treatment method with certain safety and worthy of clinical promotion. |
