Effect Of Jiawei Guizhi Fuling Pill On The Improvement Of Neural Microcirculation And The Expression Of CD34 And VWF In PDPN Rat

Purpose:In this study,the mechanism of Jiawei Guizhi Fuling Pill in the treatment of painful diabetic peripheral neuropathy(PDPN)was discussed from the perspective of improving nerve microcirculation through in vivo experiments,which provided a deep theoretical basis for the clinical treatment of PDPN.Material and method : Using random number table method,12 rats were randomly selected from 104 SPF healthy male SD rats as normal group(CON group),and the rest rats were made diabetes model.Rats that need to be modeled are fed adaptively for one week.After fasting for 12 hours,50mg/kg of 2% streptozotocin(STZ)solution is injected intraperitoneally.After 72 hours,rats whose blood sugar concentration is higher than 16.7mmol/L will continue to be included in the follow-up study.Rats fed with ordinary feed for 4 weeks,and the mechanical pain threshold or thermal pain threshold decreased by 50% after 4 weeks,were considered as successful PDPN modeling.The rats successfully established were randomly divided into model group(MOD group),low-dose group of Jiawei Guizhi Fuling Pill(GZFL-L group),middle-dose group of Jiawei Guizhi Fuling Pill(GZFL-M group),high-dose group of Jiawei Guizhi Fuling Pill(GZFL-H group)and pancreatic kininogenase enteric-coated tablet group(YJTY group),and the treatment lasted for 8 weeks.During the experiment,the behavior of rats was observed,blood sugar and weight were dynamically monitored,and the effect of Jiawei Guizhi Fuling Pill on the pain threshold of rats was detected by hot plate test and Von Frey mechanical pain threshold test.Motor nerve conduction velocity(MNCV)and sciatic nerve blood perfusion(BNF)were measured.The expression of VEGF in serum was determined by Elisa.The expression of hematopoietic progenitor cell antigen(CD34)and von Willebrand factor(VWF)in sciatic nerve were determined by real-time PCR(QCPR)and Western blot.Using SPSS software for statistical analysis,P<0.05 was statistically significant.Results:1.The rats in 1.MOD group have poor mental state,heavy smell,slow movement,dry and dull hair,and the symptoms of "three more and one less" appear.After 8 weeks of intragastric administration,all the rats in other treatment groups had the above manifestations,but the symptoms were improved compared with those in MOD group.2.Before treatment,the blood sugar level in MOD group was significantly higher than that in CON group(P < 0.01).After 8 weeks of treatment,the blood sugar level of rats in each dose group of Jiawei Guizhi Fuling Pill was lower than that in MOD group(P < 0.05),but there was no significant difference between YJTY group and MOD group(P > 0.05).3.Compared with the rats in CON group,the weight of the rats in 3.MOD group and other treatment groups decreased significantly(P < 0.01).After 8 weeks of treatment,the weight level of rats in each treatment group increased compared with that in MOD group(P < 0.01).4.Before treatment,compared with the rats in CON group,the pain threshold in MOD group and each treatment group was significantly decreased(P < 0.01).After 8weeks of treatment,the pain threshold of rats in other treatment groups was improved(P < 0.01).5.Before treatment,compared with the rats in CON group,the motor nerve conduction velocity in MOD group and each treatment group decreased significantly(P < 0.01).After 8 weeks of treatment,the motor nerve conduction velocity of rats in each treatment group and MOD group showed a downward trend,but the downward trend of the velocity of rats in treatment group was slower than that in MOD group(P< 0.01).6.Compared with CON group,the blood perfusion of sciatic nerve in MOD group decreased significantly(P < 0.01),and after 8 weeks of treatment,the blood perfusion of rats in each treatment group increased compared with MOD group(P < 0.01).7.Compared with CON group,the expression of VEGF in serum of MOD group decreased significantly(P < 0.01).Compared with MOD group,the content of VEGF in each treatment group increased(P < 0.01).8.Compared with CON group,the expression of CD34 gene and protein in sciatic nerve of other groups decreased significantly(P < 0.01),while the expression of v WF gene and protein increased significantly(P < 0.01).Compared with MOD group,the expression of CD34 in each dose group and YJTY group of Jiawei Guizhi Fuling Pill increased,while the expression of v WF decreased(P < 0.01),especially in GZFL-H group.Conclusion:Jiawei Guizhi Fuling Pill may improve nerve microcirculation by up-regulating the expression of VEGF in serum and CD34 in sciatic nerve of PDPN rats,and down-regulating the expression of v WF,thus repairing damaged nerve.
