Objective: To compare the clinical effect of arthroscopic ACL reconstruction with or without remnant preservation.Methods: Between January 2011 to December 2012,146 patients with ACL injury were selected as research object, all the patients were randomly divided into two groups; preserving-remnant group and removing-remnant group. Autologous hamstring tendons were selected. Evaluation of knee mobility, Lysholm Scale, IKDC Scores and knee stability was recorded before operation and 6 months,12 months after operation. Proprioception was recorded before operation and 3 months,6 months,12 months after operation.The comparative analysis was carried out on these data between the group and between involved and un-involved limbs.Results: 114 patients were followed up longer than 12 months, the average followed-up periods was 20.46±5.43 months. The preserving-remnant group had 61 patients and the removing remnant had 53 patients. There were significantly statistical improvement based on knee mobility, Lysholm Scale, IKDC Score and knee stability 6 months and 1 year after surgery(P < 0.01), and there were statistically difference between two groups on Lysholm Scale, IKDC Score and knee stability 6 months after surgery(P > 0.05). The knee proprioception was significantly statistical improvement 3 months,6 months and 1 year after surgery(P<0.01). The proprioception of the preserving-remnant group was better than the removing-remnant group 3 months,6 months after surgery, but there was no statistically difference between two groups 1 year after surgery(P < 0.05). The proprioception of the involved limbs had no difference from the un-involved limbs in the preserving-remnant group 6 months after surgery(P > 0.05). The proprioception of the involved limbs had no difference from the un-involved limbs in the removing remnant group 1 year after surgery(P > 0.05).Conclusion: ACL reconstruction with remnant preservation is beneficial to the recovery of postoperative proprioception and knee function, which is worth clinical popularizing and application.
