Clinical Observation On Dachengqi Decoction Plus Retention Enema In The Treatment Of Acute Lung Injury (Yangming Fu Shi Syndrome) After Thoracotom

Objective:Acute lung injury(ALI)is a serious complication after thoracotomy.Its pathogenesis remains unclear and its clinical treatment is complicated.The purpose of this study was to observe the efficacy of Dachengqi Decoction supplemented with retention enema in the treatment of acute lung injury after thoracotomy(Yangming Fu-organ demonstration),and to provide a new TCM treatment idea for clinical treatment.Methods:A total of 60 patients with acute lung injury after thoracotomy were randomly divided into control group and treatment group,with 30 cases in each group.Control group:equal amount of normal saline retention enema+postoperative conventional Western medicine treatment,treatment group:Dachengqi Decoction supplemented retention enema+postoperative conventional Western medicine treatment,the course of treatment was 7 days.Inflammatory indexes(CRP,PCT,IL-6),serum albumin(ALB)and oxygenation index(OI)of the two groups were observed before treatment and after 3 and 7 days of treatment.Murray lung injury score,TCM syndrome score,ALT,AST,serum Cr and Urea were observed before and after treatment,and the total effective rate was compared after treatment.SPSS26.0 was used to analyze the above statistical data,and P<0.05 was considered as significant difference and statistically significant.Results:①Before treatment,there were no significant differences in gender,age,operation duration,inflammatory indicators(CRP,PCT,IL-6),albumin(ALB),PaO2/FiO2(OI),AST,ALT,Cr,Urea, Murray lung injury score and TCM syndrome score between the two groups(P>0.05),indicating that the two groups were comparable.②After 3 days of treatment,there were no significant differences in inflammation levels between the two groups(P>0.05).After 7 days of treatment,the inflammation level in the treatment group was lower than that in the control group(P>0.05).③After 3 days of treatment,there was no significant difference in ALB level between the two groups.After 7 days of treatment,the level of ALB in treatment group was higher than that in control group(P>0.05).④ After 3 days and 7 days of treatment,OI levels in the treatment group were higher than those in the control group(P<0.05).⑤After 7 days of treatment,Murray lung injury score in the treatment group was lower than that in the control group(P<0.05).⑥After 7 days of treatment,the TCM syndrome score of the treatment group was lower than that of the control group(P<0.05),and the total effective rate of the treatment group(96.67%)was higher than that of the control group(83.33%),with significant difference(P<0.05).⑦There was no significant difference in liver function and renal function between the two groups before and after treatment(P>0.05).Conclusion:①Dachengqi decoction with flavor retention enema can reduce the relevant inflammatory indicators of ALI(Yangming Fu-organ demonstration)after thoracotomy,reduce the inflammatory response of the body,improve oxygenation,and reduce the degree of lung injury.②Dachengqi decoction flavored retention enema can reduce the TCM syndrome score of patients with postoperative ALI(Yangming Fu-organ demonstration),relieve clinical symptoms,and promote postoperative recovery.
