Epidermodysplasia verruciformis - associated human papillomaviruses ( EV HPV) are a group of double stranded small DNA viruses, composed mainly of HPV-5, 8, 9, 12, 14, 15, 17, 19-25, 36-38,47, 49, et al. They are epi-dermotrophic viruses. EV HPVs were first found only in the skin lesions of a rare genetic disorder-epidermodysplasia verruciformis (EV). With the development of PCR technique, EV HPVs could be found in normal skin and form la: tent infection. In non - melanoma skin tumors and keratoacanthomas, EV HPVs were found more prevalent than in healthy controls. This led to the thoughts of the possible relationship between EV HPVs and skin diseases besides EV.Recently, some authors found that EV HPV were more prevalent in psoriat-ic lesions than in normal controls. In the serum of patients with psoriasis, antibodies could be detected with the virus like particles of HPV-5 or HPV-8 using enzyme - linked immunosorbent assay ( ELJSA). Because of the possible difference in the distribution of HPVs, we assumed that it might be useful to detect the prevalence of HPV DNA in the skin lesion biopsies and the scrapings of patients with psoriasis in China. We also want to know the possible role of HPVs in the pathogenesis of psoriasis.HPVs have the ability to cause skin tumors. They are believed to help the formation of some premalignant and malignant tumors. Seborrheic keratosis is a benign epidermal tumor. Its etiology is still unclear. Since its clinical and pathological manifestations are somewhat similar to skin warts, its relationship with HPV infection has been studied for a long time. However no definite results have been found because of the limitation of methods. Although some authors reported that they had detected HPV in genital seborrheic keratosis, yet others insisted that genital seborrheic keratosis with the infection of HPVs was actually condylo-ma acuminata. We detected 55 cases of seborrheic keratosis to find the possible infection of HPVs in nongenital seborrheic keratosis.Although follicular keratosis is a genetic disorder, it could have papilloma-tous proliferation and vegetation, resembling the infection of HP Vs. Thus we suspected that patients of follicular keratosis might infect HPVs on the genetic background. We detected the skin lesion biopsies and scrapings of two cases of follicular keratosis and analyzed the possible infection of HPVs. Retinoids have potent ability of antipapilloma, therefore we choose one of the third generation arotinoids-arotinoid ethylester to treat the two cases.In order to increase the detection rate of HPV DNA, we chose three groups of HPV consensus primers pairs- MY09/11; CP65/70 and CP66/69; FAP59/ 64 and a primer pair for b - globulin as the inner control. We detected 62 cases of psoriasis, 55 cases of seborrheic keratosis, two cases of follicular keratosis, and 48 cases of healthy control. We retracted the PCR products of two positive cases in order to get their nucleotide sequences and analyze their HPV types. The results were as follows:1. Among the three groups of consensus primer pairs, MY09/11 was designed for mucosal type HPVs. It only amplified HPV DNA in the positive controls (HPV-16 plasmid and a case of condyloma acuminata) , but it didnt amplify any HPV in the skin lesion biopsies or scrapings of psoriatic patients. Although FAP59/64 could combine with the sciences of at least 65 types of HPVs and amplify them, yet they only got positive results in the positive control of HPV plasmids. Since the number of HPVs might be very small in the samples, so they could not easily be detected by the one - step PCR method. The primer pairs CP65/70 and CP66/69 were proved to be more potent and more sensitive. They were designed for EV HPVs. Besides, the nested PCR could greatly improve both the sensitivity and specificity of detection. There are so many EV HPV types, that choosing suitable primer pairs is very important for the detection work.2.
