The systemical research was made on asthma through three parts of theory inquiry, clinical observation and animal experiment.In the first part, the literature related to asthma in Traditional Chinese Medicine(TCM)and the recognition of asthmatic etiology and pathogenesis were overviewed, generalized and summarized. For present worse situation of asthma, on one hand, we emphasized the pathogenesis of phlegm-stasis coexisting, recommended the therapeutic rules of eliminating phlegm and removing blood stasis. On the other hand, according to the latest immunol study, asthma is a kind of disease with the immunol disfunction of whole body. Therefore it can remedy the defect of inhaling corticosteroid which was recommended by GINA to inhibit airway inflammation by regulating the overall immunol function. TCM and herb showed advantages and great potential in both blood-activating and overall immunol regulation. The prescription of Pingxiao-mixture was formed on the base of above theories, and it has good effects on asthma in clinical practice. To explore the therapeutic mechanism of Pingxiao-mixture befits us to expend minds to find better therapeutic approach, and can provide a scientific explanation for effectiveness of TCM.In the second part, the clinical effects of Pingxiao-mixture were observed and its mechanism was explored. 74 asthmatic patients in moderate acute phase were randomly divided into two groups:34 patients were treated with Pingxiao-mixture and aminophylline as the treatment group, and 40 patients were theated with budesonide(BUD) andaminophylline as the control group. The improvements of main symptoms and signs of all patients were ovserved ,and the level of IgE and eosinophil in peripheral blood were measured. In the treatment group, plumonary ventilational function(FEV1 PEF FEV1/FVC),the function of small airway(V50 V25,MMEF), airway resistance(Zrs R5,R20),the metabolism indexes of oxygen free radical(SOD LPO), inflammatory mediators such as NO,SIL-2R, T lymphcyte subgroups (CD4+ CD8+ )and blood rheology were measured respectively before and after treatment. The results showed the comprehensive therapeutic effect of the treatment group was superior to that of the control group(P<0.05). After treatment the decreasement of IgE and eosinophil were obvious in these two groups, and there weren't significantly difference between them. In the treatment group, after treatment the pulmonary function and blood rheology were significantly improved, the contents of NO and SIL-2R markedly lowered, the disorder state of immune and oxygen free radical metabolism were obviously adjusted. The clinical research proves that Pingxiao-mixture is effective complex prescription for asthma, which is valuable for further reasearch and utilizing.It's mechanism lies in antagonizing inflammatory mediators, improving microcirculation, regulating overall immunol function and oxygen free radical metabolism, et al.In the third part, by means of modern experimental method,the influence of Pingxiao-mixture on airway inflammatory cellar infiltration was observed and its molecular mechanism was further explored. The asthma models of guinea pigs were established by ovalbumin sensitization. After one week for treatment,the incubation period of asthma wasrecorded .Then the pathologic histology of lung was observed and bronchoalveolar lavage(BAL) was performed to examine the total count and cytological classification. The apoptotic index of T lymphcytes isolated from BALF was analyzed by flow cytometry.The results showed Pingxiao-mixture could obviously prolong the incubation period of asthma, relieve airway inflammation, promote the apoptosis of T lymphcytes in airway, which effects were superior to that of Ding-chuan-zhi-ke-pian group(P<0.05),but there was no significant difference between Pingxiao-mixture group and dexamethasone group .The extent of aiway infiltration was related to the apoptotic indexes of T lymphocytes,so it is concluded that the main mechanism of anti-inflammation...
