Part 1: Animal Trials of Interventional Alcoholic Therapy in EndometriomsObjective: To find out the effects of absolute alcohol kept in endometrial cavities of white rabbits and rats for different durations on the endometrium in vivo, the effects of alcohol diluted into different percentage on the endometrium in vivo through analyzing trials on animals. To evaluate the coagulation effects of absolute alcohol kept in the cavity of induced endometriomas on the ectopic endometria using rat model and to find out the optimal duration of destroying the endometriomas, the effects of alcohol diluted into different percentage on the endometriomas. Samples of the endometrium in different places of different phase in rat models were taken out to study the expressions of vascular endothelial growth factor (VEGF). Materials and methods: ? female white rabbits have 14 uteri. Absolute alcohol was aseptically injected into the endometrial cavities of 10 uteri and kept for 30 sec, 1 min, 1 min 30 sec, 2 min, 3 min, 5 min (2 uteri) , lOmin, 15min and 30min respectively, 80% and 70% alcohol injected into 1 uterus respectively for 3min, 50% alcohol into 1 uterus for 2 min 30 sec. The left one uterus was served as control. The rabbits were killed in 4 or 5 days, or in 1 or 2 weeks, examined for the condition of the 2 uteri, and then taken out. The sections stained by HE then examined for any pathological changes microscopically. (2)50 female rats having had 90 artificially inducedendometriomas. 56 of these endometriomas were given absolute alcohol(22), 90% alcohol (9), 80% alcohol(10), 75% alcohol(15) and preserved for 1, 2, 3 or 5 min respectively. 47 normal uteri were given absolute alcohol(19), 90% alcohol (9), 80% alcohol(10), 75% alcohol(9) and preserved for 1, 2, 3 or 5 min respectively. 34 surgically induced endometriomas and 3 normal uteri were kept as control. The rats were bred for another 2, 4 or 6 weeks before performing laparotomy. The status of the induced endometrioma, the uterus and the abdominal cavity were examined. Samples were obtained, sectioned, stained by HE and observed for pathological changes microscopically. (3)We took 14 pieces of rat endometrium from the remaining of establishing endometrial models, 18 pieces of endometrial wall induced in 3, 5, 7, and 9 weeks respectively, 6 pieces of uterus wall from the control, 10 pieces of induced endometrial wall treated with alcohol and 12 pieces of uterus wall treated with alcohol. The samples were sectioned and stained with Streptavidin/Peroxidase immunohistochemical method to reveal the vascular endothelial growth factor (VEGF) in it. Expressions of the VEGF were observed microscopically.Results: (1) Absolute alcohol preserved in the white rabbits endometrial cavity resulted in coagulated necrosis of endometrium in vivo. Complete necrosis of local endometrium was observed when absolute alcohol kept for 3 and 5 min while complete necrosis of entire endometrium was observed when absolute alcohol kept for 10 min or more. Alcohol with percentage lower than 80% kept for less than 3 min did not result in coagulated necrosis on endometrium in vivo. (2) Absolute alcohol injected into the endometriomas can eliminate it and thus cure the endometriomas. The eliminate rate of the endometriomas was 86.4% with absolute alcohol, 88.9% with 90% alcohol, 80% with 80% alcohol and 60% with 75% alcohol respectively. The eliminate rates of trialgroups were significantly higher than that of the control group (7.4%) but there were no significant differences between the trial groups yet. The endometria of endometriomas disappeared completely when absolute alcohol preserved for 1 min and 90% alcohol preserved for 3 min. The remaining endometriomas treated with 75% alcohol have an endometrial livability of 66.7%. The endometria of uterus showed partially necrosis when absolute alcohol was remained for 1 min and 90% alcohol was remained for 3 min and it showed complete coagulated necrosis after absolute alcohol was remained for 2 min and 90% alcohol was remaine...
